Siman 201 is not a siman of study only: it is the specification of a structure that is built and watched over. This page sorts what follows from it into three columns — what all hold, what belongs to a custom the decisors support, and what is not decided without the Rav.

This page is not a permission. A mikveh is designed, built, filled and supervised under the authority of a Rav competent in the matter, and its use is settled with the Rav, the balanit and the moret kalla. What follows sets out the line of the sources — their words, their references, and what they leave open. No conclusion on this page constitutes approval of an actual pool.
1. What is held, without dispute
Three conditions, and no one disputes them. All the disagreement of the siman concerns their rank and their remedies, never them.
(1) Forty se’ah, and the whole body at once. Washing does not purify, however abundant:
« אין האשה עולה מטומאתה ברחיצה במרחץ ואפילו עלו עליה כל מימות שבעולם עדיין היא בטומאתה וחייבים עליה כרת עד שתטבול כל גופה בבת אחת במי מקוה או מעיין שיש בהם מ׳ סאה » (שו״ע יו״ד ר״א:א׳) The measure is exact — the least deficiency and the mikveh is invalid, and it does not vary with the size of the body (ש״ך יו״ד ר״א ס״ק ג):
(2) The water must not have been drawn, and must not be in a vessel.
(3) Rainwater purifies only at rest; a spring purifies even while flowing.
And the threshold that governs practice: so long as the forty se’ah are not there, three log of drawn water invalidate; once they are, drawing no longer invalidates
2. Construction — the order of operations, and it cannot be retrieved
What the siman says about the building site holds in four lines, but each decides a design.
Channelling (המשכה). Drawn water that has run over the ground no longer invalidates, provided the majority of the forty se’ah is already valid:
And its surface, according to the Rama, which is the accepted practice:
The order, and this is the mistake that cannot be retrieved. First the valid majority, then the channelling. Channelling the drawn water first into a pool that does not hold its twenty se’ah, and then bringing rain over it, helps nothing — the Rama says so at seif 44, and the Chakh specifies the case at ס״ק צ״ט. Hence a mikveh is filled first from its rain pool and only then from the channelled water.
The materials — what receives impurity. A detached metal pipe is forbidden; sealed to the ground, it is permitted:
This is the rule that governs all the plumbing of a modern mikveh.
The joining — and the clause that decides the design. The hole between two pools must be as wide as the neck of a wineskin, nothing must stand in it, and a doubt about the width invalidates:
« צריך שיהא נקב שביניהם רחב כשפופרת הנאד (וקילוח המים יהיה כרוחב הנקב) (ריב״ש סימן רל״ב בשם א׳ מהמפרשים וכן כתב הבית יוסף) ולאחר שנתערב הפסול עם הכשר אפי׳ רגע נשאר לעולם בהכשרו אפי׳ נסתם הנקב אח״כ כל שיעמוד כשפופרת הנאד ממעטו אפילו דבר שהוא מבריית המים ספק אם הנקב רחב כשפופרת הנאד אם לאו פסול » (שו״ע יו״ד ר״א:נ״ב) The leniency of the hairbreadth, at seif 53, is not the accepted ruling: the Rama writes that it applies only to a rabbinic invalidity — and that even there some require more:
« ודוקא בפסול שאיבה שהיא מדרבנן אבל בפסול דאורייתא בעינן כשפופרת הנאד (ת״ה ור״ש במקואות) כמו שנתבאר ואפילו בפסול שאיבה יש חולקין (טור והרא״ש) וכן ראוי להורות » (שו״ע יו״ד ר״א:נ״ג) And the Arukh HaShulchan gives the reason for stringency in doubt: measures are a law of Moshe from Sinai:
The perforation of a vat already sealed into the ground, and its stopping, are not decided here. The Merkevet HaMishneh and the Be’er Yaakov permit both; Rabbi Akiva Eiger permits the perforation and refuses the stopping; the Arukh HaShulchan reports the same division. A question to bring to the Rav before pouring.
3. The heated mikveh — the line of psak, in three stages
This is the most instructive case in the siman: a prohibition reported, a custom tolerated, and then a declaration so firm that it changes the question.
First stage — the prohibition
« יש מי שאוסר להטיל יורה מלאה מים חמין לתוך המקוה לחממו » (שו״ע יו״ד ר״א:ע״ה) And its reason, in five words of the Taz (ט״ז יו״ד ר״א ס״ק פ״ח):
Second stage — the Rama, and the bounded tolerance:
Third stage — the Arukh HaShulchan, and this is the line on which today’s universal practice rests
And the neighbouring question: washing after the immersion. The Rama reports the prohibition and the custom:
« ולאחר הטבילה במי מקוה כשרים מותרת ליכנס למרחץ כדי שתחמם עצמה (מרדכי) אבל לחזור ולרחוץ אח״כ יש אוסרים (מרדכי בשם רשב״ם) וכן נהגו » (שו״ע יו״ד ר״א:ע״ה) The Arukh HaShulchan gives its source — one of the eighteen decrees, which declared impure one whose head and most of his body came into drawn water — and marks its limit:
What the Arukh HaShulchan writes applies to heating a valid mikveh, not to a pool of hot water standing in place of a mikveh, which seif 75 expressly addresses in its second half. The distinction between the two, in a real installation, is made with the Rav.
