This siman teaches the kavanah of the blessings and the language of prayer: one must have כוונה בכל הברכות, and if not in all, לפחות באבות (at least the first blessing); one enunciates the words with the lips (מחתך הדברים בשפתיו) and lets them be heard בלחש (in a whisper), without letting one's voice be heard; on ר״ה ויום כיפור it is permitted to raise the voice; one may pray בכל לשון, but an individual prays only בלשון הקודש. Our Sages taught (ברכות ל״א.): from the prayer of Chana we learn several laws of prayer — וְחַנָּה הִיא מְדַבֶּרֶת עַל לִבָּהּ, רַק שְׂפָתֶיהָ נָּעוֹת וְקוֹלָהּ לֹא יִשָּׁמֵעַ — « על לבה » : one needs כוונת הלב (intention of the heart) ; « שפתיה נעות » : one must enunciate with the lips ; « וקולה לא ישמע » : one does not let one's voice be heard. We study here at the level of principle; for personal conduct, one refers to the custom and to the ruling of the Rav.

כַּוָּנָה בַּתְּפִלָּה — kavanah in prayer — two requirements are joined: the intention of the heart (כוונת הלב), at least in the first blessing (אבות), without which the tefila is deficient; and the speech of the lips — one does not fulfil the tefila בלב alone, but מחתך הדברים בשפתיו, in a whisper (בלחש). Hence too the exception of ר״ה ויום כיפור and the din of the language: בכל לשון for the צבור, but לשון הקודש for the יחיד.
Seif 1 — כוונה בברכות: kavanah, at least באבות
המתפלל צריך שיכוין בכל הברכות, ואם אינו יכול לכוין בכולם — לפחות יכוין באבות. ואם לא כיון באבות, אף על פי שכיון בכל השאר — יחזור ויתפלל. (הגה: והאידנא אין חוזרין בשביל חסרון כוונה, שאף בחזרה קרוב הוא שלא יכוין, אם כן למה יחזור.)
Seif 1: one who prays must direct his intention in all the blessings; and if he cannot in all of them, at least let him have kavana in אבות (the first blessing). And if he had no kavana באבות, even if he had it in all the rest, he returns and prays again (יחזור ויתפלל). Rama: nowadays we do not repeat for a lack of kavana, for even in repeating it is likely he will have no kavana; if so, why should he return?
Kavana is required in all the blessings; but if one cannot manage it, the first blessing — אבות — is the indispensable minimum, for there one stands before the King. Per the Mechaber, a lack of kavana באבות requires one to repeat; but the Rama rules that nowadays אין חוזרין — we do not repeat — since the second prayer too is liable to be without kavana.
Seif 2 — מחתך בשפתיו בלחש: the speech and the voice
ולא יתפלל בלבו לבד, אלא מחתך הדברים בשפתיו ומשמיע לאזניו בלחש, ולא ישמיע קולו. ואם אינו יכול לכוין בלחש — מותר להגביה קולו, והני מילי בינו לבין עצמו, אבל בצבור אסור דאתי למטרד ציבורא. (הגה: ואם משמיע קולו בביתו כשמתפלל כדי שילמדו ממנו בני ביתו — מותר.)
Seif 2: and one should not pray in the heart alone, but enunciate the words with the lips and let them be heard to one's own ears in a whisper (בלחש), without letting one's voice be heard. And if he cannot have kavana in a whisper — he is permitted to raise his voice; but this only when he is by himself (בינו לבין עצמו), whereas with the congregation it is forbidden, for it disturbs the assembly (דאתי למטרד ציבורא). Rama: and if he lets his voice be heard at home when praying so that the members of his household may learn from him — it is permitted.
One does not pray by thought alone: one must enunciate the words with the lips and let them be heard to oneself, in a whisper. One does not let one's voice be heard by others. Exception: one who cannot concentrate בלחש may raise his voice — but only alone; in public it is forbidden, for it disturbs the assembly.
