1. The text of the Shulchan Aruch

Siman רצ"ד contains 5 seifim of the Mechaber (Rabbi Yosef Karo) codifying the laws relating to havdalah in tefillah.

Seif א — Havdalah in the berachah of "Chonen ha-Da'as"

דיני הבדלה בתפלה. ובו ה סעיפים:
אומרים הבדלה בחונן הדעת ואם טעה ולא הבדיל משלים תפלתו ואינו חוזר מפני שצריך להבדיל על הכוס ואם טעם קודם שהבדיל על הכוס צריך לחזור ולהבדיל בתפלה:
Translation: "One says havdalah in the berachah of "Chonen ha-Da'as" ("Atta chonen le-adam da'as", the 4th berachah of the Amidah). If one forgot to say it, one completes the tefillah and does not repeat — since one must make havdalah on the cup [so one will make up for it that way]. But if one tasted something before making havdalah on the cup, one must go back and say havdalah in tefillah."
Place of havdalah in the Amidah: in the 4th berachah "Atta chonen", one inserts "Atah Chonantanu" ("You graced us with knowledge of Your Torah... You distinguished between holy and mundane, between light and darkness, between Israel and the nations, between the 7th day and the 6 days of work...") just before "ve-chonnenu mei'itcha".
Rationale of "no need to repeat": havdalah is really 2 parallel mitzvos: (1) in tefillah and (2) on the cup. If one misses the first, one can rely on the second. But if one tastes before havdalah on the cup — one has lost the second, so one must redo the first.

Seif ב — Forgot + no cup available

טעה ולא הבדיל בתפלה ואין לו כוס בלילה וסבור שאף למחר לא יהיה לו צריך לחזור ולהתפלל:
Translation: "If one forgot and did not say havdalah in tefillah, and one has no cup [of wine] at night, and one thinks one will not have one tomorrow either — one must repeat the tefillah."
Inverse case of seif א: without a cup available (neither tonight nor tomorrow), the only way to fulfill havdalah is in tefillah. So one must absolutely repeat it — even to the point of redoing the entire Amidah.

Seif ג — Tisha b'Av falling on Sunday

תשעה באב שחל להיות באחד בשבת טעה ולא הבדיל בתפלה א"צ לחזור ולהתפלל כיון שמבדיל על הכוס במוצאי תשעה באב:
Translation: "Tisha b'Av that falls on Sunday [so Tisha b'Av begins Saturday night, motzaei Shabbat] — if one forgot and did not say havdalah in the tefillah of Maariv, one does not need to repeat the tefillah, since one will make havdalah on the cup at the conclusion of Tisha b'Av [Sunday night]."
Special case: Tisha b'Av on Sunday → one must fast all day Sunday, so no havdalah on the cup Saturday night (prohibited to drink wine during the fast). It is pushed off to motzaei Tisha b'Av (Sunday night, end of the fast). So no urgency to say it in tefillah — it will come anyway Sunday night.

Seif ד — No going back even before the next berachah

במקום שאמרו שאינו חוזר להתפלל מיד כשסיי' הברכ' אין לו לחזור אע"פ שלא פתח בברכה שלאחריה:
Translation: "In a case where one said that one does not repeat the tefillah (= case of seif א, where one has a cup), as soon as one has concluded the berachah [of Atta chonen], one must not go back [to add Atah Chonantanu] — even if one has not yet begun the following berachah."
Strict rule of irreversibility: normally, in tefillah, if one missed an insertion, one can go back as long as the next berachah has not begun. Here (havdalah) it is different: as soon as one concludes "Atta chonen", it is too late, even if one has not begun "Re'eh be-onyeinu". The make-up will be done on the cup.

Seif ה — Optional stringency

במקום שאמרו שאינו חוזר אם רצה להחמיר על עצמו לחזור אם סיים תפלתו רשאי אבל אם עדיין לא סיים תפלתו אינו רשאי לחזור:
Translation: "In a case where one said that one does not repeat, if one wishes to be stringent on oneself and repeat — if the tefillah is entirely completed, one is permitted to do so. But if one has not yet completed one's tefillah, one is not permitted to repeat."

What does this siman discuss?

Our siman codifies havdalah in the tefillah of Maariv of motzaei Shabbat — the insertion of "Atah Chonantanu" in the 4th berachah of the Amidah ("Atta chonen"). It is the 1st of the 2 havdalos of motzaei Shabbat — the 2nd being on the cup (wine/juice) at home.

Source of havdalah in tefillah: Berachos 33a — "the Anshei Knesses HaGedolah instituted Kiddush and havdalah in tefillah. When they became wealthy, they placed it on the cup; when they became impoverished, they returned it to tefillah. Therefore one says both".
