A whole siman held up by a single sentence. It opens with two prayers that arrive too late, draws a rule from them — pray for the future, give thanks for the past — and then puts it to work four times: at a city gate, beside a threshing floor, at the door of a bathhouse, under the bleeder’s lancet. One rule, five times over. What is not prayed for? — what is already settled: the cry already raised, the child already formed, the heap already measured. What is prayed for? — the threshold not yet crossed: the city one is entering, the bath one is going down into, the remedy not yet taken. And the past? — it awaits no request but a word of thanks; and at the end of the siman, one sentence to repeat always: כל מה דעביד רחמנא לטב עביד. We study here at the level of principle; for personal conduct, follow the ruling of your Rav.

A. תפלת שוא — the prayer with nothing left to take hold of

תְּפִלַּת שָׁוְא — “a vain prayer”. The Mechaber gives no definition: he gives two cases and lets the reader draw the rule. The first is a man who enters a city, hears a cry, and asks that the cry not come from his own house; the second, a husband who asks for a son when the child is already formed. In both the request comes too late: what it asks for is already settled. « המתפלל על מה שעבר כגון שנכנס לעיר ושמע קול צוחה בעיר ואמר יהי רצון שלא יהא קול זה בתוך ביתי » (שו״ע או״ח ר״ל:א) And the mishnah said exactly this: « הַצּוֹעֵק לְשֶׁעָבַר, הֲרֵי זוֹ תְּפִלַּת שָׁוְא. כֵּיצַד. הָיְתָה אִשְׁתּוֹ מְעֻבֶּרֶת, וְאָמַר, יְהִי רָצוֹן שֶׁתֵּלֵד אִשְׁתִּי זָכָר, הֲרֵי זוֹ תְּפִלַּת שָׁוְא. הָיָה בָא בַדֶּרֶךְ וְשָׁמַע קוֹל צְוָחָה בָּעִיר, וְאָמַר יְהִי רָצוֹן שֶׁלֹּא יִהְיוּ אֵלּוּ בְּנֵי בֵיתִי » (משנה ברכות פרק ט משנה ג)

