It is a level of practical ruling: what one does, and whom to ask.

How to read this level. Every statement here rests either on the text of the Shulchan Arukh and its nosei kelim (Taz, Shakh, Baer Hetev), or on a named and checkable posek (Rambam, Tur, Beit Yosef, gemara Avodah Zarah). On Yoreh De'ah there is no Mishnah Berurah (which comments only on Orach Chaim) and no Shulchan Arukh HaRav (the Admur HaZaken did not write Yoreh De'ah). The Pit’hei Teshuva does not comment on this siman — he is absent here, and nothing is put in his place. The Arukh HaShulchan is not cited: his text on this siman is not accessible to us, and one does not quote what one cannot check. Siman 138 describes equipment that no longer exists in that form; what survives intact are its criteria — the material, the duration of contact, the depth of absorption. Where the question outruns the siman, this level says so and stops: halakha lema‘asse goes through your Rav.
This siman does not judge the wine: it judges the vessel. And it measures neither the gravity of the prohibition nor the quantity that ran — it measures what the material drank. Three data settle everything, and they are always asked in the same order.
The Taz fixes in his very first line the exact place of the winepress in the system of vessels, and from there flows all the severity of seif 1:

And the Rambam had already bounded the severity from above:

The eleven seifim, with their text and what must be taken from each in practice.

Seif 1 — the base case. A stone winepress. Two configurations bring it within our siman: a non-Jew coated it with pitch and put wine in after the coating; or a Jew coated it and the non-Jew touched the wine while it still lay moist on the wall. In both: ניגוב, the ashes and water of seif 10.

Treading moves the rung. If the non-Jew trod in a pitched press, scouring no longer suffices. Three paths then, at choice: peel off the pitch and then scour; or a עירוי without peeling — the Shakh specifies the three days of siman 135; or twelve months of rest.

Wood: one regime for three cases. Pitched with wine put in, pitched and then trodden, or touched in משקה טופח — the Mehaber joins them expressly: דין אחד להם. One must peel and scour, or else a עירוי without peeling. Wood therefore knows no extra rung for treading: it is already at the top.
« ואם יש נעורת של פשתן בין נסר לנסר או בלאי בגדים אין די לה בניגוב אלא צריכא מילוי ועירוי או הגעלה » (שו״ע יו״ד קל״ח:א)
And the case of the joints. Flax tow between the boards, or rags — scouring no longer suffices: מילוי ועירוי or הגעלה. The Taz widens the rule to any fissure, even an unstopped one: it is not the tow that aggravates, it is the fissure.

Earthenware, without treading. Here peeling is of no use: אינה ניתרת בקליפה וניגוב. Only מילוי ועירוי — and without any need to peel. This is the clause the Beit Yosef emended in the Tur and the Taz rebuilds otherwise; the Shulchan Arukh rules thus.
« ואם זפתה העובד כוכבים ודרך בה צריכה קליפה ומילוי ועירוי או הגעלה בלא קליפה או יישון י״ב חדש או להסיקה בכבשן עד שירפה הזפת » (שו״ע יו״ד קל״ח:א)
Earthenware that was trodden in: the summit of the siman. Four paths, and they are the four heaviest operations: peeling + מילוי ועירוי; or הגעלה without peeling; or twelve months; or the kiln, until the pitch loosens. The Shakh refers here to siman 134.

And if the vessel is not pitched — the commonest case today, when nothing is pitched any more. Everything then turns on the first use. By a non-Jew: ניגוב; and the Rama adds, for earthenware, a עירוי if he trod in it. By a Jew: a rinsing suffices for wood and stone, but earthenware keeps its ניגוב.

Seif 2 — the masonry winepress. Bricks, lime, gypsum: the law of earthenware. This is the seif that, transposed, bears most directly on the tanks of our day — everything cast, rendered or built of mineral material.

Seif 3 — the key to the whole exercise. Whatever needs a ניגוב may be treated by scalding by way of pouring: boiling water is poured from a primary vessel. And the Mehaber gives the reason in the form of a rule — this scalding shall not be less effective than the scouring.
« גיגית גדולה שדורכין בה והמשפך וכלי המדה והמחץ והוא כלי שדולין בו מהבור לחבית כולם דינם כגת » (שו״ע יו״ד קל״ח:ד)
Seif 4 — the four implements that follow the winepress. The large tub in which one treads, the funnel, the measuring vessel, and the dipper that goes from the pit to the cask. כולם דינם כגת: each according to its own material, but under the winepress’s regime.

Seif 5 — and the two that do not follow it. The press beam and the boards laid on the vat: a rinsing. Wine does not linger in them. The Taz narrows further — the beam needs this rinsing only where it touches the grapes.

Seif 6 — the cords, by their material. Palm frond and hemp: ניגוב. שיפה and גמי absorb more: twelve months. The Rama adds reeds to them, in the name of the Semag.

« ואם רצה לטהרן מיד מגעילן ברותחין או חולטן במי זיתים (דעת הרמב״ם) או מניחן תחת צנור שמימיו מקלחין או במעיין שמימיו רודפין י״ב שעות ואח״כ יותרו » (שו״ע יו״ד קל״ח:ו)
And the three shortcuts. For one who will not wait twelve months: scald in boiling water; steep in olive water; or place under running water for twelve hours. These three paths are the Rambam’s, and the Shulchan Arukh reproduces his wording word for word.

