3. Hierarchy of cases

During labor and the first 3 days: full sakanah — one is mechalel for all her needs, even if she says she needs nothing.
From the 3rd to 7th day: one transgresses if she says "I need"; abstains if she says "I don't need".
From the 7th to 30th day: status of choleh without sakanah; no Torah issur, but refuah and fire permitted.
At all times: what can be done with shinui — without delay — is done with shinui.

Q2 — How many days since birth? ≤3 → full sakanah; 3-7 → per her words; 7-30 → choleh without sakanah.
Q3 — Can this action be done with shinui without delay? Yes → with shinui. No → normally.
Q4 — Care for the newborn? Viable (7/9 months) → permitted. Doubt (8 months without signs) → consult your Rav.

5. The 3 / 7 / 30 day scale: descending severity

The yoledes has, initially, the full status of חולה שיש בו סכנה. But this status does not extinguish at once: it decreases in steps, and the practical difficulty is knowing, each passing day, which regime applies. This is the central mechanism of the siman — understanding that the same danger is also measured in time.

The three steps and their logic

≤ 3 days → one is mechalel for everything, even against her word.
3-7 days → her word decides: "need" → one transgresses; "don't need" → abstains.
7-30 days → choleh without sakanah; fire permitted against cold.

Shinui: a transverse requirement at all stages

To this time scale a second rule applies, valid at any moment: what can be done with a change in the way of acting (שינוי) must be done so — but on the sole condition that this causes no delay or difficulty. Birth being predictable, many acts can be anticipated or arranged; but at the slightest risk of dragging, shinui falls and one acts normally, since saving overrides minimization.

Remember: two axes cross — the time elapsed (3/7/30) sets the regime; shinui modulates the manner, without ever slowing the essential.

7. Pitfalls to avoid

Pitfall 1 — hesitating during labor. From signs of onset, the yoledes is a choleh in sakanah: one acts for all her needs, even against her word.
Pitfall 2 — confusing shinui and delay. Shinui applies only if it causes no delay or difficulty. At the slightest risk of dragging, one acts normally.
Pitfall 3 — not anticipating. A woman in her 9th month must prepare every erev Shabbos the necessary — not preparing in advance causes unnecessary transgression.
Pitfall 4 — refusing the lamp. One lights for the yoledes even if she is blind, even if she asks for nothing: her calming (yishuv ha-da'as) is part of pikuach nefesh.