Seif 1: one who prays over what is past — for instance he entered a city and heard a cry there, and said “may it be Your will that this voice not be in my house”; or his wife was pregnant, after forty days of pregnancy, and he said “may it be Your will that my wife bear a male” — that is a vain prayer. Rather, let a man pray for what is to come and give thanks for what has passed. Thus one who enters a walled city says: “may it be Your will, Hashem our God, to bring me into this city in peace”; having entered in peace he says: “I give thanks before You, Hashem our God, that You brought me into this city in peace”; when he asks to leave he says: “may it be Your will, Hashem our God, to bring me out of this city in peace”; having gone out in peace he says: “I give thanks before You, Hashem my God, that You brought me out of this city in peace, and just as You brought me out in peace, so lead me in peace”, and so on, until “Blessed are You Hashem, who hears prayer”. And this is the prayer of the road, which has been written, it and all its laws, in siman 110.
The cry in the city. The case is a man who has not yet learned anything: he hears, he does not know. His request seems to bear on the unknown — but the unknown is not the future. « המתפלל על מה שעבר כגון שנכנס לעיר ושמע קול צוחה בעיר ואמר יהי רצון שלא יהא קול זה בתוך ביתי » (שו״ע או״ח ר״ל:א) The mishnah makes it the first example of the category: « הָיָה בָּא בַּדֶּרֶךְ וְשָׁמַע קוֹל צְוָחָה בָּעִיר, וְאוֹמֵר: ״יְהִי רָצוֹן שֶׁלֹּא תְּהֵא בְּתוֹךְ בֵּיתִי״ — הֲרֵי זוֹ תְּפִלַּת שָׁוְא » (ברכות נ״ד ע״א) And the Aruch HaShulchan states the rule in a sentence: « דבר מובן בשכל שעניין התפלה הוא רק על העתיד ולא על העבר, דמה יועיל על העבר, ועל העבר לא שייך רק לשון הודאה, לתת תודה לו יתברך על הטוב שעשה לו » (ערוך השלחן או״ח סימן ר״ל) summary: my ignorance does not make a thing future; the cry already had its cause, and there is nothing left to ask for.
What stays open: trust. Where request has no purchase, another kind of speech remains — Hillel’s: « תָּנוּ רַבָּנַן: מַעֲשֶׂה בְּהִלֵּל הַזָּקֵן שֶׁהָיָה בָּא בַּדֶּרֶךְ וְשָׁמַע קוֹל צְוָחָה בָּעִיר, אָמַר: מוּבְטָח אֲנִי שֶׁאֵין זֶה בְּתוֹךְ בֵּיתִי. וְעָלָיו הַכָּתוּב אוֹמֵר: ״מִשְּׁמוּעָה רָעָה לֹא יִירָא נָכוֹן לִבּוֹ בָּטֻחַ בַּה׳״ » (ברכות ס׳ ע״א) The Aruch HaShulchan brings it as the conduct of the righteous: « אבל יכול לומר: ׳מובטחני שאין זה בתוך ביתי׳ אם הוא צדיק גמור, כמעשה דהלל הזקן שאמר כן » (ערוך השלחן או״ח סימן ר״ל) and gives it a second, wholly domestic reading: « ולכן הנכנס לעיר ושמע קול צווחה בעיר מאיזה צרה שנתהוה בה, ואומר: ׳יהי רצון שלא תהיה זה בתוך ביתי׳ - הרי זו תפלת שוא, דזהו תפלה על העבר ומאי דהוה הוה » (ערוך השלחן או״ח סימן ר״ל) summary: “I pray that it not be in my house” is vain; “I am confident that it is not in my house” is not — it is no longer a request but a state of soul.
The forty days. The seif does not say one never prays for a boy: it sets a boundary. « או שהיתה אשתו מעוברת אחר מ׳ יום לעיבורה ואמר יהי רצון שתלד אשתי זכר ה״ז תפלת שוא » (שו״ע או״ח ר״ל:א) The Tur writes it from both sides: « ודוקא אחר ארבעים יום לעיבורה אבל תוך ארבעים יום מועלת תפלתו » (טור או״ח סימן ר״ל) The Mishnah Berurah gives the reason: « שכבר נצטייר צורת הולד אבל תוך מ״ם יום מועיל תפלה » (משנה ברורה, מ״ב ר״ל:א) And the Gemara had already divided pregnancy into stages of prayer: « שְׁלֹשָׁה יָמִים הָרִאשׁוֹנִים — יְבַקֵּשׁ אָדָם רַחֲמִים שֶׁלֹּא יַסְרִיחַ. מִשְּׁלֹשָׁה וְעַד אַרְבָּעִים יְבַקֵּשׁ רַחֲמִים שֶׁיְּהֵא זָכָר. מֵאַרְבָּעִים יוֹם וְעַד שְׁלֹשָׁה חֳדָשִׁים — יְבַקֵּשׁ רַחֲמִים שֶׁלֹּא יְהֵא סַנְדָּל » (ברכות ס׳ ע״א) summary: before forty days the form is not yet taken and the prayer holds; after them it would bear on an accomplished fact.
What is still left to ask, even afterwards. The Mishnah Berurah does not leave the father without prayer: « ואפילו אחר מ״ם יום יוכל להתפלל שיהיה הולד זרע קיימא ויהיה עוסק במצות ומע״ט » (משנה ברורה, מ״ב ר״ל:א) The Eliyah Rabbah had written so in the name of the Solet Belula: « כתב בסולת בלולה דאפילו אחר ארבעים יום יוכל להתפלל שיהא הולד הזה זרע חייא וקיימא עוסק בתורה ומצוות ומעשים טובים » (אליה רבה או״ח סימן ר״ל) summary: the sex is fixed, the life and uprightness of the child are not — and there prayer finds its object again.
The rule, at last stated. After the two counter-examples, one line carries the whole siman: « אלא יתפלל אדם על העתיד לבא ויתן הודאה על שעבר » (שו״ע או״ח ר״ל:א) The Rambam places it as the conclusion of his whole chapter: « כְּלָלוֹ שֶׁל דָּבָר לְעוֹלָם יִצְעַק אָדָם עַל הֶעָתִיד לָבוֹא וִיבַקֵּשׁ רַחֲמִים וְיִתֵּן הוֹדָיָה עַל מַה שֶּׁעָבַר וְיוֹדֶה וִישַׁבֵּחַ כְּפִי כֹּחוֹ. וְכָל הַמַּרְבֶּה לְהוֹדוֹת אֶת יְיָ׳ וּלְשַׁבְּחוֹ תָּמִיד הֲרֵי זֶה מְשֻׁבָּח » (רמב״ם הלכות ברכות פרק י הלכה כו) summary: two verbs and two tenses — יתפלל for the future, ויתן הודאה for the past; the four seifim that follow are only applications.

