The universal principle of Siman קס״ט:

This siman (ג׳ סעיפים) governs conduct toward the שמש — the one who serves the table without sitting to eat with the diners. A single thread runs through it: the honor due to the servant (חסד ודרך ארץ) tempered by the honor due to Heaven (מוראת שמים). Hence the whole conduct: one gives him at once from any fragrant dish he craves — כל דבר שמביאין לפני האדם שיש לו ריח והאדם תאב לו צריך ליתן ממנו לשמש מיד — מפני עגמת נפש, and it even touches on danger — הכל משהין בפני השמש חוץ מבשר ויין (כתובות ס״א ע״א); but one gives him bread only to one who washed his hands and will bless — לא יתן לאכול אלא למי שיודע בו שיברך; and he himself blesses over each cup, since he is כנמלך.

⚖ The structure of the siman at a glance

The whole siman governs conduct toward the shamash. Hence (①) the duty to give him at once and from each kind, מפני עגמת נפש — צריך ליתן ממנו לשמש מיד ; (②) the limit of מוראת שמים — bread only to one who washed and will bless — לא יתן לאכול אלא למי שיודע בו שיברך ; and (③) the shamash's blessings — over each cup (כנמלך) and joining the zimoun — לפי שהוא כנמלך. 📚 The yesod of giving to the shamash: the shamash sees and smells the dishes and craves them; sparing him this עגמת נפש (distress of soul) is a duty of derekh eretz, and it even touches on danger — the Gemara (כתובות ס״א ע״א) says הכל משהין בפני השמש חוץ מבשר ויין: one may make the shamash wait for anything, except fat meat and wine, which are given him at once. So the Mehaber: from any fragrant dish he craves צריך ליתן ממנו לשמש מיד, and it is מדת חסידות to give him מכל מין ומין. One hands him nothing while the cup is in the blesser's hand (so as not to disturb the berakha), and — the limit of מוראת שמים — one gives him bread only if one knows he washed his hands: ואסור ליתן לו פרוסת פת אלא אם כן נטל ידיו, so as not to feed him without netilat yadayim.

📚 The yesod of "to one who will bless": one gives food to eat only to one known to bless — לא יתן לאכול אלא למי שיודע בו שיברך — for giving to one who will not bless is like causing him to stumble (eating without a berakha) and letting the honor of Heaven be profaned at one's table. Nonetheless, some are lenient when giving to a poor person by way of צדקה — ויש מקילין אם נותן לעני בתורת צדקה — for that is not an immediate feeding but a gift of charity, which one does not withhold from the poor for fear he may not bless.

For bread, all depends on settled intent: if there is an adam chachouv at the meal from whom he knows he will be given all he needs, his mind is settled and he does not re-bless — ואינו צריך לברך על כל פרוסה ופרוסה אם יש אדם חשוב בסעודה ; otherwise bread is like wine — ואם אין אדם חשוב בסעודה צריך לברך על כל פרוסה ופרוסה כמו על היין. Finally the Rama: when two eat together (with no obligation of zimoun), the shamash eats with them even without asking permission, so that they be three and join for the zimoun — שנים שהיו אוכלין ביחד השמש אוכל עמהם בלא נטילת רשות כדי שיצטרפו לזימון (see siman ק״ע:כ״א).
