This siman teaches the places where it is forbidden to say the ק״ש and words of holiness: one does not walk in the מבואות המטונפות (filthy alleyways) placing his hand over his mouth to recite ; even one who had already begun pauses (מפסיק) upon reaching the alleyway ; on coming out he resumes at the point of interruption (מחבר) or at the beginning (רמ״א) ; and in a place of real filth (בית הכסא, מרחץ, צואה ומי רגלים), even the thought (הרהור) of Torah is forbidden. Our Sages taught (ברכות כ״ה.) from the verse והיה מחניך קדוש ולא יראה בך ערות דבר (דברים כ״ג) that words of holiness require a clean place. Hence: one who passes through a filthy place does not say the ק״ש ; and in a place of real filth even the thought (הרהור) of Torah is set aside. We study here at the level of principle; for personal conduct, one refers to the custom and to the ruling of the Rav.

A. Forbidden places for the ק״ש — the ב׳ seifim
מְבוֹאוֹת הַמְטֻנָּפוֹת — filthy alleyways — the principle (דברים כ״ג: « ולא יראה בך ערות דבר ») is that words of holiness require a clean place. We distinguish דיבור (spoken words) from הרהור (mere thought): in a filthy alleyway one does not articulate the ק״ש (séif 1); and in a place of real filth (bathroom, bathhouse, a place with צואה ומי רגלים) even the הרהור of words of Torah is forbidden (séif 2), whereas secular matters and the qinuyim are permitted there.
Séif 1 — מבואות המטונפות: the filthy alleyway and the pause
לא ילך אדם במבואות המטונפות ויניח ידו על פיו ויקרא קריאת שמע, ואפילו אם היה קורא ובא — צריך להפסיק כשיגיע למבוי המטונף, וכשיוצא משם אפילו שהה כדי לגמור את כולה אינו צריך לחזור אלא למקום שפסק. הגה: ויש אומרים שחוזר לראש, וכן עיקר [ועיין לעיל סימן ס״ה].
Séif 1: one does not walk in filthy alleyways (מבואות המטונפות) placing his hand over his mouth to recite the ק״ש. And even one who was reciting and comes there (היה קורא ובא) — he must pause (צריך להפסיק) upon reaching the filthy alleyway. And when he comes out from there, even if he delayed long enough to complete the whole of it (כדי לגמור את כולה), he resumes only at the point where he paused (למקום שפסק). Rama: and some say that he returns to the beginning (חוזר לראש), and that is the essential ruling [see above סימן ס״ה].
The hand over the mouth is no permitted device: one cannot say the ק״ש in a filthy alleyway by muffling his voice. And one who reaches it mid-reading pauses (מפסיק). On coming out: according to the מחבר, he resumes only at the point of interruption; but the רמ״א rules that he returns to the beginning (חוזר לראש), וכן עיקר — as in siman ס״ה on interruption.
Séif 2 — הרהור בד״ת: the place of real filth
אפילו להרהר בדברי תורה אסור בבית הכסא ובבית המרחץ ובמקום הטינופת, והוא המקום שיש בו צואה ומי רגלים. דברים של חול מותר לאמרם שם בלשון הקודש, וכן הכינויים כגון רחום נאמן וכיוצא בו מותר לאמרם שם, אבל השמות שאינם נמחקין אסור להזכירם שם.
Séif 2: even to think (הרהור) words of Torah is forbidden in the bathroom (בית הכסא), the bathhouse (בית המרחץ) and the place of filth (מקום הטינופת) — that is, a place where there is excrement and urine (צואה ומי רגלים). Secular matters (דברים של חול) may be said there even in the holy tongue; likewise the qinuyim (כינויים) such as רחום, נאמן and the like may be said there; but the Names that may not be erased (השמות שאינם נמחקין) — it is forbidden to mention them there.
In a place of real filth, the stringency rises a notch: not only does one not say words of Torah, but even to think them (הרהור) is forbidden. By contrast, secular matters and the qinuyim (רחום, נאמן — attributes also used for people) are permitted there; only the Names that may not be erased are barred from that place.
The siman in one sentence: one does not say the ק״ש in a filthy alleyway, even with the hand over the mouth, and one who reaches it while reading pauses (on coming out: למקום שפסק per the מחבר, חוזר לראש per the רמ״א, וכן עיקר); and in a place of real filth (bathroom, bathhouse, צואה ומי רגלים) even the הרהור of Torah is forbidden, whereas the כינויים and secular matters are permitted there, but not the Names that may not be erased.
Practical cases
Case 1 — Passing through a filthy alleyway while reciting the ק״ש
Situation: he is reciting the ק״ש while walking and reaches a filthy alleyway. May he continue by placing his hand over his mouth?
Conduct: no — the hand over the mouth is no solution; he must pause (מפסיק) upon reaching the filthy alleyway and not utter the ק״ש there. For the practical application, one follows one's Rav.
Case 2 — Where to resume on coming out
Situation: he paused, passed the alleyway, and comes out. Where does he resume his reading?
Conduct: according to the מחבר, even if he delayed long enough to complete the whole reading, he resumes only at the point where he paused (למקום שפסק). According to the רמ״א, וכן עיקר, he returns to the beginning (חוזר לראש), as in siman ס״ה. To rule le-maaseh, one follows one's Rav.
Case 3 — Thinking Torah in the bathroom or a filthy place
Situation: in the bathroom or a place where there is filth, may he at least think words of Torah?
Conduct: no — even the mere thought (הרהור) of Torah is forbidden in a place of real filth (צואה ומי רגלים). He may say there secular matters and the qinuyim (רחום, נאמן), but not the Names that may not be erased. For borderline cases, one follows one's Rav.