B. הנכנס לכרך — the four formulas

כְּרַךְ — not any town, but the great walled city, where a man may be seized and no one knows him. There the Mechaber stages his rule: two requests and two thanksgivings, alternating, on entering and on leaving.
The four formulas. They read two by two, and each thanksgiving answers word for word the request that precedes it. « כגון הנכנס לכרך אומר יהי רצון מלפניך ה׳ אלהינו שתכניסני לכרך הזה לשלום » (שו״ע או״ח ר״ל:א) « נכנס בשלום אומר מודה אני לפניך ה׳ אלהינו שהכנסתני לכרך הזה בשלום » (שו״ע או״ח ר״ל:א) « בקש לצאת אומר יהי רצון מלפניך ה׳ אלהינו שתוציאני מכרך זה לשלום » (שו״ע או״ח ר״ל:א) « יצא בשלום אומר מודה אני לפניך ה׳ אלהי שהוצאתני מכרך זה לשלום וכשם שהוצאתני לשלום כן תוליכני לשלום וכו׳ עד בא״י שומע תפלה » (שו״ע או״ח ר״ל:א) The baraita gives them in full: « תָּנוּ רַבָּנַן: בִּכְנִיסָתוֹ מַהוּ אוֹמֵר? — ״יְהִי רָצוֹן מִלְּפָנֶיךָ ה׳ אֱלֹהַי שֶׁתַּכְנִיסֵנִי לִכְרַךְ זֶה לְשָׁלוֹם״ » (ברכות ס׳ ע״א) « יָצָא, אוֹמֵר: ״מוֹדֶה אֲנִי לְפָנֶיךָ ה׳ אֱלֹהַי שֶׁהוֹצֵאתַנִי מִכְּרַךְ זֶה לְשָׁלוֹם. וּכְשֵׁם שֶׁהוֹצֵאתַנִי לְשָׁלוֹם, כָּךְ תּוֹלִיכֵנִי לְשָׁלוֹם, וְתִסְמְכֵנִי לְשָׁלוֹם, וְתַצְעִידֵנִי לְשָׁלוֹם, וְתַצִּילֵנִי מִכַּף כׇּל אוֹיֵב וְאוֹרֵב בַּדֶּרֶך » (ברכות ס׳ ע״א) summary: four utterances for two thresholds — and the fourth spills past the threshold onto the road.
Why the walled city and not a small town. The Gemara gives two versions of the criterion: « אָמַר רַב מַתְנָא: לֹא שָׁנוּ אֶלָּא בִּכְרַךְ שֶׁאֵין דָּנִין וְהוֹרְגִין בּוֹ, אֲבָל בִּכְרַךְ שֶׁדָּנִין וְהוֹרְגִין בּוֹ — לֵית לַן בַּהּ. » (ברכות ס׳ ע״א) « אִיכָּא דְּאָמְרִי, אָמַר רַב מַתְנָא: אֲפִילּוּ בִּכְרַךְ שֶׁדָּנִין וְהוֹרְגִין בּוֹ, זִימְנִין דְּלָא מִתְרְמֵי לֵיהּ אִינָשׁ דְּיָלֵיף לֵיהּ זְכוּתָא. » (ברכות ס׳ ע״א) The Magen Avraham cites Rashi: « שם מצוים מושלים רעים ומחפשין עלילות (רש״י) » (מגן אברהם או״ח סימן ר״ל) The Mishnah Berurah rules for practice: « דוקא לכרך דשם מצויים ממונים רעים ומחפשים עלילות אבל לעיר קטנה א״צ לומר כשנכנס לה ולא אח״כ כשיצא ממנה » (משנה ברורה, מ״ב ר״ל:ב) summary: the danger here is not that of the road but that of arbitrary power — a city where they judge and execute, and where a stranger has no one to plead for him.
And the road? The seif closes by pointing elsewhere: « וזו היא תפלת הדרך שנכתבה היא וכל דיניה בסימן ק״י » (שו״ע או״ח ר״ל:א) The Mishnah Berurah explains when one merges into the other: « והכא מיירי בנכנס לכרך ולן שם בענין שצ״ל תה״ד שנית בבוקר בשביל הדרך שרוצה לנסוע » (משנה ברורה, מ״ב ר״ל:ג) « בזה אמר שיעשה משניהם אחד ויחתום בה » (משנה ברורה, מ״ב ר״ל:ג) and recalls, for a small town: « אלא אח״כ כשמחזיק בדרך אומר תפלת הדרך משום סכנת דרכים וכמבואר לעיל בסימן ק״י » (משנה ברורה, מ״ב ר״ל:ב) summary: leaving the walled city and setting out on the road may fall on the same morning; one then makes a single prayer of the two, sealed with שומע תפלה. C. הגורן — blessing before measuring

דָּבָר הַסָּמוּי מִן הָעַיִן — “a thing hidden from the eye”. Seif 2 carries the rule of seif 1 into a field: the blessing is said over the heap before it is measured; once counted it is past, and there is nothing left to ask for it.

