A first approach to Siman 198, one of the longest of this block with its 48 seifim. After the previous siman set the time of the טבילה, this one describes the immersion itself and its great rule: she must immerse her whole body at once (כל גופה בפעם אחת), so nothing that interposes (חוצץ), even the smallest thing (כל שהוא), may be on her. The whole siman applies this principle across the body: hair and threads, the eye and kohl, wounds and bandages, the body and nails, jewelry, and the conduct of the טבילה. The decisive criterion is everywhere the same: מקפיד / אינו מקפיד — whether one minds it or not. So you don't drown in 48 cases, we have grouped them into 7 families. These are practical (למעשה) rules, to be learned with a Rav or a balanit / moret kalla.

1. The principle (§1) — the whole body at once

Siman 198 is the technical heart of the טבילה. The previous siman settled the time of immersion; this one describes the immersion itself. With its 48 seifim it is one of the longest in this block — yet everything in it flows from a single rule, stated in seif 1. Let us read it.

Here it is in the wording of the Shulchan Arukh: the Hebrew text of seif 1 and its full rendering are given below, in section 4, at the head of the forty-eight seifim. What this chapter draws out are the criteria that this seif establishes — the minding (מקפיד) and the extent of the body covered (רוב הגוף) — on which all the rest of the siman depends.

The founding principle. The טבילה is valid only if the water touches the entire body at once. The slightest object that comes between the water and the skin — the חציצה — obstructs the immersion. The whole siman is nothing but the application of this single rule, case by case.
מקפיד / אינו מקפיד — "to mind / not to mind." This is the central criterion. In brief: if the thing covers the majority (רוב) of the body or hair and she minds it → חוצץ by Torah law; if it is a minority (מקצת) and she minds it, or the majority but she does not mind it → חוצץ by Rabbinic law; if it is a minority and she does not mind it → אינו חוצץ. And it is enough that "people sometimes mind it" (דרך בני אדם להקפיד) for it to interpose.
Remember the grid in one phrase: רוב + מקפיד = חציצה דאורייתא; מקצת + מקפיד or רוב + אינו מקפיד = חציצה דרבנן; מקצת + אינו מקפיד = אינו חוצץ. This is the key that opens the 48 seifim.

2. The 48 seifim, in 7 families

Forty-eight seifim is a lot. But they are not a disordered list: the Mehaber reviews the body, head to foot, applying everywhere the same grid Here is the map of the whole siman — seven families that cover all of it.

Family Seifim The essential
1. The principle §1 The whole body at once; nothing interposing even the smallest thing; the מקפיד criterion
2. Hair, threads, knots §2-6 Tied threads and ribbons → חוצץ; hair not; knots; matted hair
3. The eye, kohl, discharge §7-8 לפלוף and כחול: inside the eye אינו חוצץ, outside the eye חוצץ
4. Wounds, blood, dressing, thorn §9-11 Dry blood and the discharge in the wound; the dressing; the thorn visible / hidden
5. Grime, mud, inks and dyes §12-17 Grime of sweat and crust, muds, ink and saps, clinging blood, women's dye and dyed hands
6. Nails, hanging flesh, ornaments §18-23 Dirt and dough under the nail (a major point); nail and flesh that hang; jewelry loose
7. The conduct of the טבילה and limit cases §24-48 Teeth, mouth, posture, the balanit, dust on the feet, immersing clothed / unintentionally
A single idea, declined. No need to memorize 48 isolated cases: each seif asks the same thing — this object, this substance, does it interpose between the water and the skin? and do people mind it? To master the grid of §1 is to hold the key to the whole siman. The next section gives all forty-eight, in the order of the book.

3. The key concepts of this siman

To read Siman 198 you must master a small vocabulary — that of the חציצה and its criterion.

חציצה — "interposition": anything that forms a screen between the mikvé water and the skin (or the hair). The verb is חוצץ ("it interposes"). If a חציצה remains during the טבילה, the immersion is invalid (לא עלתה לה טבילה): it must be redone (§1).
כל גופה בפעם אחת — "her whole body at once": the water must envelop the entire body simultaneously. From this requirement the whole law of חציצה is born: any barrier prevents "the whole body" from being immersed at one stroke (§1).
מקפיד — "one who minds": who is keen to remove this thing, not to keep it. Its opposite is אינו מקפיד. The "minding" may be personal or collective: it is enough that most people mind it (דרך בני אדם להקפיד) for it to interpose, even if she does not (§1).
רובו / מיעוטו — "the majority / the minority": the portion of the body (or hair) the thing covers. Combined with the מקפיד criterion, it determines whether the interposition is by Torah law, by Rabbinic law, or nonexistent (§1).
לכתחילה / בדיעבד — "initially / after the fact": לכתחילה, the Rama rules out even what does not interpose (as a precaution); but בדיעבד, if a non-interposing thing remained, the immersion counts. A distinction that recurs throughout the siman.
The keyword of the siman: חוצץ / אינו חוצץ. At each seif the question is binary — does this thing interpose or not? — and the answer depends on two parameters: the portion covered (רוב / מקצת) and the minding (מקפיד / אינו מקפיד). These assessments, fine and sometimes doubtful, are always decided with a Rav or a balanit / moret kalla.

4. The 48 seifim, in the order of the Shulchan Arukh

The forty-eight seifim of the siman, in the order of the Shulchan Arukh: for each one, the Hebrew text as it stands, then its rendering clause by clause. Glosses carried inside a seif are announced “Gloss:” and are never merged into the text of the Mehaber. The seven families serve only to guide the reading; they move no seif from its place.

