The purpose of the public reading of the Torah is that the community hear it read with exactness, as it was given. Hence the מחבר rules: קרא וטעה אפי׳ בדקדוק אות אחת מחזירין אותו — an error, even in the precise reading of a single letter, requires going back. The רמ״א defines the measure: one goes back only when the error changes the meaning. We study here at the level of principle; for personal conduct, one refers to the custom and to the ruling of the Rav.

מִי שֶׁקָּרָא וְטָעָה — one who reads the Torah and errs — the principle: the reading must be exact. An error of דקדוק (the precise reading of the letters) that changes the meaning (שנוי שמשתנה) is a bar: מחזירין אותו, we make him go back and re-read correctly. But a fault of נגינת הטעם (the cantillation melody) or ניקוד (vocalization) that does not change the meaning — אין מחזירין אותו אבל גוערין בו, we do not make him go back but we rebuke him. Hence also the case of the ישוב where no one can read precisely: they read nonetheless בברכה כהלכתה.
Séif 1 — קרא וטעה: an error that changes the meaning
קרא וטעה אפי׳ בדקדוק אות אחת מחזירין אותו. הגה: וכן דין החזן הקורא, ודוקא שנוי שמשתנה ע״י זה הענין, אבל אם טעה בנגינת הטעם או בניקוד אין מחזירין אותו אבל גוערין בו. (ב״י ופסקי מהרי״א סי׳ קפ״א).
Séif 1: one who read and erred, even in the precise reading (בדקדוק) of a single letter — we make him go back (מחזירין אותו). Rama: and such is also the law for the reader (the חזן); and this specifically means a change that alters [the meaning] through this matter (שנוי שמשתנה), but if he erred in the cantillation melody (נגינת הטעם) or in the vocalization (ניקוד) — we do not make him go back, but we rebuke him (גוערין בו).
Two levels of error. A fault of דקדוק — the precise reading of the letters — that changes the meaning of the word: מחזירין אותו, we make him go back to the word and read it correctly, for the Torah must be heard as it is. But a fault that does not touch the meaning — a misplaced טעם melody or an inexact ניקוד with no change of meaning — does not require going back: גוערין בו, we only rebuke him, that he read with care.
Séif 2 — ישוב שאין בו מדקדק: reading with the blessing nonetheless
וישוב שיש שם מנין ואין מי שיודע לקרות בתורה כהלכתה בדקדוק ובטעמים, אפ״ה יקראו בתורה בברכה כהלכתה. ומפטירין בנביא, ועיין בסמוך ריש סי׳ קמ״ד.
Séif 2: and a place (ישוב) that has a מנין but no one who knows how to read the Torah as required (כהלכתה), with precision (בדקדוק) and with the cantillation (ובטעמים) — even so (אפ״ה), they read the Torah with the blessing, as required (בברכה כהלכתה). And they read the haftara from the prophet (ומפטירין בנביא), and see nearby, at the beginning of siman קמ״ד.
The absence of a fully expert reader does not annul the public reading. A community that has a מנין but no one to read with full precision (דקדוק) and cantillation (טעמים) still reads the Torah with the blessing, as required — and also reads the haftara from the prophet. The mitzva of the Torah reading is thereby fulfilled.

Practical cases
Case 1 — An error that changes the meaning
Situation: the reader (the חזן) reads a word changing a letter or a vocalization, and this alters the meaning of the word. What to do?
Conduct: since the meaning is altered (שנוי שמשתנה), מחזירין אותו — we make him go back, re-read the word correctly and continue. The same rule applies to the person called up and to the baal koreh. For the concrete application (which words precisely), one follows one's Rav.
Case 2 — A fault of טעם or ניקוד
Situation: the reader errs in the cantillation melody (נגינת הטעם) or a ניקוד, but without changing the meaning. Must he go back?
Conduct: as long as the meaning is not altered, אין מחזירין אותו — we do not make him go back. We rebuke him (גוערין בו) that he read with more care. For borderline cases (a cantillation that would itself change the meaning), one follows one's Rav.
Case 3 — A community with no expert reader
Situation: a place has a מנין, but no one knows how to read the Torah with full precision and cantillation. Does one give up the reading, or the blessing?
Conduct: one does not give up. אפ״ה יקראו בתורה בברכה כהלכתה — they read the Torah with the blessing, as required, and ומפטירין בנביא (haftara). The public reading is fulfilled despite the absence of a perfect reader. For the practical arrangement, one follows one's Rav (see also the beginning of siman 144).