Seif 2 — הנכנס למוד את גרנו

הנכנס למוד את גרנו אומר יהי רצון מלפניך ה׳ אלהי שתשלח ברכה בכרי הזה התחיל למוד אומר ברוך השולח ברכה בכרי הזה מדד ואח״כ בירך ה״ז תפלת שוא שאין הברכה מצויה אלא בדבר הסמוי (פי׳ הנעלם ואינו נראה) מן העין:
Seif 2: one who goes in to measure his threshing floor says: “may it be Your will, Hashem my God, to send blessing into this heap”. He has begun to measure: he says “blessed is He who sends blessing into this heap”. If he measured and only then blessed, that is a vain prayer — for blessing is found only in a thing hidden (that is, concealed and not seen) from the eye.
Two utterances, not one. The seif carefully distinguishes their form: the first is a request, the second a blessing. « הנכנס למוד את גרנו אומר יהי רצון מלפניך ה׳ אלהי שתשלח ברכה בכרי הזה » (שו״ע או״ח ר״ל:ב) « התחיל למוד אומר ברוך השולח ברכה בכרי הזה » (שו״ע או״ח ר״ל:ב) The Eliyah Rabbah explains why the second is already in the past tense: « השולח רצה לומר אשר שלח כמו המוציא לחם, כי בוטח הוא בשם שכבר שלח בו ברכה כיון שהתחיל למוד כדי לעשר » (אליה רבה או״ח סימן ר״ל) summary: השולח does not mean “who will send” but “who has sent” — once the measuring begins, the man holds it settled that the blessing is there.
The principle and its source. « מדד ואח״כ בירך ה״ז תפלת שוא שאין הברכה מצויה אלא בדבר הסמוי (פי׳ הנעלם ואינו נראה) מן העין » (שו״ע או״ח ר״ל:ב) The Beit Yosef points to the two places it comes from: « הנכנס למוד גרנו וכו׳ עד אלא בדבר הסמוי מן העין » (בית יוסף או״ח סימן ר״ל) Rabbi Yitzchak states it: « וְאָמַר רַבִּי יִצְחָק: אֵין הַבְּרָכָה מְצוּיָה אֶלָּא בְּדָבָר הַסָּמוּי מִן הָעַיִן, שֶׁנֶּאֱמַר: ״יְצַו ה׳ אִתְּךָ אֶת הַבְּרָכָה בַּאֲסָמֶיך » (תענית ח׳ ע״ב) and the school of Rabbi Yishmael says it otherwise: « תָּנָא דְּבֵי רַבִּי יִשְׁמָעֵאל: אֵין הַבְּרָכָה מְצוּיָה אֶלָּא בְּדָבָר שֶׁאֵין הָעַיִן שׁוֹלֶטֶת בּוֹ, שֶׁנֶּאֱמַר: ״יְצַו ה׳ אִתְּךָ אֶת הַבְּרָכָה בַּאֲסָמֶיך » (תענית ח׳ ע״ב) summary: one verse, two readings — what is stored away and what the eye does not govern; either way, counting shuts the door.
The baraita, word for word. « תָּנוּ רַבָּנַן: הַנִּכְנָס לָמוֹד אֶת גׇּרְנוֹ, אוֹמֵר: ״יְהִי רָצוֹן מִלְּפָנֶיךָ ה׳ אֱלֹהֵינוּ שֶׁתִּשְׁלַח בְּרָכָה בְּמַעֲשֵׂה יָדֵנוּ״. הִתְחִיל לָמוֹד, אוֹמֵר: ״בָּרוּךְ הַשּׁוֹלֵחַ בְּרָכָה בַּכְּרִי הַזֶּה״ » (תענית ח׳ ע״ב) « מָדַד וְאַחַר כָּךְ בֵּירַךְ — הֲרֵי זוֹ תְּפִלַּת שָׁוְא, לְפִי שֶׁאֵין הַבְּרָכָה מְצוּיָה לֹא בְּדָבָר הַשָּׁקוּל, וְלֹא בְּדָבָר הַמָּדוּד, וְלֹא בְּדָבָר הַמָּנוּי, אֶלָּא בְּדָבָר הַסָּמוּי מִן הָעַיִן » (תענית ח׳ ע״ב) The Aruch HaShulchan gives the reason: « שאין הברכה מצויה אלא בדבר הנעלם מן העין כדי שלא יהא נראה להדיא כנגד הטבע ממש, דרוב נסים הם נסתרים » (ערוך השלחן או״ח סימן ר״ל) summary: blessing hides not out of modesty but by nature — a miracle that lets itself be measured is no longer a hidden miracle.
With or without the Name. The formula opens with ברוך, but the Mishnah Berurah holds it back: « וכתב הא״ר דיאמר בלא הזכרת השם דדוקא במודד כדי לעשר מצינו שהבטיח הקב״ה ברכה עד בלי די » (משנה ברורה, מ״ב ר״ל:ד) summary: the promise “beyond sufficiency” was made to one measuring in order to tithe; outside that case one says the formula without the Name. D. המרחץ וההקזה — the bathhouse and the bloodletting