Family 1 — The principle (§1)

Seif 1 — The great rule: the whole body at once

Heading of the siman ובו מ״ח סעיפים:) — The laws of the טבילה and of what interposes in it; and it contains forty-eight seifim.
She must immerse her whole body at once (כל גופה בפעם אחת); therefore nothing that interposes may be upon her (שום דבר החוצץ), even the slightest thing (ואפילו כל שהוא). And if it is the way of people sometimes to mind it (דרך בני אדם לפעמים להקפיד עליו) — it interposes, even if she does not mind it now (אינה מקפדת עליו עתה), or even if she never minds it (אינה מקפדת עליו לעולם), since most people are in the habit of minding it (דרך רוב בני אדם להקפיד עליו) — it interposes. And if it covers the greater part of the body (חופה רוב הגוף), even if people are not in the habit of minding it — it interposes.
Gloss: and לכתחילה she shall not immerse even with things that do not interpose (ולכתחלה לא תטבול אפילו בדברים שאינן חוצצין), a preventive decree on account of the things that do interpose (גזרה אטו דברים החוצצין).
The founding principle. The טבילה is valid only if the water touches the whole body at once. The least object interposing between water and skin — the חציצה — blocks the immersion. The forty-seven seifim that follow are nothing but the application of this single rule, case by case.
Seif 1 does not stop at the minding (מקפיד — and it is enough that most people mind it, even if she does not): it also sets out the surface covered (רוב הגוף), where minding is not required at all. Each suffices on its own.

Family 2 — Hair, threads, knots (§2-6)

Seif 2 — Threads and ribbons tied in the hair; the tight neckband

These are the things that interpose (אלו הדברים שחוצצין): threads of wool, threads of linen, and ribbons wound into the hair of the head (חוטי צמר וחוטי פשתן ורצועות שכורכין בהם השער בראש) — she does not immerse with them until she loosens them (לא תטבול בהם עד שתרפם). And if they are inside her braid, loosening them is of no avail (ואם הם בתוך קליעת שערה אינו מועיל בהם רפיון): they must be undone. And if they are wound on other parts of the body (ואם הם כרוכים בשאר מקומות בגוף), she does not immerse with them until she loosens them — except if they are wound on the neck, which do not interpose (חוץ מאם הם כרוכים בצואר שאינם חוצצין), because she does not tighten them there (לפי שאינה מהדקן). But the קטלא — a smooth, wide band she winds around her neck — does interpose (קטלא שהיא רצועה חלקה ורחבה שכורכת סביב צוארה חוצצת), because she chokes herself with force so that her flesh should bulge and she appear well-fleshed (מפני שחונקת עצמה בחוזק כדי שיהיה בשרה בולט ותראה בעלת בשר); and since the band is smooth and wide it does not hurt her (ומתוך שהרצועה חלקה ורחבה אינה מזיקתה) — which is why she can tighten it so (רש״י).
Why threads interpose and the neck is an exception — and why the קטלא is not one. A thread wound tight in the hair keeps the water out → חוצץ; loosening usually suffices, except in a braid, where it must be undone. At the neck one does not tighten — the water passes — so אינו חוצץ. But the קטלא is precisely the band one does tighten at the neck, out of vanity: the criterion is not the place, it is the tightening.

Seif 3 — Hollow threads, woven like a net

If these threads are hollow, made like a net — they do not interpose (אם החוטין האלו חלולין עשוי מעשה רשת אינם חוצצין): the water passes through them (טור in the name of the ראב״ד).

Seif 4 — Strands of hair do not interpose… unless she minds them

Strands of hair do not interpose
Gloss: but if they were gilded they do interpose, for she minds them lest she soil them (ואם היו מוזהבות חוצצין דמקפדת עליהם שלא תטנפם); and likewise if they were soiled beforehand, she minds them lest she be dirtied by them in the water — and they interpose (וכן אם היו מטונפים תחלה מקפדת עליהם שלא תתלכלך מהן במים וחוצצין) (טור).
The very same object — hair — interposes or not according to the minding: ordinary hair → אינו חוצץ; gilded or soiled, which she wishes to preserve → מקפדת → חוצץ. It is the exact illustration of the grid of seif 1.

Seif 5 — The knot in the hair: two hairs, one hair, the majority

Two hairs or more that were tied together in a single knot — do not interpose
Gloss: and there is no difference between tying two hairs with two hairs, or tying two hairs by themselves (ואין חילוק בין אם קשר ב׳ שערות עם שתי שערות או שקשר ב׳ שערות בפני עצמן) (ב״י in the name of the רשב״א and the ר״ן).
The Mehaber resumes: and a single hair that was knotted — interposes (ושערה אחת שנקשרה חוצצת), provided that she minds it (והוא שתהא מקפדת עליה); but if she does not mind it, her immersion is valid (אבל אם אינה מקפדת עליה עלתה לה טבילה) — unless the majority of her hair is knotted, hair by hair, each on its own (עד שיהא רוב שערה קשור נימא נימא בפני עצמו).

Seif 6 — Hair stuck together by sweat, according to the place on the body

Hair opposite the heart and hair of the beard, stuck to one another from sweat — interposes (שיער שכנגד הלב ושבזקן הנדבק זה בזה מחמת זיעה חוצץ); that of the head and that of the armpit (שבראש ושבבית השחי) does not interpose; and that of that place (ושבאותו מקום): in a man it does not interpose (באיש אינו חוצץ); and in a woman — in a married woman it interposes, in an unmarried woman it does not interpose (בנשואה חוצץ בפנויה אינו חוצץ).
Gloss: and those women who have a sort of braids of hair stuck one to another (ואותן שיש להן כמין קליעות שערות דבוקות זו בזו), formed at night by a שד, and which it is dangerous to remove (ונעשית בלילה על ידי שד וסכנה להסירם) — do not interpose (לא חייצי) (מרדכי, Hilkhot Nidda, in the name of the ראבי״ה; אגור; הגהות ש״ד).

Family 3 — The eye: discharge and kohl (§7-8)

Seif 7 — Eye discharge (לפלוף): outside the eye and in the eye

The לפלוף — that is, the matter of the eye — that is outside the eye, interposes (שחוץ לעין חוצץ), even if it is moist (אפי׳ הוא לח); and the לפלוף that is in the eye (שבעין) does not interpose (אינו חוצץ); and if it is dry (ואם הוא יבש) — it interposes (חוצץ), and that is when it has begun to yellow (והוא שהתחיל להוריק) (so ruled the תוספות and the סמ״ג in the name of ר״ת, and the רמב״ם).