4. Immersion in a river, a lake and the sea
The question of places without a mikveh, and the only one of the siman’s great leniencies expressly called a custom.
The sea: seif 5 gives it the status of a spring, and it therefore purifies even while flowing. This is the simplest permission of the siman, and it depends on no custom.
The river: the Mehaber teaches stringency — if the rainwater prevails, an אשבורן is required. The Rama reports the lenient custom, “and one does not protest against those who are lenient, for they have whom to rely upon” (שו״ע יו״ד ר״א:ב), and himself sets the limit: a river formed entirely by rain, which ceases when it does not rain, is excluded.
And the Arukh HaShulchan’s precision, which is the written practice for such places (ערוך השולחן יו״ד ר״א:מ״ב): a true river that does not run dry, and not a pond that dries up — but a pond whose water does not flow does have the status of a mikveh:
Immersion in a river or a lake is not decided by reading a map. The nature of the watercourse, the season, the exact point of immersion and the slope of the bank all enter the question — the Taz at ס״ק ג adds that the slope itself may break the joining. This is a question for the Rav, on site.
5. Maintenance — emptying, cleaning, measuring
The siman does not stop at the day of inauguration. Three of its seifim describe the everyday running of a pool.
Emptying without invalidating: perforate the bucket at its bottom, however slightly, so that it ceases to be a vessel The Bach reports the custom of perforating it “as the entry of a liquid” (ש״ך יו״ד ר״א ס״ק פ״ד):
« כתב הב״ח שנהגו לנקבו ככונס משקה » (ש״ך יו״ד ר״א ס״ק פ״ד) And the conduct the Chakh himself argued (ש״ך יו״ד ר״א ס״ק נ״ז):
And for a spring? The Mehaber writes that it is not needed, a spring not being invalidated by drawing:
The Rama, for his part, reports the custom of stringency even there:
Measuring: a mikveh presumed to diminish is not presumed full, and an immersion made in it must be repeated so long as it is not known that there were forty se’ah (שו״ע יו״ד ר״א:ס״ה and ש״ך יו״ד ר״א ס״ק קל״ה):
And the Mehaber recommends in every case to inspect beforehand:
The Arukh HaShulchan explains why one does not say here “go by the present state”: there are two unfavourable presumptions at once — the woman’s presumption of impurity and the pool’s presumption of diminishing
6. Doubts — those that are eased, and those that are not
The rule is lenient, and it has three exceptions. Knowing them prevents the commonest error on this siman.
The rule: a doubt about drawn water is pure (שו״ע יו״ד ר״א:ס״ז), at its three degrees. But the Chakh sets a condition upon it
The three exceptions.(1) When the invalid is manifest — הפסול מוכיח (seif 70). (2) When the doubt is about the person and not the water: the impure remains in his presumption (seif 71). (3) When a general presumption has already decided: every mikveh that is found is invalid (seif 74):
« כל המקואות הנמצאות פסולין שחזקתם שאובים » (שו״ע יו״ד ר״א:ע״ד) And the Chakh gives the reason (ש״ך יו״ד ר״א ס״ק קמ״ט):
A real doubt about a pool in service — the level, the joining, the origin of the water, an incident during filling — does not belong to these pages but to the Rav, and it is raised before the next use, not after. A person’s presumption of impurity is not lifted retroactively.
7. Table of psak — what is held, what is custom, what goes to the Rav
The same content, arranged for decision. The third column is not a stylistic caution: every line in it is a ma’hloket left undecided in the sources cited.

8. And the Rambam’s word, at the end of הלכות מקואות
Seventy-five seifim of measures, vessels and pipes; and at the end of the treatise that answers to them, the Rambam recalls what it was all about.
Impurities and purities are decrees of Scripture, he says — impurity is not mud or filth that water would carry away. And yet: the matter depends on the intention of the heart. The measures of this siman are therefore neither hygiene nor superstition: they are the conditions laid down so that an act of the will may find its support in the world.
Before digging, before pouring, before opening. Three moments at which the question arises and cannot be retrieved afterwards: the design (joining, channelling, materials), the filling (order of operations, origin of the water), and the operation (measurement, maintenance, supervision). At each of the three, the Rav. And for the woman who immerses: the Rav, the balanit, the moret kalla.
