1. The axiom: two conditions, not one

The axiom. A vow is born neither of speech alone nor of intention alone, but of their meeting. The seif says it in one sentence: he must have articulated it with his lips, and his mouth and heart must be alike. The whole siman consists in examining what happens when the two do not coincide exactly.
« אין הנדר חל עד שיוציא בשפתיו ויהיו פיו ולבו שוים » (שו״ע יו״ד ר״י:א)
« שיוציא בשפתיו. שנאמר לבטא בשפתים » (ט״ז יו״ד ר״י ס״ק א)
Three consequences. (1) Speech without matching intention: nothing. (2) Speech wider than the intention: the vow holds on the intersection. (3) Neither speech nor intention — the dream: nothing by the letter, but the Mehaber requires it to be reckoned with, and the Shach writes that such is the practice.

2. The three reflex questions

1. Did the word come out? No — nothing, whatever the intention was. Yes — next question.
▼
2. Does the word said contain the word thought? No — nothing, on either. Yes — the vow holds on what the two share. And if the wide word was chosen deliberately, see the dispute.
▼
3. Did he retract? Aloud, and within the interval counted from the vow — the vow is undone. Inwardly, or too late — it stands.

3. Master table — the three seifim clause by clause

SeifThe clause of the Shulchan ArukhWhat it decides
א« אין הנדר חל עד שיוציא בשפתיו ויהיו פיו ולבו שוים » (שו״ע יו״ד ר״י:א)The two conditions of a vow: speech, and agreement of mouth and heart
א« לפיכך היה בלבו לידור מפת חטים והוציא בשפתיו פת שעורים מותר בשניהם » (שו״ע יו״ד ר״י:א)A word other than the thought: nothing takes hold, on either
א« אבל אם היה בלבו פת חטים (או פת שעורים) והוציא בשפתיו פת סתם » (שו״ע יו״ד ר״י:א)A word wider than the thought: the reversed case
א« אסור בשל חטים לבד (או בשל שעורים לבד) (דכן משמע בסימן רי״ז ס״ט » (שו״ע יו״ד ר״י:א)The vow holds on what the two have in common
א« ואם נדר על דעת אחרים אינו תלוי בלבו אלא כפי מה שיאמרו אחרים שהוא דעתם כך יחול הנדר » (שו״ע יו״ד ר״י:א)A vow made on the mind of others: their heart replaces his
ב« הנודר בחלום אינו כלום ואינו צריך שאלה » (שו״ע יו״ד ר״י:ב)A vow pronounced in a dream is nothing, and needs no שאלה
ב« ויש אומרים שיתירו לו עשרה דידעי למקרי (ויתירו לו בחרטה כאלו נדר בהקיץ) (תשובת הגאונים » (שו״ע יו״ד ר״י:ב)The opposing view: ten who know how to read release him, by regret
ב« ויש לחוש לדבריהם » (שו״ע יו״ד ר״י:ב)The Mehaber does not rule like them, but requires that they be reckoned with
ב« מיהו אי לית ליה בקלות עשרה דידעי למקרי יתירנו בשלשה כשאר נדר » (רמ״א יו״ד ר״י:ב)Gloss: failing ten, three suffice, as for any vow
ג« הנודר וחוזר בו תוך כדי דיבור או שמיחו בו אחרים תוך כדי דבור וקבל דבריהם » (שו״ע יו״ד ר״י:ג)Immediate retraction, or acceptance of another’s protest
ג« תוך כדי דיבור לנדרו) (סברת הרב ובזה יישב השגת הראב״ד על הרמב״ם ודלא כב״י » (רמ״א יו״ד ר״י:ג)Gloss: the interval is counted from the vow, not from the protest
ג« לא הוי נדר והוא שאומר כך בפיו אבל אם אמר חזרתי בלבי אינו כלום » (שו״ע יו״ד ר״י:ג)The retraction must leave the mouth; the heart alone is worth nothing

5. The open disputes among the commentators

Three, and they are named.

6. The vocabulary of the siman

TermMeaning
פיו ולבו שויןMouth and heart alike — the central condition of the siman
לבטא בשפתיםThe verse from which the requirement of speech comes
לכל אשר יבטא האדםThe second verse, which lets in thought covered by speech
פת סתםThe wide word that contains the narrow one
דברים שבלב אינם דבריםThe opposing principle; the Ba’h’s position and the close of seif 3 rest on it
על דעת אחריםA vow made on another’s intention: their heart replaces his
תוך כדי דיבורThe interval for retraction, counted from the vow
שלום עליך רביThe measure of that interval, in the Prisha and in Rambam
עשרה דידעי למקריThe ten who know how to read, required by the second view in seif 2
לרווחא דמילתאAs a precaution — the register in which the Rashba places his requirement

7. Rules of thumb, a mnemonic, and standard pitfalls

Two threads, one intersection, one interval. Two threads: speech and intention. One intersection: what the word said and the word thought share. One interval: the time of a greeting, counted from the vow.