Seif 8 — Kohl (כחול): in the eye, outside the eye, and the blinking eye

Kohl that is in the eye does not interpose, and that which is outside the eye interposes And if she was opening and closing (ואם היתה פותחת ועוצמת) her eyes (עיניה) continually (תדיר), even that which is outside the eye does not interpose (אף שחוץ לעין אינו חוצץ).
The border of the eye. The inside of the eye is a "hidden place" the mikvé water need not reach: what is there — moist discharge, kohl — does not interpose. But once the matter passes outside the eye, onto the eyelid or the skin, it forms a screen → חוצץ. Seif 8 adds a second ground for leniency: an eye that constantly opens and closes lets the water in. Hence, in practice, the importance of removing all eye makeup before the טבילה.

Family 4 — Wounds, blood, dressing, thorn (§9-11)

Seif 9 — Dry blood on the wound, and the discharge within it

Dry blood that is on the wound — interposes (דם יבש שעל המכה חוצץ); and the discharge inside it — does not interpose (וריר שבתוכה אינו חוצץ). If the discharge came out of the wound (יצא הריר מתוכה): anything within three days is moist and does not interpose (כל תוך ג׳ ימים לח הוא ואינו חוצץ); after that it is dry and it interposes (לאחר מכאן יבש הוא וחוצץ). Therefore a woman (לפיכך אשה) who has scabs must soak them in water until they soften (בעלת חטטים צריכה לחוף במים עד שיתרככו).

Seif 10 — The dressing laid on the wound

The dressing that is on the wound — interposes

Seif 11 — An arrow or a thorn embedded in the flesh

An arrow or a thorn embedded in the flesh (חץ או קוץ התחוב בבשר): if it is visible from without — it interposes (אם נראה מבחוץ חוצץ); and if it is not visible (ואם אינו נראה) — it does not interpose (אינו חוצץ).
The logic of the "hidden." What is buried in the body — the discharge deep in the wound, the invisible thorn — is not a חציצה, since the water need not penetrate the flesh. But everything laid upon it and visible — dry blood, dressing, protruding thorn — forms a screen. Today's medical dressing is exactly this case: it must be removed, and if it cannot be (suture, cast), it is a שאלה to put to a Rav.

Family 5 — Grime, mud, inks and dyes (§12-17)

Seif 12 — The grime of sweat, and grime that has crusted over

Grime that is on the flesh from sweat — does not interpose (לכלוכי צואה שעל הבשר מחמת זיעה אינם חוצצין); if it has peeled off as a crust — it interposes (נגלד כגליד חוצץ).

Seif 13 — Rolled-up specks of dirt on the skin

The מלמולין — little rolled-up specks of dirt — that are on the flesh, interpose (מלמולין שעל הבשר חוצצין).

Seif 14 — The three muds that always interpose, and all the others

Swamp mud, potters' clay, and the mud of the roads that is always found there, even in the summer days (טיט היון וטיט היוצרים וטיט דרכים הנמצא שם תמיד אפילו בימות החמה) — all of these interpose (כל אלו חוצצין). And all other mud: when it is moist it does not interpose, for it dissolves in the water (ושאר כל הטיט כשהוא לח אינו חוצץ שהרי הוא נמחה במים); and when it is dry it interposes (וכשהוא יבש חוצץ) (רמב״ם and סמ״ג).
An addition in parentheses: however, if she minds it, even a moist substance interposes (מיהו אם היא מקפדת אפילו בדבר לח חוצץ) (רוקח and מרדכי, Hilkhot Nidda).

Seif 15 — Ink, milk, honey, blood and the four saps

Ink, milk, honey and blood (הדיו החלב והדבש והדם), the sap of the fig, the sap of the mulberry, the sap of the carob and the sap of the sycamore (שרף התאנה ושרף התות ושרף החרוב ושרף השקמה) — that is, a species among the species of figs — : when dry they interpose; when moist they do not interpose (יבשים חוצצין לחים אינם חוצצין). And all the other saps interpose even when moist

Seif 16 — Blood that clings to the flesh

Blood (דם) that has clung to the flesh — interposes even when moist (שנסרך בבשר אפילו לח חוצץ).

Seif 17 — Women's dye and the dyer's hands

The dye with which women colour their faces and their hands (צבע שצובעות הנשים על פניהן וידיהן), and the hair of their head — does not interpose (ושער ראשן אינו חוצץ). And likewise one who is a dyer and whose hands are dyed — it does not interpose
Gloss: and likewise one (וכן מי) whose trade is to be a שוחט or a butcher, and whose hands are always soiled with blood — it does not interpose (שאומנותו להיות שוחט או קצב וידיו תמיד מלוכלכות בדם אינו חוצץ), for most people of this trade do not mind it (שרוב בני אומנות זו אינן מקפידים) (ב״י in the name of the ר״י).
Seifim 12 to 17 make one lesson: it is not the dirt that interposes, it is the wish to be rid of it. Grime from sweat, women's dye, the hands of the dyer or the butcher — one does not mind them, hence אינו חוצץ. A crust, potters' clay, clinging blood — one minds them, hence חוצץ.

Family 6 — Nails, hanging flesh, ornaments (§18-23)

Seif 18 — Dirt and dough under the nail, and where the custom of cutting comes from

Dirt that is under the nail (צואה שתחת הצפורן), on the side not against the flesh — interposes (שלא כנגד הבשר חוצץ); against the flesh — does not interpose (כנגד הבשר אינו חוצץ). And dough under the nail interposes even against the flesh And what is "not against the flesh"? That is where the nail extends beyond the flesh And because they cannot judge precisely what is called against the flesh or not against the flesh (ולפי שאינן יכולות לכוין מה נקרא כנגד הבשר או שלא כנגדו), women took the custom of cutting (נהגו הנשים ליטול) their nails at the time of the טבילה (צפרניהם בשעת טבילה).
Where the custom of cutting the nails comes from. The rule of dirt under the nail is subtle — against the flesh / not against the flesh — and the Mehaber himself defines the border: "not against the flesh" is the part of the nail that extends beyond the flesh. Rather than risk error, the custom became fixed: the nails are cut before the טבילה. It is one of the most concrete points of the siman, and a pillar of the חפיפה (the preparation).