Two seifim, one gesture: one enters somewhere risky, asks to come out safe, and when he has come out, says so. Their bathhouse was not ours — the fire burned in a pit beneath the floor.

Seif 3 — הנכנס למרחץ

הנכנס למרחץ אומר יהר״מ ה׳ אלהי שתכניסני לשלום ותוציאני לשלום ותצילני מהאור הזה וכיוצא בו לעתיד לבא יצא בשלום אומר מודה אני לפניך ה׳ אלהי שהצלתני מהאור הזה:
Seif 3: one who enters the bathhouse says: “may it be Your will, Hashem my God, to bring me in in peace and bring me out in peace, and to save me from this fire and its like, for the time to come”. If he came out in peace he says: “I give thanks before You, Hashem my God, that You saved me from this fire”.
The Mechaber’s wording is not the Gemara’s. The baraita gave a far darker formula: « תָּנוּ רַבָּנַן: הַנִּכְנָס לְבֵית הַמֶּרְחָץ, אוֹמֵר: ״יְהִי רָצוֹן מִלְּפָנֶיךָ ה׳ אֱלֹהַי שֶׁתַּצִּילֵנִי מִזֶּה וְכַיּוֹצֵא בּוֹ, וְאַל יֶאֱרַע בִּי דְּבַר קַלְקָלָה וְעָוֹן » (ברכות ס׳ ע״א) And Abaye set it aside: « אָמַר אַבָּיֵי: לָא לֵימָא אִינָשׁ הָכִי, דְּלָא לִפְתַּח פּוּמֵּיהּ לְשָׂטָן » (ברכות ס׳ ע״א) The Beit Yosef explains where the Tur’s wording then comes from: « ואע״פ שבגמרא לא אמרו אלא י״ר שתצילני מזה ומכיוצא בו הרי״ף והרמב״ם והרא״ש כתבו כמ״ש רבינו ואפשר שכך היתה גירסתם בגמרא » (בית יוסף או״ח סימן ר״ל) summary: what we say comes from the Rif, the Rambam and the Rosh — the formula that opens the mouth to the Satan was left aside.
Coming out. « יצא בשלום אומר מודה אני לפניך ה׳ אלהי שהצלתני מהאור הזה » (שו״ע או״ח ר״ל:ג) The Gemara says it in the name of Rav Acha: « כִּי נָפֵיק מַאי אוֹמֵר? אָמַר רַב אַחָא: ״מוֹדֶה אֲנִי לְפָנֶיךָ ה׳ שֶׁהִצַּלְתַּנִי מִן הָאוּר״. » (ברכות ס׳ ע״א) and immediately tells the story that gives the prayer its point: « רַבִּי אֲבָהוּ עָל לְבֵי בָנֵי. אִפְּחִית בֵּי בָנֵי מִתּוּתֵיהּ, אִתְרְחִישׁ לֵיהּ נִיסָּא קָם עַל עַמּוּדָא, שֵׁזִיב מְאָה וְחַד גַּבְרֵי בְּחַד אֵבְרֵיה » (ברכות ס׳ ע״א) summary: Rabbi Abbahu’s story is no anecdote — it is the bathhouse floor giving way beneath him, precisely what the prayer asked to be spared.
And today? The Mishnah Berurah notes that the practice has lapsed, and says why: « ועכשיו לא נהגו בתפלה זו והטעם דלא ניתקן אלא בימיהם שהיו מסיקין האש מלמטה בחפירה ורוחצים באמבטי מלמעלה ע״ג תקרה שע״ג ויש חשש פן תפחת ויפלו לתוכה » (משנה ברורה, מ״ב ר״ל:ה) « אבל בזה״ז עומד התנור בזוית וליכא סכנה » (משנה ברורה, מ״ב ר״ל:ה) The Taz had written it before her: « עכשיו לא נהגו לומר כן ונתן מו״ח ז״ל טעם דלא נתקן אלא בימיהם שהיו מסיקין האש מלמטה בחפירה ורוחצים באמבטי למעל » (ט״ז או״ח סימן ר״ל) « אבל בזמן הזה התנור עומד בזוית ליכ׳ סכנה » (ט״ז או״ח סימן ר״ל) The Baer Hetev sums it up: « ובמרחץ דידן א״צ להתפלל תפלה זו. ב״ח מ״א ט״ז » (באר היטב או״ח סימן ר״ל) summary: the prayer was born of one precise danger; the furnace having moved, the danger went and the practice with it. Seif 4 — הנכנס להקיז דם

