The siman rests on two sentences, and the second mirrors the first. Seif 1 looks at a Jew who has borrowed from a non-Jew: may another Jew sign for him? Seif 2 reverses the scene — the non-Jew is the borrower, from a Jew this time, and again a Jew is asked to stand surety. The Rama’s two glosses, wildly unequal in length, carry the entire practical weight of the siman.

We picture interest as a surcharge paid by the borrower to the lender. Here nobody pays anything extra: a third man signs, and that is all. The prohibition arises from a detour, and the detour is a rule of procedure, not of finance.
Non-Jewish law, as the Mehaber describes it, claims the surety before the borrower. The surety therefore pays first — principal and interest — and then turns on the borrower to recover what he laid out. But what he laid out includes interest. He is thus claiming interest from a Jew, and claiming it in his own name.

Everything else follows. If the non-Jewish lender undertakes not to claim the surety first, the detour vanishes: the surety is nothing but a surety, and no interest passes from Jew to Jew.

The source is a baraita reported in chapter איזהו נשך. It reads the verse about the גר תושב and derives from it a permission, seemingly very wide:

The gemara immediately asks: surety for whom? If for a Jew, the mishna already said the surety transgresses. So it is for a non-Jew — and here arises the difficulty out of which our siman is born.

Rav Sheshet’s answer is a distinction of forum: the non-Jew accepted to be judged by Jewish law for the procedure, and not for the interest.

The Rambam restates this sugya almost word for word, and it is from him that the Mehaber takes his wording.

Two seifim, and thirty-six entries of apparatus: ten ס״ק of the Shach, eight of the Taz, ten of the Baer Hetev, three of the Pit’hei Teshuva and five of the Nekudot HaKesef. It is there, not in the text, that this siman lives.
The Shach broadens the Mehaber’s permission: it holds not only for a lump-sum loan but even for an explicitly agreed monthly interest.