Seif 19 — Swelling around the nail, which prevents cutting and cleaning

If she has a swelling at the place of the nail and can neither cut nor clean it out (אם יש לה נפח על מקום הצפורן ואינה יכולה לא לחתוך ולא לחטט): if it is swollen to the point that the dirt under the nail is no longer visible — it does not interpose (אם נפוחה כל כך שאין הטיט שתחת הצפורן נראה אינו חוצץ).

Seif 20 — The nail itself does not interpose — yet the gloss requires another immersion

It is only the dough under the nail that interposes (דוקא בצק שתחת הצפורן חוצץ); but the nail itself does not interpose (אבל הצפורן עצמה אינה חוצצת), and even if it was long, due to be cut, protruding and extending past the flesh — it does not interpose (ואפילו אם היתה גדולה ועומדת ליחתך ופורחת ועוברת מכנגד הבשר אינה חוצצת).
Gloss: however, all this only where there was no dirt or dough beneath it at the time she immersed (מיהו כל זה דוקא שאין צואה או בצק תחתיו בשעה שטבלה); and since the custom has already been taken to cut the nails, even if one single nail (ומאחר דכבר נהגו ליטול הצפרנים אפילו אם צפורן אחת) remained on her hand and she immersed (נשאר בידה וטבלה) — she requires another טבילה (צריכה טבילה אחרת) (הגהות ש״ד), and so is the practice (וכן נוהגין).
A stringency born of custom, not of law. The Mehaber is explicit: the nail itself does not interpose, however long. The gloss does not dispute this; it reasons otherwise — since the custom of cutting is already established, a nail left on becomes a thing one minds, and the immersion must be repeated. It is a clear case where the מנהג creates the חציצה that the law alone did not.

Seif 21 — The half-torn nail: the minority and the majority

A nail that hangs loose, a minority of which has separated — interposes (צפורן המדולדלת שפירשה מיעוטה חוצצת); if the majority of it has separated — it does not interpose (פירשה רובה אינה חוצצת).

Seif 22 — A limb and flesh that hang; a wart and a supernumerary finger

A limb and flesh that hang loose — interpose
An addition in parentheses: but a wart or a supernumerary finger, which do not hang loose — do not interpose (אבל יבלת או יתרת ואינן מדולדלין אינן חוצצים) (ב״י in the name of the סמ״ג, siman רמ״ח, in the name of the ר״י).

Seif 23 — Bracelets, earrings, rings, necklaces: loose or tight

Bracelets, earrings, rings and necklaces (השירים והנזמים והטבעות והקטלאות): if they are loose, they do not interpose (אם הם רפויים אינם חוצצים); and if they are tight, they interpose (ואם הם מהודקים חוצצים). And the same law applies to the bandage on the wound and to the splints on a fracture
Always the same grid: it is not the jewel as such, it is the tightness. A tight jewel keeps the water out → חוצץ; loose → the water passes → אינו חוצץ. In practice, all jewelry is removed before the טבילה — simpler and safer than judging "loose" case by case.

Family 7 — The conduct of the טבילה and the limit cases (§24-48)

Seif 24 — Cleaning between the teeth, and the customs around it

She must clean between her teeth, so that nothing interposing be in them (צריכה לחצוץ שיניה שלא יהא בהם דבר חוצץ), for if she immersed and something was found stuck in them — her immersion is invalid (שאם טבלה ונמצא שום דבר דבוק בהם לא עלתה לה טבילה). And some have the custom not to eat meat on the day they go to the immersion-house, because it gets between the teeth more than any other food (ויש נוהגות שלא לאכול בשר ביום לכתן לבית הטבילה מפני שהוא נכנס בין השינים יותר ממאכל אחר); and although they check and clean their teeth, they are concerned that something may remain without their noticing (ואף על פי שבודקות וחוצצות השינים חוששות דילמא תשתייר מיניה ולאו אדעתה) — and it is a fine custom (ומנהג יפה הוא).
Gloss: and she should not eat between the washing and the immersion (ואין לה לאכול בין הרחיצה לטבילה) (הרא״ש, בה״נ); and she should not handle dough or wax candles all the day before the immersion, so that nothing stick to her (ואין לה לעסוק כל היום קודם הטבילה בבצק או בנרות של שעוה שלא ידבק בה) (ש״ד) — and so is the practice (וכן נהגו).

Seif 25 — The hidden folds: not having checked, and nothing having been found

If she did not rinse her hidden places and her folds, and something interposing was found in them — her immersion is invalid And if nothing interposing was found upon her, even though she did not check before the immersion — her immersion is valid (ואם לא נמצא עליה דבר חוצץ אף על פי שלא בדקה קודם טבילה עלתה לה טבילה): this is not like the בדיקה of the body and the חפיפה of the head (ואינו דומה לבדיקת הגוף וחפיפת הראש) — which are required in themselves.

Seif 26 — Found afterwards, after she busied herself with other things: we attribute it to after

If she did not check before the immersion, neither between her teeth nor her hidden places (אם לא בדקה קודם טבילה בין שיניה ולא בית הסתרים שלה), and after the immersion too she did not check, until she had busied herself with stains and with cooked dishes (ואחר טבילה נמי לא בדקה עד שנתעסקה בכתמים ובתבשילין), and afterwards she checked and found a bone between her teeth or something interposing in her folds (ואחר כך בדקה ומצאה עצם בין שיניה או דבר חוצץ בין סתריה) — we attribute it leniently, and we say that it entered her after the immersion (תלינן לקולא ואמרינן דבתר טבילה עיילי בה).