הנכנס להקיז דם אומר יהר״מ ה׳ אלהי שיהא עסק זה לי לרפואה כי רופא חנם אתה ולאחר שהקיז יאמר ברוך רופא חולים:
Seif 4: one who goes in to be bled says: “may it be Your will, Hashem my God, that this business be a healing for me, for You are a free healer”. And after he has been bled let him say: “blessed is the Healer of the sick”.
The same architecture, a third time. « הנכנס להקיז דם אומר יהר״מ ה׳ אלהי שיהא עסק זה לי לרפואה כי רופא חנם אתה » (שו״ע או״ח ר״ל:ד) « ולאחר שהקיז יאמר ברוך רופא חולים » (שו״ע או״ח ר״ל:ד) The Gemara gives the full formula: « דְּאָמַר רַב אַחָא: הַנִּכְנָס לְהַקִּיז דָּם, אוֹמֵר: ״יְהִי רָצוֹן מִלְּפָנֶיךָ ה׳ אֱלֹהַי שֶׁיְּהֵא עֵסֶק זֶה לִי לִרְפוּאָה, וּתְרַפְּאֵנִי. כִּי אֵל רוֹפֵא נֶאֱמָן אָתָּה וּרְפוּאָתְךָ אֱמֶת, לְפִי שֶׁאֵין דַּרְכָּן שֶׁל בְּנֵי אָדָם לְרַפּאוֹת אֶלָּא שֶׁנָּהֲגו » (ברכות ס׳ ע״א) and Abaye again cuts off its end: « אָמַר אַבָּיֵי: לָא לֵימָא אִינָשׁ הָכִי, דְּתָנֵי דְּבֵי רַבִּי יִשְׁמָעֵאל: ״וְרַפֹּא יְרַפֵּא״ — מִכָּאן שֶׁנִּיתְּנָה רְשׁוּת לָרוֹפֵא לְרַפּאוֹת » (ברכות ס׳ ע״א) summary: “men are not in the habit of healing” was set aside; the Torah gave the physician leave to heal, and the prayer does not deny it.
Not only bloodletting. « וה״ה בכל מידי דרפואה יאמר זה ולא יחשוב שתהיה איזה דבר לו רפואה אלא ע״י הבורא ית״ש » (משנה ברורה, מ״ב ר״ל:ו) « ולכן ע״י תפלה זו ישים בטחונו בו ויבקש ממנו שתהיה לו לרפואה » (משנה ברורה, מ״ב ר״ל:ו) The Magen Avraham had written it: « נ״ל דה״ה בכל מידי דרפואה יאמר זה » (מגן אברהם או״ח סימן ר״ל) The Eliyah Rabbah brings the contrary view: « והוא הדין בכל מיני רפואה, מגן אברהם » (אליה רבה או״ח סימן ר״ל) and so does the Baer Hetev: « אבל ברד״א משמע דוקא הקזה שהיא סכנה שמא יחתוך ורידי הדם וימות ח״ו » (באר היטב או״ח סימן ר״ל) The Aruch HaShulchan describes the practice of the scrupulous: « ונכון שכל רפואה שנוטל יאמר: ״יהי רצון מלפניך ד׳ אלקי שתהא זה לי לרפואה״, וכן נוהגין המדקדקים » (ערוך השלחן או״ח סימן ר״ל) summary: for some, bloodletting is only an example and every medicine falls under the rule; for Rabbi David Abudraham, only bloodletting, because it is a danger. ברוך רופא חולים — with Name and Kingship? The Beit Yosef: « וכ׳ בסמ״ג בא״י רופא חולים וכ״כ הרמב״ם » (בית יוסף או״ח סימן ר״ל) « ורבינו שלא הזכיר לא שם ולא מלכות נראה שסמך על מ״ש בסימן רי״ח שבכל הברכות צריך להזכיר שם ומלכות » (בית יוסף או״ח סימן ר״ל) The Taz: « פשיטא שצריך שם ומלכות כמו כל הברכו׳ שאמרו לעיל דהכלל בהם כן » (ט״ז או״ח סימן ר״ל) The Baer Hetev: « ופשוט שצריך שם ומלכות כמו כל הברכות » (באר היטב או״ח סימן ר״ל) But the Mishnah Berurah describes what she saw: « וצריך לומר בשם ומלכות [ב״י וט״ז] אבל לא ראיתי נוהגין כן » (משנה ברורה, מ״ב ר״ל:ז) summary: in law a blessing is said with Name and Kingship; in fact the Mishnah Berurah did not see it done. And sneezing. The Mishnah Berurah adds in the same place a custom that has nothing to do with bloodletting — and yet does: « מי שמתעטש וחבירו אומר לו אסותא יאמר לו ברוך תהיה ואח״כ יאמר לישועתך קויתי ה׳ » (משנה ברורה, מ״ב ר״ל:ז) The Magen Avraham gives the reason: « מי שמתעטש וחבירו א״ל אסותא יאמר לו ברוך תהיה ואח״כ יאמר לישועתך קויתי ה׳ דהמתפלל על חבירו הוא נענ׳ תחלה » (מגן אברהם או״ח סימן ר״ל) summary: here too one does not pray over the sneeze that is past but over the health to come, and one who prays for his fellow is answered first.