Seif 7 — the wine strainer. Three materials, three degrees, in the exact order of absorption: hair → rinsing; wool → ניגוב; flax → twelve months.
« כלי חלף (פירוש מין עשב שעושין ממנו מחצלאות וחבלים) והוצין וכיוצא בהן מכפיפות שמסננים בהם היין אם היו תפורים בחבלים מדיחן ואם היו אחוזות זו בזו בחיבור קשה צריכים ניגוב ואם היו תפורות בפשתן מיישנן » (שו״ע יו״ד קל״ח:ח)
Seif 8 — the filtering baskets, by their assembly. Sewn with ropes: rinsing. Tightly plaited: ניגוב. Sewn with flax: twelve months. It is no longer the material of the basket that decides, but what holds it together.

The Rama’s gloss — the one domestic case in the siman. A non-Jew’s wine fallen on a tablecloth: a rinsing suffices, even if it was boiling. The Shakh specifies that a cold rinsing is meant, and recalls a heavier requirement for what clings to the cloth.

Seif 9 — the knots. One unties for the ניגוב; one unties neither for the מילוי ועירוי nor for the twelve months. This is the one place in the siman where the Shakh openly contradicts the Mehaber — see section 10.
« כל מקום שאמרנו צריך ניגוב אם הם לחים מקנחן באפר ואח״כ מדיחן במים ואח״כ מקנחן באפר פעם שנית ואח״כ מדיחן במים » (שו״ע יו״ד קל״ח:י)
Seif 10, wet vessel. Ashes, water, ashes, water. Four rounds: one begins by scouring, because wine still remains.
« ואם הם יבשים מקדים מים ואח״כ אפר ואח״כ מים ואח״כ אפר ואח״כ מים » (שו״ע יו״ד קל״ח:י)
Seif 10, dry vessel. Water, ashes, water, ashes, water. Five rounds: one begins by wetting, otherwise the ashes slide. This is the reading the Taz confirms in ס״ק י״ג.
« גת שדרך בה עובד כוכבים כשבא ישראל לנגבה מכבד כדרכו ואין מחייבין אותו ללקט החרצנים » (שו״ע יו״ד קל״ח:י״א)
Seif 11 — the measure of good sense that closes the siman. One sweeps as usual, and is not obliged to gather the soaked pips left at the bottom. The Taz explains why this is no deliberate nullification of a prohibition.

Two shortcuts are added to this ladder, and seif 6 reserves them for the cords: steeping in olive water, and running water for twelve hours. The Shakh extends the second to the whole winepress and its vessels, under one precise physical condition:

The Taz confirms the reading of the printed text and ties it to the Ramah, reported by the Tur:

The Rambam, for his part, holds four rounds in both cases and varies only the order:
« מְדִיחָן בְּמַיִם וּבְאֵפֶר אַרְבַּע פְּעָמִים וְדוֹרֵךְ בָּהֶן. וְאִם הָיְתָה בָּהֶם לַחְלוּחִית מַקְדִּים הָאֵפֶר לַמַּיִם. וְאִם לָאו מַקְדִּים הַמַּיִם » (רמב״ם הלכות מאכלות אסורות פרק י״א הלכה כ׳)

Seif 3 is the seif that makes the siman workable: a vat cannot be immersed anywhere.
The Taz gives the reason, in the name of the Rashba — and that reason holds also for vessels that could be immersed:
« לפי שפליטתן קלה שנבלעו בצונן. רשב״א » (ט״ז יו״ד קל״ח ס״ק ט)

And on the comparison between scalding and pouring, the Taz decides:

This is the most directly transposable passage in the siman, and the only one where a commentator uses a formula of protest.

The Shakh names the source of the Mehaber’s position and rejects it:

All the foregoing is a לכתחילה. The Shakh alone says what happens when the preparation was not done — and his answer changes the colour of the whole siman.

The siman names stone, wood and earthenware — and, in seif 2, everything built of bricks, lime or gypsum, ranked with earthenware. A tank of concrete, cement or mineral render falls naturally under that last description; a wooden vat belongs to the wood column. Metal and plastics do not appear in the siman: their standing belongs to siman 135 and to a Rav’s ruling. What the siman supplies in every case are the questions: did wine linger in it, and was the first use a non-Jew’s?
This is not our siman. The Taz founds the whole severity of the winepress on its resembling a storage vessel without being one; a barrel is one outright, and therefore belongs to siman 135, which is stricter. To confuse the two is to apply a winepress’s leniencies to a jar.
This is the case the Taz treats by name, in ס״ק י״א, adding the decanting tube to the measuring vessel. His reason is mechanical and holds as it stands: the wine gathers in the bend and does not come out with a rinsing. He closes with the siman’s only protest. What matters here, before the method, is dismantling: an elbow that cannot be opened cannot be treated. Ask your Rav.
Seifim 7 and 8 give three regimes according to absorbing power — hair, wool, flax — and two according to assembly — sewn with ropes, tightly plaited. A contemporary filter medium does not appear among them; the question the siman puts is clear: which of these categories does it resemble? For a disposable consumable the question does not arise as it does for a reusable support. Ask your Rav.
This is the Rama’s gloss on seif 8, and the domestic case of the siman: a rinsing suffices, even for boiling wine. The Shakh specifies that a cold rinsing is meant, and reports from the Asheri a heavier requirement — the customary laundering — for what clings to the cloth. The practical distinction therefore runs between a splash and an ingrained stain.
This is the case of the Shakh in ס״ק ד. The duty to kasher stands whole before use; but if the vintage has already been trodden without preparation, the Shakh writes that it is obvious that after the fact the wine is permitted, since there is sixty. He himself sets the limit: a very wide and shallow press. A modern press, low and wide, is exactly the shape he has in view — one more reason not to settle into the after-the-fact. Ask your Rav.