Seif 27 — Hair in the mouth, pursed lips, a clenched hand

If she put her hair in her mouth, or pursed her lips, or clenched her hand (נתנה שערה בפיה או קרצה שפתותיה או קפצה ידה) in such a way that the water did not come to them — her immersion is invalid (בענין שלא באו המים בהם לא עלתה לה טבילה).

Seif 28 — The companion's hand holding her during the immersion

Her companion shall not hold her with her hands at the time of immersion (לא תאחוז בה חברתה בידיה בשעת טבילה), unless she loosened her hand so that the water may come to the place where her hands hold her (אלא אם כן רפתה ידה כדי שיבואו המים במקום אחיזת ידיה). And if she first rinsed her hands in the water, it is permitted (ואם הדיחה ידיה במים תחלה שרי), for the moisture on her hands joins with the waters of the mikvé (שמשקה טופח שעל ידיה חבור למי המקוה).

Seif 29 — Exactly forty se'ah: "press your hand upon me"

One who immerses in a mikvé that holds no more than exactly forty se'ah (הטובל במקוה שאין בו אלא מ׳ סאה מצומצמין) — if he said to his fellow, "press your hand upon me in the mikvé," this is unseemly (אם אמר לחבירו כבוש ידך עלי במקוה הרי זה מגונה).

Seif 30 — Feet resting on the floor: the water gets there first

She need not lift her feet at the time of her immersion, if there is no mud there (אינה צריכה להגביה רגליה בשעת טבילתה אם אין שם טיט): although she treads on the floor, there is no חציצה here (אע״פ שדורסת על הרצפה אין כאן חציצה), because the water gets to her feet first (מפני שהמים מקדימים לרגליה).

Seif 31 — One does not immerse standing on a vessel — and the wooden steps of the mikvé

One does not immerse standing upon vessels Therefore, if there was mud in the place where she immerses (לפיכך אם היה טיט במקום שטובלת), she shall not stand upon a wooden vessel, which contracts impurity from its back (לא תעמוד על גבי כלי עץ שמקבלין טומאה מגבן), nor upon boards fit to be trodden on (ולא על גבי נסרים שראוים למדרסות), nor upon any vessel susceptible to מדרס, and immerse (ולא על שום כלי הראוי למדרס ותטבול) — because of the decree concerning bathhouses, made on account of vessels (משום גזירת מרחצאות של כלים). If she transgressed and immersed — her immersion is invalid But she may place bundles of vine-shoots under her feet, on account of the mud And likewise she shall not stand upon an earthenware vessel nor upon a split log, and immerse (וכן לא תעמוד על גבי כלי חרס ולא על גבי בקעת ותטבול) — and although an earthenware vessel does not contract impurity from its back and is not susceptible to מדרס (ואף על פי שאין כלי חרס מטמא מגבו ולא ראוי למדרס), it is a concern of the Sages: perhaps she will be afraid of falling and will not immerse properly (חשש חכמים הוא שמא תפחד שלא תפול ולא תטבול כראוי). If she transgressed and immersed upon these — her immersion is valid And according to this, a mikvé that has rungs (ולפי זה מקוה שיש בו שליבות) — that is, steps — of wood: if she immersed standing upon those rungs (של עץ אם טבלה על גבי השליבות), even if they are attached to the walls of the mikvé (אפילו אם הם מחוברים לכותלי המקוה) (תשובת הרשב״א) — her immersion is invalid, for they are flat wooden vessels (לא עלתה לה טבילה דפשוטי כלי עץ הם). And one must make in their place a step of stone (וצריך לעשות במקומן מדרגה של אבנים), and the step shall be four wide, the place where the foot is set (ותהיה המדרגה רחבה ד׳ מקום הנחת הרגל), so that there be in it the measure of the mikvé, that she not fear to fall from it (כדי שיהא בה שיעור מקוה לבל תפחד ליפול ממנה).

Seif 32 — The wooden pipe fixed in the floor of the mikvé

A wooden pipe fixed in the floor of the immersion pool (סילון של עץ הקבוע בקרקעית הטבילה): if it has no rim (אם אין לו לבזבז) — that is, a frame, in which case it has no receptacle — she is permitted to stand upon it and immerse (מותרת לעמוד עליו ולטבול).

Seif 33 — Mud at the bottom of the pool

She shall not immerse in a place whose floor has mud, because of חציצה (לא תטבול במקום שיש בקרקעיתו טיט משום חציצה), unless she places upon it vine-shoots or the like, something that contracts no impurity (אלא אם כן תתן עליו זמורות וכיוצא בהם דבר שאינו מקבל שום טומאה). And if she immersed — some say her immersion is invalid (ואם טבלה י״א שלא עלתה לה טבילה) (ראב״ד and רש״י; but most decisors permit).

Seif 34 — A place where she might be seen

She shall not immerse in a place where there is concern that people may see her (לא תטבול במקום שיש חשש שיראוה בני אדם), because on that account she hurries to immerse and is not careful in her immersion (מפני שמתוך כך ממהרת לטבול ואינה מדקדקת בטבילה). However, after the fact, her immersion is valid

Seif 35 — The posture of immersion: neither too upright nor too bent

She shall not immerse with the body upright, because there are places in her that become hidden (לא תטבול בקומה זקופה מפני שיש מקומות שמסתתרים בה); and let her not bend much, to the point that her folds cling one to another (ואל תשחה הרבה עד שידבקו סתריה זה בזה); but she bends a little, until the folds of the lower body are visible as they are when she kneads (אלא שוחה מעט עד שיהיו סתרי בית הערוה נראים כדרך שנראית בשעה שהיא עורכת), and the underside of her breasts is visible as it is when she nurses her child (ויהיה תחת דדיה נראה כדרך שנראה בשעה שמניקה את בנה), and the underside of the armpit is visible as it is when she weaves at an upright loom (ויהיה תחת בית השחי נראה כדרך שנראה כשאורגה בעומדין). And she need not separate her thighs from one another more than is fitting (ואינה צריכה להרחיק ירכותיה זו מזו יותר מדאי), nor separate her arms from the body more than is fitting (וגם לא להרחיק זרועותיה מהגוף יותר מדאי), but as they are when she walks (אלא כדרך שהם בעת הילוכה). And if she did otherwise — for instance bent excessively or stood excessively upright — her immersion is valid (ואם שינתה כגון ששחתה ביותר או זקפה ביותר עלתה לה טבילה) (ערוך, סמ״ג, רשב״ץ and רמב״ם); and there is one who says it is not valid (ויש מי שאומר שלא עלתה).