E. כל מה דעביד רחמנא לטב עביד — the last word

The siman ends on a sentence that is neither a blessing nor a prayer: a habit of speech. Having four times shut the door on requests about the past, the Mechaber finally says what is to be done with the past.

Seif 5 — לעולם יהא אדם רגיל לומר

לעולם יהא אדם רגיל לומר כל מה דעביד רחמנא לטב עביד:
Seif 5: let a man always be accustomed to say: whatever the Merciful One does, He does for good.
The source, and the name that carries it. « אָמַר רַב הוּנָא אָמַר רַב מִשּׁוּם רַבִּי מֵאִיר, וְכֵן תָּנָא מִשְּׁמֵיהּ דְּרַבִּי עֲקִיבָא: לְעוֹלָם יְהֵא אָדָם רָגִיל לוֹמַר: ״כׇּל דְּעָבֵיד רַחֲמָנָא לְטָב עָבֵיד״. » (ברכות ס׳ ע״ב) summary: it is a baraita in the name of Rabbi Meir and Rabbi Akiva — and it is Rabbi Akiva’s story that carries it.
The story. « כִּי הָא דְּרַבִּי עֲקִיבָא דַּהֲוָה קָאָזֵיל בְּאוֹרְחָא. מְטָא לְהַהִיא מָתָא, בְּעָא אוּשְׁפִּיזָא לָא יָהֲבִי לֵיהּ. אֲמַר: ״כׇּל דְּעָבֵיד רַחֲמָנָא — לְטָב״ » (ברכות ס׳ ע״ב) Then, in the night: « אֲתָא זִיקָא כַּבְיֵיהּ לִשְׁרָגָא. אֲתָא שׁוּנָּרָא אַכְלֵיהּ לְתַרְנְגוֹלָא. אֲתָא אַרְיָה אַכְלֵיהּ לַחֲמָרָא » (ברכות ס׳ ע״ב) And by morning: « בֵּיהּ בְּלֵילְיָא אֲתָא גְּיָיסָא, שַׁבְיַיהּ לְמָתָא » (ברכות ס׳ ע״ב) summary: over each of the three losses — the lamp, the rooster, the donkey — Rabbi Akiva repeated the same sentence; and that very night the raiders carried off the town, while he, left outside and without a light, was not taken.
What seif 5 does to the whole siman. The Aruch HaShulchan closes the siman on the same idea: « דבר מובן בשכל שעניין התפלה הוא רק על העתיד ולא על העבר, דמה יועיל על העבר, ועל העבר לא שייך רק לשון הודאה, לתת תודה לו יתברך על הטוב שעשה לו » (ערוך השלחן או״ח סימן ר״ל) and the Rambam had set it down as a conclusion: « כְּלָלוֹ שֶׁל דָּבָר לְעוֹלָם יִצְעַק אָדָם עַל הֶעָתִיד לָבוֹא וִיבַקֵּשׁ רַחֲמִים וְיִתֵּן הוֹדָיָה עַל מַה שֶּׁעָבַר וְיוֹדֶה וִישַׁבֵּחַ כְּפִי כֹּחוֹ. וְכָל הַמַּרְבֶּה לְהוֹדוֹת אֶת יְיָ׳ וּלְשַׁבְּחוֹ תָּמִיד הֲרֵי זֶה מְשֻׁבָּח » (רמב״ם הלכות ברכות פרק י הלכה כו) summary: the siman does not only say when to pray; it says what the past becomes once one stops demanding it otherwise — it becomes matter for thanksgiving.
The siman in one sentence: אלא יתפלל אדם על העתיד לבא ויתן הודאה על שעבר — and all the rest, the city, the threshing floor, the bathhouse, the bloodletting, is only that sentence tested at four thresholds.

