After the previous siman, which dealt with what constitutes an interruption (הפסק) of the meal, the Choulhan Aroukh devotes this siman to what constitutes a lapse of attention (היסח הדעת). The core idea: as long as one is « in » the meal, a single blessing covers everything; but once one marks the end of the meal — by doing the final waters (מים אחרונים), by announcing « הב לן ונברך », or by taking the cup of the Birkat haMazon — one has « detached » from the table, and must bless again before eating or drinking. The siman clarifies the case of the guest who depends on the host, that of cups handed by several groups, of fruits brought successively, and closes on the practice of salt and water. We study here at the level of principle; for personal conduct, one refers to the ruling of the Rav.

A. גמר סעודה והיסח הדעת — the end of the meal and the lapse of attention
הֶסֵּחַ הַדַּעַת — a « lapse of attention ». As long as one is engaged in the meal, the initial blessing covers everything one eats and drinks. But as soon as one marks the end of the meal — by doing the final waters (מים אחרונים), by announcing that one is about to bless, or by taking the cup of the Birkat haMazon — one has « detached »: one must bless again before resuming.
Seif 1 — גמר סעודה, מים אחרונים and « הב לן ונברך »
גמר סעודתו ונטל ידיו מים אחרונים אינו יכול לאכול ולא לשתות עד שיברך ברכת המזון ואם אמר הב לן ונברך הוי היסח הדעת ואסור לשתות אלא אם כן יברך עליו תחילה ואכילה דינה כשתיה להרא״ש אבל להר״ר יונה והר״ן אכילה שאני שאע״פ שסילק ידו מלאכול ואפילו סילקו השלחן אם רצה לחזור לאכילתו אין צריך לברך פעם אחרת שכל שלא נטל ידיו לא נסתלק לגמרי מאכילה:
Seif 1: if he finished his meal and washed his hands [with] the final waters (mayim acharonim), he may not eat or drink until he recites the Birkat haMazon. And if he said « Hav lan unevarech » (give us [the cup] and we shall bless), this is a lapse of attention, and he may not drink unless he first recites a blessing over it. And eating has the same law as drinking according to the Rosh; but according to Rabbeinu Yonah and the Ran, eating is different — for even though he withdrew his hand from eating, and even if the table was removed, if he wishes to return to his eating he need not bless a second time, since as long as he has not washed his hands he has not fully withdrawn from the meal.
Three ways to mark the end. The meal ends — and attention « detaches » — either by an act (the final waters, מים אחרונים) or by words (announcing « הב לן ונברך »). Then, to drink again, one must re-bless. As for eating, the Rishonim differ: the Rosh equates eating with drinking (re-bless too), while Rabbeinu Yonah and the Ran hold אכילה שאני — as long as one has not washed his hands, one has not fully left the meal.
B. סומך על שלחן אחרים — the guest at another's table
Seif 2 — עד שיאמר בעל הבית : the attention follows the host
מי שסומך על שלחן אחרים אפי׳ אמר הב לן ונברך לא הוי היסח הדעת עד שיאמר בעל הבית:
Seif 2: one who depends on another's table (סומך על שלחן אחרים) — even if he said « Hav lan unevarech », it is not a lapse of attention, until the host says so.
The guest is not the master of the meal. Since the guest depends on the host, his attention is subordinate to the host's: it is not his own word that ends the meal, but the host's. As long as the baal habayit has not announced that they will bless, the guest has not detached, and may still drink without re-blessing.
C. נטל הכוס לברך — taking the cup of the Birkat haMazon
Seif 3 — משנטל הכוס לברך : the act that marks the end
אם לא אמר הב לן ונברך וגם לא נטל ידיו משנטל הכוס לברך הוי היסח הדעת:
Seif 3: if he did not say « Hav lan unevarech » and also did not wash his hands — once he has taken the cup to bless [the Birkat haMazon], it is a lapse of attention.
The gesture is as good as the announcement. Even without having said « הב לן ונברך » and without having done the final waters, the mere act of taking the cup meant for the Birkat haMazon shows that one has moved to the close of the meal. It is therefore, on its own, a היסח הדעת: to drink afterward, one will have to re-bless.
D. חבורות ונמלך — cups handed and the נמלך
נִמְלָךְ — one who « reconsiders ». When the arrival of a cup is unforeseen (one did not plan to drink it), the preceding blessing did not cover it: each new cup is a case of נמלך and requires its own blessing (see also siman קע״ד).