Seif 36 — The height of the water: a span above the navel

The mikvé must rise above her navel by at least a זרת (צריך שיהיה המקוה גבוה ממעל לטבורה זרת לפחות) — a span, the width of the open hand.

Seif 37 — One opinion: the body pressed to the ground suffices

There is one who says that even if the depth of the mikvé waters does not suffice for her whole body to be covered in them (יש מי שאומר שאע״פ שאין בגובה מי המקוה לעלות בהם כל גופה), save by having her face and body pressed to the ground — that is acceptable (אלא אם כן פניה וגופה כבושים בקרקע שפיר דמי). (And see further, siman ר״א seif ס״ו, in the laws of the mikvé.)

Seif 38 — The mouth: neither open nor clenched — a middling contact

She need not open her mouth for the water to enter it (אינה צריכה לפתוח פיה כדי שיכנסו בה המים), and let her not clench it more than is fitting (ולא תקפוץ אותה יותר מדאי); and if she clenched it — her immersion is invalid (ואם קפצה לא עלתה לה טבילה); but she shall bring her lips together with a middling contact (אלא תשיק שפתותיה זו לזו דיבוק בינוני).

Seif 39 — The eyes: neither shut too tight nor opened too wide

She shall not shut her eyes too tight nor open them too wide (לא תעצים עיניה ביותר ואל תפתחם ביותר); and if she did so, some say her immersion is invalid (ואם עשתה כן יש אומרים שלא עלתה לה טבילה) (two opinions in the טור and among the decisors; see the ב״י).

Seif 40 — The woman who supervises the immersion, and what to do without her

One must station beside her an adult Jewish woman, over twelve years and one day, at the time she immerses (צריך להעמיד על גבה יהודית גדולה יותר מי״ב שנה ויום א׳ בשעה שהיא טובלת), so that she may see that none of her hair remains floating on the surface of the water (שתראה שלא ישאר משער ראשה צף על פני המים). And if she has no one to stand beside her, or it is night (ואם אין לה מי שתעמוד על גבה או שהוא בלילה) — she shall wind her hair upon her head with woollen threads, or with the ribbon that is on her head, provided she loosens them (תכרוך שערה על ראשה בחוטי צמר או ברצועה שבראשה ובלבד שתרפם), or with little chains of hollow threads (או בשרשרות של חוטים חלולות), or she ties a loose garment over her hair (או קושרת בגד רפוי על שערותיה).

Seif 41 — One who immerses with her child slung on her back

A woman who carries her child slung behind her while she is naked, and immersed — her immersion is invalid (המפשלת בנה לאחוריה כשהיא ערומה וטבלה לא עלתה לה טבילה), for perhaps there was mud on the child's feet or hands, which stuck to his mother and interposed at the time of immersion (שמא היה טיט ברגלי התינוק או בידיו ונדבק באמו וחצץ בשעת טבילה), and after she came up it fell off (ואחר שעלתה נפל).

Seif 42 — Pebbles and splinters lodged in the cracks of the feet

If pebbles and splinters entered the cracks of her feet from below — they interpose

Seif 43 — Plasters and dressings on hidden places: "fit" to receive the water

A plaster, a poultice and a dressing that are on the hidden places — interpose (אספלנית מלוגמא ורטיה שעל בית הסתרים חוצצין): although the water need not enter them (אף על פי שאינם צריכים שיכנסו בהם המים), they must be fit [to receive it] and there must be nothing interposing in them (צריכי׳ שיהיו ראוים ולא יהא בהם דבר חוצץ).
Gloss: some say that a woman must relieve herself of urine before the immersion, if she needs to (יש אומרים שהאשה צריכה להטיל מים קודם טבילה אם היא צריכה לכך); she must also check herself for both needs, so that she not have to hold herself in and these places cease to be fit for the coming of the water (גם צריכה לבדוק עצמה בגדולים ובקטנים שלא תהא צריכה לעצור עצמה ולא יהיו ראויים לביאת מים); she must also remove the mucus of the nose (גם צריכה להסיר צואת החוטם) (ש״ד and ראב״ן, siman שכ״ו).
"Fit for the coming of the water" (ראוי לביאת מים). The hidden places — mouth, nose, folds — do not need the water to enter them; but they must remain capable of receiving it. Hence a dressing laid there interposes, and hence too the reason the gloss gives for relieving oneself before the טבילה: holding oneself in makes the place unfit.

Seif 44 — Hairs stuck fast: to the wound, to mud, to the eyelids

If there was on her one hair or two outside the wound of her head, stuck to the wound (היתה בה שערה אחת או שתים חוץ למכת ראשה מודבק למכה), or two hairs of her head were stuck together by mud or by filth (או שהיו שתי שערות ראשה מודבקות בטיט או בצואה), or there were two hairs in her lower eyelids which pierced through and she brought them out through her upper eyelids (או שהיו שתי שערות בריסי עיניה מלמטה ונקבו והוציאתן בריסי עיניה מלמעלה), and likewise if two hairs of her lower eyelids were stuck to her upper eyelids (וכן אם היו ב׳ שערות ריסי עיניה של מטה מדובקות בריסי עיניה של מעלה) — these interpose (הרי אלו חוצצים).

Seif 45 — The dust on her feet

She shall not immerse with the dust of her feet (לא תטבול באבק של רגליה); and if she immersed — there is one who says it does not interpose (ואם טבלה יש מי שאומר שאינו חוצץ), and there is one who says it does interpose (ויש מי שאומר שחוצץ), unless she rubbed it off or immersed in hot water (אלא אם כן שפשפה או שטבלה בחמין).