Practical cases

Case 1 — The result is already decided

Situation: the exam is over, the paper is marked, I do not yet know the grade. May I pray that it be good?
Conduct: this is exactly the case of the cry heard in the city — « המתפלל על מה שעבר כגון שנכנס לעיר ושמע קול צוחה בעיר ואמר יהי רצון שלא יהא קול זה בתוך ביתי » (שו״ע או״ח ר״ל:א) and the rule that follows: « אלא יתפלל אדם על העתיד לבא ויתן הודאה על שעבר » (שו״ע או״ח ר״ל:א) What stays open is what the Mishnah Berurah opened for the father: « ואפילו אחר מ״ם יום יוכל להתפלל שיהיה הולד זרע קיימא ויהיה עוסק במצות ומע״ט » (משנה ברורה, מ״ב ר״ל:א) and Hillel’s way: « אבל יכול לומר: ׳מובטחני שאין זה בתוך ביתי׳ אם הוא צדיק גמור, כמעשה דהלל הזקן שאמר כן » (ערוך השלחן או״ח סימן ר״ל) summary: one does not ask that the past have been otherwise; one asks about what follows, and over the past gives thanks. Case 2 — I am off to a big city

Situation: I leave my town in the morning, spend the day in a big city, and come home at night. What do I say, and when?
Conduct: the four formulas of the seif are said in their place — « כגון הנכנס לכרך אומר יהי רצון מלפניך ה׳ אלהינו שתכניסני לכרך הזה לשלום » (שו״ע או״ח ר״ל:א) « יצא בשלום אומר מודה אני לפניך ה׳ אלהי שהוצאתני מכרך זה לשלום וכשם שהוצאתני לשלום כן תוליכני לשלום וכו׳ עד בא״י שומע תפלה » (שו״ע או״ח ר״ל:א) but the Mishnah Berurah separates the cases: « י״ל דהתם מיירי ביוצא מעיר קטנה שאין שם מורא כלל לצאת מן העיר וצ״ל תה״ד רק משום מורא הדרך ולכך אין לומר אותה רק כשהחזיק בדרך וכמ״ש בס״ק ב׳ » (משנה ברורה, מ״ב ר״ל:ג) and for leaving a small town: « אלא אח״כ כשמחזיק בדרך אומר תפלת הדרך משום סכנת דרכים וכמבואר לעיל בסימן ק״י » (משנה ברורה, מ״ב ר״ל:ב) summary: leaving one’s house is not leaving a כרך; the tefilat haderech of siman 110 is said once the road is taken, and one does not repeat it on leaving the big city the same day. Case 3 — I am about to start a treatment

Situation: a procedure, a blood test, a course of treatment. Is there something to say before, and after?
Conduct: the seif gives it for bloodletting — « הנכנס להקיז דם אומר יהר״מ ה׳ אלהי שיהא עסק זה לי לרפואה כי רופא חנם אתה » (שו״ע או״ח ר״ל:ד) « ולאחר שהקיז יאמר ברוך רופא חולים » (שו״ע או״ח ר״ל:ד) the Mishnah Berurah extends it and states its spirit: « וה״ה בכל מידי דרפואה יאמר זה ולא יחשוב שתהיה איזה דבר לו רפואה אלא ע״י הבורא ית״ש » (משנה ברורה, מ״ב ר״ל:ו) « ולכן ע״י תפלה זו ישים בטחונו בו ויבקש ממנו שתהיה לו לרפואה » (משנה ברורה, מ״ב ר״ל:ו) and the Aruch HaShulchan describes the practice: « ונכון שכל רפואה שנוטל יאמר: ״יהי רצון מלפניך ד׳ אלקי שתהא זה לי לרפואה״, וכן נוהגין המדקדקים » (ערוך השלחן או״ח סימן ר״ל) summary: a request before, thanks after, and between them the awareness that the remedy is not the healing.
