1. הדין העולה — the line of psak of the siman

The line of psak. The siman reduces to four decisions. (1) A vow does not take hold of what has no substance — an act, a state. (2) But such a vow nonetheless calls for a rabbinic שאלה. (3) The same intention expressed by naming the limb produces a vow from the Torah. (4) And two particular cases: “I shall study this chapter” stands like a vow of charity, and the conditional vow binds from today when the condition lies tomorrow.
This siman does not apply itself, and for a reason it shows in every line: everything turns on the formula used, word for word, and not on what was meant. Reconstructing from memory what was uttered is precisely what must not be done. For application to your own situation, consult your Rav.

2. מה שאין בו ממש — what a vow does not take hold of

What is decided. Speaking, doing, walking, sleeping, hearing — these are acts, and a vow does not fasten onto them. The Rambam gives the reason in five words: And this remains true when the act is turned into a noun: “my speech” is no more a thing than “to speak.”
But do not turn the rule into a permission: an undertaking may well have been an oath, and the siman has nothing to say about oaths. The first question is always that one. For application to your own situation, consult your Rav.

3. שאלה מדרבנן — what remains due when nothing took effect

What is decided. This is the most practical point of seif 1, and the easiest to miss: a vow that does not take effect still calls to be released, rabbinically, and a door is opened for it as for any vow. The Shach stresses it expressly: the שאלה is due אפי׳ בדבר שאין בו ממש. The seif gives a concrete case, from the Rashba: one who forbade himself to hear another’s prayer.
The Shach, at ס״ק ב, notes what depends on the rank of the vow — notably the penalty of days of siman 208, which is due only for a Torah vow. In other words: saying “it is only rabbinic” is not saying “it is nothing,” but it does change something. For application to your own situation, consult your Rav.

4. לשון האבר — the formulation that binds from the Torah

What is decided. As soon as the formula names the mouth, the hands, the feet or the eyes, the vow takes effect from the Torah, with all the consequences of a full vow. And if the prohibition is borne explicitly on the limb — “let my mouth be forbidden to me for my speech” — it is clearer still: the Taz explains that there is no longer any doubt to settle.
A Torah vow is not handled like a rabbinic one: its release falls under the ordinary rules of vows, and the penalty of days of siman 208 may come into play if it was transgressed. For application to your own situation, consult your Rav.

5. הגהת הרמ״א — the intermediate case, and what is not decided there

The point, and it is not settled. Between the bare act and the named limb, the Rama opens a third case: the act named with its object — “the speech of my mouth,” “the taking of a stone.” He reports it as a י״א, not as a decision, and the Shach gives the reason: since he mentioned the object. The gloss does not say that this is the halakha.
This is exactly the kind of formulation one does not assess oneself. The distance between “I forbid myself to speak” and “the speech of my mouth is forbidden to me” is one word, and it separates a case with no vow from a disputed one. For application to your own situation, consult your Rav.

6. אשנה פרק זה — the undertaking of study and of mitzvah

What is decided. Seif 2 takes a declaration of intent to study and makes of it an undertaking that stands — not as a vow of prohibition, but The Shach, at ס״ק ז, extends the case to all the mitzvot and refers to siman 258; the Ba’er Hetev repeats it.
Two remarks. First, the regime is that of charity, which sends one to siman 258 and not to the rules of prohibition. Second, the Tur and the Ran do not construct the case in the same way — a full vow for one, an oath for the other — and that may bear on practice. For application to your own situation, consult your Rav.

7. נדר שוא — when the vow does not take effect at all

What is decided. The Shach lays down a condition prior to all the rest: a vow on sleep holds only if a duration was stated, and he gives the order of magnitude — a day or two. Without a duration, or for three days, it is a נדר שוא: it does not take effect at all. He refers to siman 236.
Siman 236 is the proper place of this matter, and the Shach refers to it expressly. An outsized undertaking is therefore not “a difficult vow”: it may be no vow at all — which is not concluded on one’s own. For application to your own situation, consult your Rav.

8. תנאי — the conditional vow

What is decided. When the condition is in the future and the prohibition immediate, one abstains from now: it is feared he will forget his condition and fulfil it. When the prohibition is in the future and the condition immediate, he may act today: it is not feared that he will forget a prohibition. The Taz gives the phrase.
The direction of the condition — which of the two clauses lies in the future — decides everything, so the exact formula must be established before any reasoning. The Shach, at ס״ק ח, refers to simanim 220 and 239 for the continuation of the matter. For application to your own situation, consult your Rav.

9. מה שאין בסימן זה — what is not found here, and what will not be written

What is not in this siman. The Nekudot HaKesef write nothing on this siman; they are read on the neighbouring simanim — 209, 210, 211, 215. The Taz is present here with three ס״ק, all cited at level 1. As for the Aruch HaShulchan, it is not accessible to us for this siman; none of its words is therefore reproduced here. None of this has been reconstructed: an entry that does not exist is not replaced by a paraphrase.

“I forbade myself to speak to him”

The first question is not halakhic but factual: which formula exactly? קונם שאני מדבר עמך and קונם פי מדבר עמך sound alike and do not produce the same result. And if it was an oath, this siman does not apply. For application to your own situation, consult your Rav.

“I forbade myself to hear so-and-so’s classes”

This is the very case of the seif, taken from the Rashba: it is not a Torah vow, and a rabbinic שאלה is due. The seif does not say “there is nothing to be done”; it says what rank what was done has. For application to your own situation, consult your Rav.

“I announced I would finish this tractate”

Seif 2 deals with exactly this, and gives it the regime of charity — which sends one to siman 258. The Shach extends it to the other mitzvot. It remains to know whether the words spoken were an intention or an undertaking: a question of fact, then a question for a Rav. For application to your own situation, consult your Rav.

A resolution for a period

“I shall not do that again until such a date.” At least three things must be established: vow or oath; whether the formula used named a thing; and, if there is a condition, which way it runs. None of them is settled from memory. For application to your own situation, consult your Rav.

An outsized undertaking

The Shach, at ס״ק ה, warns: beyond a certain measure it is no longer a difficult vow but a נדר שוא, which does not take effect at all — and he refers to siman 236. That is a door that does not open of itself. For application to your own situation, consult your Rav.