Seif 46 — A נדה who immersed in her clothes

A נדה who immersed in her clothes — is permitted to her husband (נדה שטבלה בבגדיה מותרת לבעלה).

Seif 47 — The kind of lice that cling to the skin

A kind of lice that cling to the flesh, biting the skin where the hair is and attaching themselves firmly to the flesh (מין כנים שדבוקים בבשר ונושכים בעור במקום שיער ונדבקים בחוזק בבשר) — one must remove them by means of hot water and scrape them off with the nail (צריך להסירן ע״י חמין ולגורדן בצפורן); and if one cannot remove them — it does not interpose (ואם אינו יכול להסירן אינו חוצץ).

Seif 48 — One who immersed unintentionally, and the discretion of the night of immersion

A נדה who immersed without intention — for instance she fell into the water or went down to cool off (נדה שטבלה בלא כוונה כגון שנפלה לתוך המים או שירדה להקר) — she is permitted to her husband (הרי זו מותרת לבעלה).
Gloss: and some are stringent and require her to immerse again (ויש מחמירין ומצריכין אותה טבילה אחרת) (ב״י in the name of the רשב״א, ר׳ ירוחם, the רוקח and the הגהות אשיר״י), and one should be stringent לכתחילה (ויש להחמיר לכתחלה). Some have written that a woman should be modest on the night of her immersion (יש שכתבו שיש לאשה להיות צנועה בליל טבילתה); and so women took the custom of concealing the night of their immersion, not to go about in a commotion or before people, so that they not be noticed (וכן נהגו הנשים להסתיר ליל טבילתן שלא לילך במהומה או בפני הבריות שלא ירגישו בהן בני אדם); and of her who does not do so it is said: ארור שוכב עם בהמה (ומי שאינה עושה כן נאמר עליה ארור שוכב עם בהמה). And women should take care, on coming out of the immersion, that it be their companion who meets them (ויש לנשים ליזהר כשיוצאות מן הטבילה שיפגע בה חברתה), and that no impure thing or idolater meet them first (שלא יפגע בה תחילה דבר טמא או עובד כוכבים); and if these things did meet her — if she is God-fearing, she will go back and immerse again (ואם פגעו בה דברים אלו אם היא יראת שמים תחזור ותטבול) (ש״ד, כל בו and רוקח). (And see further, at the end of siman ר״א, whether it is permitted to pour hot water into the mikvé, and whether it is permitted to wash after the immersion.)
The spirit of the last family. Seifim 24 to 48 settle the procedure: cleaning the teeth and the folds, posture (neither too bent nor too upright), the depth of the water, the woman who watches that no hair floats, the floor of the pool, and limit cases — dust on the feet, immersion clothed or involuntary. The bottom line: בדיעבד one is lenient (the involuntary or clothed immersion counts, and what is found late is attributed to afterwards), but לכתחילה one is stringent, and the gloss even inclines to repeat the טבילה.

5. The Taz, the Shach, the Pitchei Teshuva

In Yoreh De'ah, the Shulchan Aruch is never read alone. Three great commentaries are indispensable here — all the more so as Siman 198 is one of the most commented in all of Yoreh De'ah, so rich is the matter of חציצה.

The Taz (ט״ז) — abbreviation of טורי זהב, Turei Zahav, by Rabbi David haLevi Segal (Poland, 17th century). It is the great reference of this siman: he comments on nearly every seif, refines the grid of חציצה and resolves concrete cases. On the טבילה, one studies it line by line.
The Shach (ש״ך) — abbreviation of שפתי כהן, Siftei Kohen, by Rabbi Shabtai haCohen (Lithuania, 17th century). The reference analytical commentary on Yoreh De'ah; here it is completed by the works dedicated to nidda — the Sidrei Tahara and the Chochmat Adam.
The Pitchei Teshuva (פתחי תשובה) — by Rabbi Avraham Tsvi Hirsch Eisenstadt (19th century). On this siman it is especially rich: it gathers the responsa of the Acharonim on concrete cases — חציצות, nails, jewelry, teeth, טבילה בלא כוונה — which bridge to today's practice.
One sees the method: these commentaries add no arbitrary rules — they apply the grid of §1 to new situations (a medical object, a dye, an adornment), asking each time: does it cover רוב or מקצת? do people mind it? This is exactly what is deepened at the Lamdan level, then at the Halacha lema'asse level with the contemporary poskim.

6. The gloss of the Rama (הגה)

The Rama (Rabbi Moshe Isserles) adds to the Mehaber's text glosses reflecting the Ashkenazi custom and tightening practice. Here are his most notable interventions in our siman.

On seif 1 — stringency

Gloss of the Rama: ולכתחלה לא תטבול אפילו בדברים שאינן חוצצין גזרה אטו דברים החוצצין (רמ״א יו״ד קצ״ח:א) — "לכתחילה, she does not immerse even with things that do not interpose, by a preventive decree (גזרה) lest she come to those that do interpose." The underlying law permits the "non-interposing"; the custom rules out everything as a precaution.

On seif 4 — gilded or soiled hair

Gloss of the Rama: ואם היו מוזהבות חוצצין דמקפדת עליהם… וכן אם היו מטונפים תחלה… וחוצצין — "and if they were gilded, they interpose for she minds them… and likewise if they were soiled beforehand… they interpose." The Rama shows that the minding (מקפיד) turns a harmless object into a חציצה.

On seif 48 — the יש מחמירין of unintentional immersion

Gloss of the Rama: ויש מחמירין ומצריכין אותה טבילה אחרת… ויש להחמיר לכתחלה (רמ״א יו״ד קצ״ח:מ״ח) — "some are stringent and require another טבילה, and one should be stringent לכתחילה." The Mehaber validates the involuntary טבילה; the Rama inclines to redo it as a precaution.
The Rama carefully distinguishes the underlying law (the Mehaber) from lived practice: he rules out לכתחילה even the "non-interposing" (§1), he stresses the role of the מקפיד (§4), and he tightens the בדיעבד of unintentional immersion (§48). This is the whole balance of the siman: the law is lenient בדיעבד, the custom stringent לכתחילה.