Seif 3 — ראש השנה ויום כיפור: the exception
יש אומרים שבראש השנה ויום הכיפורים מותר להשמיע קולו בתפלתו אפילו בציבור. (הגה: וכן נוהגין, ומכל מקום יזהרו שלא להגביה קולם יותר מדאי.)
Seif 3: some say that on Rosh Hashanah and Yom Kippur it is permitted to let one's voice be heard in one's prayer, even in public. Rama: and so is the custom; nonetheless one should take care not to raise the voice more than is fitting.
On ראש השנה ויום כיפור, the custom permits letting one's voice be heard even in ציבור — since one is not accustomed to these tefilot, and it aids the kavana. The Rama confirms the custom but cautions: שלא להגביה קולם יותר מדאי — not to raise the voice excessively.
Seif 4 — בכל לשון: the language of prayer
יכול להתפלל בכל לשון שירצה, והני מילי בצבור, אבל ביחיד לא יתפלל אלא בלשון הקודש. ויש אומרים דהני מילי כששואל צרכיו, כגון שהתפלל על חולה או על שום צער שיש לו בביתו, אבל תפלה הקבועה לצבור אפילו יחיד יכול לאומרה בכל לשון. ויש אומרים דאף יחיד כששואל צרכיו יכול לשאול בכל לשון שירצה חוץ מלשון ארמי.
Seif 4: one may pray in any language he wishes; and this applies with a congregation (בצבור), but an individual (יחיד) should pray only in the Holy Language (לשון הקודש). And some say that this applies only when he asks for his own needs (כששואל צרכיו), e.g. he prayed for a sick person or for some affliction in his home; but the fixed liturgy for the congregation, even an individual may recite it in any language. And some say that even an individual asking his needs may request them in any language he wishes, except the Aramaic language (חוץ מלשון ארמי).
One may pray בכל לשון — and this pertains chiefly to the ציבור. An individual, by contrast, prays only בלשון הקודש. Two views qualify this: (a) the restriction concerns only his personal requests (צרכיו), whereas the fixed liturgy may be said בכל לשון even alone; (b) even for his needs the yachid may use any language except Aramaic (לשון ארמי).
The siman in one sentence: one needs כוונה בכל הברכות, and at least באבות (but nowadays אין חוזרין for a lack of kavana); one מחתך הדברים בשפתיו ומשמיע לאזניו בלחש without letting one's voice be heard (alone one may raise it, not בציבור); on ר״ה ויום כיפור one may let it be heard even in public; and one may pray בכל לשון — but the יחיד לא יתפלל אלא בלשון הקודש.
Practical cases
Case 1 — Praying without kavana
Situation: he realizes he recited the Amidah by rote, without kavana, including in אבות. What to do?
Conduct: the ideal is kavana in all the blessings, and at the very least באבות. Per the Mechaber, a lack of kavana באבות would require one to repeat; but the Rama rules that nowadays אין חוזרין — one does not repeat. For the practical application, one follows one's Rav.
Case 2 — Aloud or in a whisper?
Situation: he struggles to concentrate praying בלחש and would like to raise his voice. May he?
Conduct: one enunciates with the lips and lets it be heard to oneself בלחש, without letting one's voice be heard. One who cannot concentrate בלחש may raise his voice — alone, but not in ציבור (it disturbs the assembly). On ר״ה ויום כיפור, the custom permits it even in public, without excess. For the details, one follows one's Rav.
Case 3 — Praying in another language
Situation: someone understands Hebrew poorly and would like to pray in a language he understands. May he?
Conduct: in ציבור, one may pray בכל לשון. An individual, by contrast, prays only בלשון הקודש; per one view the restriction concerns only his personal requests (צרכיו), the fixed liturgy being sayable בכל לשון even alone; per another, the yachid may use any language except Aramaic. For the concrete conduct (and the case of one who does not understand Hebrew), one follows one's Rav.