Seif 4 — כי בכל פעם הוא נמלך : several groups, several blessings
כשאדם נכנס לבית חבירו ויש שם חבורות הרבה שאוכלים וכל אח׳ מושיט לו כוס יש מי שאומר שמברך על כ״א בורא פרי הגפן כי בכל פעם הוא נמלך:
Seif 4: when a person enters his friend's house and there are many groups eating, and each one hands him a cup — there is one who says that he recites over each « בורא פרי הגפן », for each time he is nimlach (reconsidering).
Each unexpected cup is blessed. Since one did not intend to drink these cups, each cup handed by a new group is a נמלך — the preceding blessing did not encompass it. According to this view, one recites « בורא פרי הגפן » each time. For practice (how far this din reaches), one follows one's Rav.
E. פירות בזה אחר זה — fruits brought one after another
Seif 5 — אינם צריכים לברך אלא על הראשון : one blessing
הקרואים בבית בעה״ב לאכול מיני פירות ומביאין להם בזה אחר זה אינם צריכים לברך אלא על הראשון:
Seif 5: guests in the house of the host [invited] to eat kinds of fruit, and they are brought to them one after another — they need only bless on the first.
Here there is no reconsidering. Unlike the cups handed to him (seif 4), the guests knew from the start that they had come to eat fruits: the successive arrival was foreseen. Their intention therefore covers all that will come, and one blessing, on the first fruit, suffices — this is not a נמלך.
F. מלח ומים אחר הסעודה — salt and water after the meal
Seif 6 — מלח ומים : a matter of health
אכל כל מאכל ולא אכל מלח שתה כל משקה ולא שתה מים ביום ידאג מפני ריח הפה ובלילה מפני ריח הפה ומפני אסכרה (פי׳ חולי חונק) והאוכל מלח אחר אכילתו לא יאכל בגודל (גודל הוא האצבע הגס. אצבע הוא קרוב לגס. אמה הארוך. קמיצה הוא הסמוך לקטן. זרת הוא הקטן) דקשה לקבור בנים ולא בזרת דקשה לעניות ולא באצבע דקשה לש״ד אלא באמה ובקמיצה (וע״ל סי׳ ק״ע למה אין אנו נוהגים באכילת מלח ושתיית מים):
Seif 6: one who ate a food without eating salt, or drank a beverage without drinking water — by day he should worry about bad breath, and by night about bad breath and about askara (a choking illness). And one who eats salt after his meal should not eat it with the thumb — for this is harmful, [risking] burying children — nor with the little finger — for this is harmful for poverty — nor with the index finger — for this is harmful (danger of ש״ד) — but with the middle finger and the ring finger (cf. above siman ק״ע, why we no longer practice eating salt and drinking water).
An ancient custom, fallen out of use. The Choulhan Aroukh reports the old custom of eating salt and drinking water after the meal, with health warnings and a precise way of taking the salt (with the middle and ring fingers). The Mechaber himself refers to siman ק״ע to explain why we no longer keep this custom today. For practice, one follows one's Rav.
The siman in one sentence: as long as one is « in » the meal a single blessing covers everything, but as soon as one marks its end — by the final waters, by announcing « הב לן ונברך », or by taking the cup of the bench — it is a היסח הדעת that requires one to bless again; the guest follows the host, the cups handed unexpectedly are each a נמלך while the fruits that were foreseen suffice with one blessing on the first, and the siman closes on the old practice of salt and water.
Practical cases
Case 1 — I said « Hav lan unevarech », then a drink is brought
Situation: at the end of the meal I already announced « הב לן ונברך », and now another beverage is brought. May I drink it without re-blessing?
Conduct: no — saying « הב לן ונברך » is a היסח הדעת (seif 1): to drink, one must re-bless first. As for eating, it depends on the Rosh / Rabbeinu Yonah–Ran dispute (seif 1), and one follows one's Rav. The best guidance: decide clearly when you are « in » the meal and when you have closed it.
Case 2 — I am a guest at someone else's table
Situation: I am a guest of a host (סומך על שלחן אחרים); I said « let us bless », but the host announced nothing. Have I already « detached » from the meal?
Conduct: no — for the guest it is not his own word that counts but the host's: עד שיאמר בעל הבית (seif 2). As long as the host has not announced the bench, the guest has not detached and may still drink without re-blessing. In practice, one refers to one's Rav.
Case 3 — Several people each hand me a cup
Situation: I enter a friend's house where several groups are eating, and each unexpectedly hands me a cup. Must I bless on each cup?
Conduct: according to the view cited (seif 4), yes — each unexpected cup is a נמלך and one recites « בורא פרי הגפן » each time. This is the opposite of the foreseen fruits brought one after another (seif 5), where one blesses only on the first. In practice, one follows one's Rav.