7. The chatsitsa table

The whole siman fits into a two-entry grid: the portion covered (רוב / מקצת) and the minding (מקפיד / אינו מקפיד). Here is the grid of §1, then a few typical cases drawn from the seifim.

Portion covered Minding Verdict
רוב (majority) מקפיד (minds it) 🔴 חוצץ by Torah law
מקצת (minority) מקפיד 🟠 חוצץ by Rabbinic law
Case Verdict Seif
Thread / ribbon tight in the hair 🔴 חוצץ (until loosened / undone) 2
Discharge / kohl inside the eye 🟢 אינו חוצץ — outside the eye → 🔴 חוצץ 7-8
Dry blood, bandage, visible thorn 🔴
Dough / dirt under the nail → cut 🔴 חוצץ → custom of cutting the nails 18
The logic in one phrase: the water must touch the whole body at once; everything that forms a screen and that one wants to remove (מקפיד) interposes; doubt is decided by רוב / מקצת and מקפיד / אינו — and, in real life, with a Rav or a balanit / moret kalla.

8. Practical cases

How do these 48 seifim take shape today? Everything converges on a single act of preparation: the חפיפה. Here are the common situations — each closes on the same referral: the concrete decision belongs to the Rav, the balanit or the moret kalla.

Case 1 — The חפיפה: the preparation before the טבילה

The חפיפה is the meticulous preparation of the body before immersion. One washes and combs the hair (so that none is matted), and one removes everything that could interpose: loose hair and dead skin, nails (which are cut — §18), makeup and kohl (§7-8), nail polish, jewelry (§23), bandages (§10), contact lenses, etc. One also checks the "hidden places" and between the teeth (§24). All of this is learned, for the first time, with a moret kalla or a balanit.

Case 2 — Modern interpositions (חציצות)

Our era adds cases the Mehaber did not have in view, yet which fall exactly under his grid (מקפיד?). Nail polish and false nails / extensions, permanent makeup, contact lenses, fillings and dental crowns, a cast and bandages: each is a genuine שאלה (question). Most can and must be removed; some (a permanent filling, a medical cast, a suture) cannot — and then one does not decide alone: one poses the שאלה to the Rav.

Case 3 — The role of the balanit

The balanit (the mikvé attendant) accompanies the טבילה and ensures its validity: she verifies that no חציצה remains, that no hair floats on the surface (§40), that the posture lets the water reach the whole body (§35). She is the experienced eye that closes the preparation. For any doubt at the mikvé, one relies on the balanit — and, for questions of substance, on the Rav.

Case 4 — Immersed clothed or unintentionally

Two distinct cases, and the Shulchan Arukh does not treat them together. §46 — a נדה who immersed in her clothes (נדה שטבלה בבגדיה) is permitted to her husband; the seif does not qualify the garment and gives no reason. §48 — a נדה who immersed without intent (בלא כוונה), having fallen into the water or gone down to cool off, is likewise permitted. Gloss of the Rama, on seif 48 only: ויש מחמירין ומצריכין אותה טבילה אחרת, ויש להחמיר לכתחלה. Whether such a case is Whether such a case is valid is never a personal assessment: one poses the question to the Rav.
The common thread: Siman 198 describes what must form a screen to nothing — the חציצה — and its criterion, the מקפיד. In practice, everything comes down to a careful חפיפה and the verification of the balanit. The modern cases (polish, lenses, fillings, cast) are so many שאלות. The concrete decision always belongs to the Rav, the balanit or the moret kalla.

9. Synthesis of Siman 198

The essence of Siman 198 in a few phrases:
  1. The principle (§1): she must immerse her whole body at once (כל גופה בפעם אחת); nothing interposing (חוצץ), even the smallest thing, may be on her; the criterion is מקפיד / אינו מקפיד crossed with רוב / מקצת.
  2. Hair, threads, knots (§2-6): tight threads and ribbons → חוצץ (undo them); hair not, except gilded or soiled (מקפדת).
  3. Eye and kohl (§7-8): discharge and kohl inside the eye do not interpose; outside the eye, they interpose.
  4. Wounds, blood, bandages, splinters (§9-12, 22-23): dry blood, רטיה and visible thorn → חוצץ; the discharge in the wound and the hidden thorn → אינו חוצץ.
  5. Body, nails, grime (§13-21): dirt and dough under the nails → hence the custom of cutting the nails at the טבילה.
  6. Jewelry (§23): loose אינם חוצצים, tight חוצצים → remove them all.
  7. Conduct of the טבילה (§24-48): teeth, posture, balanit; dust on the feet (§45), immersed clothed (§46) or unintentionally (§48) → lenient בדיעבד, but the Rama: יש מחמירין.

Memory table

Point Rule

The criterion
Hair and threads Tight thread → חוצץ; hair → אינו (gilded / soiled → חוצץ) (§2-4)
Eye and kohl Inside the eye 🟢 — outside the eye 🔴 (§7-8)
Wounds and bandages Dry blood / רטיה / visible thorn → חוצץ (§9-11)
Nails Dough / dirt beneath → custom of cutting (§18)
Jewelry Loose 🟢 — tight 🔴 → remove them (§23)

Comprehension questions

Check your understanding:
  1. What does כל גופה בפעם אחת mean, and why does the whole law of חציצה flow from it (§1)?
  2. Explain the grid רוב / מקצת × מקפיד / אינו מקפיד: when is the interposition by Torah law, by Rabbinic law, or nonexistent (§1)?
  3. Why does a tight thread in the hair interpose, while at the neck it does not (§2)?
  4. Why does ordinary hair not interpose, but gilded or soiled hair does (§4)?
  5. What is the difference between discharge (לפלוף) inside the eye and outside the eye (§7-8)?
  6. Where does the women's custom of cutting their nails before the טבילה come from (§18)?
  7. Does a jewel always interpose? What makes the difference (§23)?
  8. Is a woman who fell into the water without intent of טבילה pure בדיעבד? What does the Rama report (§48)?
