1. הדין העולה — the line of psak of the siman

The line of psak. The siman comes down to four decisions. (1) A man may prevent his own vows from taking hold, provided he has said so — an inward reservation does nothing. (2) Said in the same breath as the vow, the disavowal prevails. (3) Laid down long before, it works only if he did not remember it while vowing; if he did, he withdrew it by that very vow. (4) And none of this reaches a vow or oath another man imposed on him.
This is the siman whose conclusion readers most often think they know in advance, on account of כל נדרי. What the Shulchan Arukh says about it takes two lines, and the second narrows the first. For your own situation, ask your Rav.

2. ביטול בפה — the annulment must be spoken

What is decided. The annulment follows exactly the rule of the vow: it goes through the lips. A decision taken inwardly, however clear, undoes nothing — הוי דברים שבלב. It is the same symmetry as in the previous siman, where an inward retraction was likewise worth nothing.
Immediate practical consequence: nobody can hold himself covered by a mental reservation. The siman knows only what was uttered.

3. בלחש — the dispute, and the ruling reported

What is settled, and what is not. Settled: the יש אומרים of the seif requires more than speech of any kind. Not settled: what. For the Ba’h and the Taz it is enough that the man heard himself, as with the Shema and prayer. For the Shach, on the Rosh and Rabbenu Yeruḥam, he must have annulled The Ba’er Hetev reports both. And the Pitchei Teshuva reports the Levushei Serad, who brings a proof for the Ba’h and concludes for practice that one may be lenient once he heard himself, even if nobody else did.
This is an open disagreement, and the Pitchei Teshuva inclines it without closing it. The question is one of fact as much as of law: what exactly happened? For application, ask your Rav.

4. תנאי מוקדם — the remembrance that reverses the result

What is decided. The old condition is not permanent armour. It operates only in forgetfulness: a man who vows while remembering that he protected himself withdraws his protection. And the יש אומרים adds a requirement — to recall the condition within the interval after the vow and to rely on it in his heart — of which the Mehaber writes that it must be reckoned with.
The Shach proposes a correction to the text of that requirement, and the Taz writes וצריך עיון about it before the Nekudot HaKesef refute him. In other words: the most demanding clause of the siman is also the least stable. All the more reason not to handle it alone.

5. על דעת ראשונה — the formula that saves the condition

What is decided. A man who remembers having made a condition without knowing what it covered must say, while vowing, The Taz pins the case down: he does remember the condition; it is its object he does not know. Without the formula the vow stands, because vowing while knowing a condition exists amounts to withdrawing it.
This is the one line of the siman that gives a reader something to do. But one must be in the exact case of seif 3, not of seif 2 — and that is the kind of distinction a man does not establish about himself.

6. כל נדרי — what the Shulchan Arukh establishes about it

What the sources say, and nothing more. (1) The declaration of the night of Yom Kippur counts as an express stipulation: it therefore falls under the regime of this siman. (2) Even so, we do not rely on it to permit without a request to a sage, save for a great need. This is the half of the gloss that is forgotten, and it is the half that speaks of practice. (3) The Shach writes that the gloss belongs to seif 2 — that is, to the regime of a condition laid down long before. (4) It applies to nezirut as well, though the word does not appear there. (5) And for an oath taken while holding an object the Shach writes לא אתבריר לן that it avails; the Ba’er Hetev repeats him.
What the sources quoted here do not say, we shall not say. They speak neither of an effect on past vows nor of a general exemption. Any question about what כל נדרי does or does not do in a given case goes to a Rav, and that reservation is written into the gloss itself.

What is decided. A prior disavowal has no purchase on a vow or oath another man imposed, and the reason is given: it is on that other man’s intention that he swears. The seif places the oath on the same footing as the vow — that is a decision, and Rambam shows it was not self-evident.
Of the whole siman this is the most useful line to keep: what is owed to somebody else cannot be annulled in advance. It joins the clause of siman 210 about a vow made על דעת אחרים.

A general declaration made once and for all

It falls under seif 2: it operates only in forgetfulness. Thinking of it at the moment of undertaking withdraws it — and the Shach adds that in seif 1 a disavowal said in the same breath stands even with remembrance.

A reservation kept in thought

Void. Seif 1 says so in so many words.

A reservation whispered

The Shach requires an audience; the Ba’h, the Taz and, for practice, the Levushei Serad reported by the Pitchei Teshuva are content that he heard himself. An open disagreement.

An undertaking made to a person or an institution

Seif 4 excludes it from the whole siman, and the Beit Yosef gives the reason in the name of the Ran and the Tosafot.

What is expected of כל נדרי

The Rama’s gloss says two things about it, and the second is a reservation: we do not rest on it to permit without a sage, save for a great need. The Shach adds the reservation about an oath taken while holding an object.
