1. הדין העולה — the line of psak of the siman

The line of psak. The siman comes down to four decisions. (1) Nothing is vowed so long as nothing was said: an inward resolve, however firm, does not bind. (2) If the word said has nothing in common with the word thought, there is no vow on either. (3) If the word said contains the word thought, the vow holds on what they share. (4) And the retraction, like the vow, must go through the mouth, and within a very short interval counted from the vow.
This siman is almost entirely made of facts: what was said, what was meant, whether the word slipped out or was chosen, how much time passed. None of these is reconstructed in good faith after the event without a third party. For your own situation, ask your Rav.

2. הוצאה בשפתיים — thought alone does not bind

What is decided. A resolve taken within oneself, however complete — “I shall not eat today”, and he holds the thing forbidden to him by oath — produces no prohibition so long as it is not uttered. Rambam writes it out, and the Taz brings the rule back to its verse in three words.
This is the gateway to the whole subject, and it works both ways: it protects the man who merely promised something to himself, and it warns the man who thinks he can get out of it by saying he did not mean what he said.

3. פיו ולבו שוין — the word that is not the one he meant

What is decided. Nothing takes hold, neither on the thing said nor on the thing thought. The Shach names the two missing halves in one line, and Rambam gives the case in another form: a man who meant to swear not to eat at Reuven’s and swore about Shimon is bound to neither.
The delicate point is not the rule but establishing the fact. That the mouth slipped is easy to claim after the event. This is exactly the case in which a man does not judge himself.

4. פת סתם — the word that is too wide, and the point of disagreement

What is settled, and what is not. Settled: a wide word that came out inadvertently binds on what it shares with the word thought — the wheat alone, if wheat was in mind. Not settled: a wide word said deliberately. The Ba’h, followed by the Shach and the Maharshal, then forbids all bread, because things of the heart are not things. The Beit Yosef had not read it so. The Chavot Yair would incline to permit the barley even there, but does not dare rule against so many authorities.
An open disagreement, and it changes the whole outcome: on one view a single kind of bread is forbidden; on the other, all of them. It goes to a Rav, and the first thing he will establish is whether the general word was chosen or slipped out.

5. על דעת אחרים — when one may no longer plead one’s own intention

What is decided. A man who vowed or swore while expressly saying that he does so by the mind of others may no longer say afterwards, “this is what I had in mind”. The reason is a formula of Rambam’s that the Shach copies:
This is the most practical clause of seif 1, and the least known. It explains why oaths taken before a court are taken by the mind of the court: the formula shuts the door on mental reservation.

6. נדר בחלום — the law, the practice, and what remains open

Three levels, and they must be kept apart. (1) The law: a vow in a dream is nothing, and needs no release at all. (2) The opposing view: ten men who know how to read release him, by simple regret. The Mehaber requires that this be reckoned with, and the Shach writes והכי נהוג and then וכן נוהגין. The Rama provides for three men where ten cannot be found. (3) What remains open: the Taz found a responsum of the Maharal of Prague ruling הלכה למעשה like the first view; and on a woman, the Ba’h and the Taz differ openly.
One point the Pitchei Teshuva adds, and it is not a nuance: when the dream concerns a mitzvah — having dreamt that one undertook to pay a debt — the Radbaz writes that the matter no longer belongs to the law of vows at all. It is therefore not a case for release: it is a case for doing the thing.

7. חזרה תוך כדי דיבור — the interval and where it starts

What is decided. Two ways of undoing a vow on the spot: to take it back oneself, or to accept another man’s protest. Three conditions: that the retraction be spoken, that it fall within the interval — the time to say שלום עליך רבי — and that the interval be counted from the vow. The last point is the Rama’s gloss, and both the Taz and the Shach uphold it against the Beit Yosef.
In practice the interval is of the order of a second. Which is to say it exists only for a man who takes it back in the same breath — and that the ordinary case, regret arriving later, belongs to release by a sage, not to this seif.

A resolve taken without being spoken

It has not the force of a vow. That does not mean it has no moral weight, but the regime of this siman does not apply to it.

A word that outran the thought

The vow holds on what the two share. And if the general word was chosen deliberately, that is the dispute of § 4 — it needs a Rav.

An undertaking made before a beit din

The clause על דעת אחרים removes mental reservation: their intention fixes the scope.

Having dreamt that one vowed

By law: nothing. The practice reported by the Shach requires ten men; the Rama allows three where ten are lacking. And if the dream concerned a mitzvah, the Radbaz treats the matter otherwise — not as a vow.

Taking it back at once

It must be spoken, and within the time of a greeting counted from the vow. After that it is no longer a retraction but a request for release.
