5. "Permitted" that means "obligatory" — the heart of the siman
Siman רע"ח hinges on a single word the Mechaber uses: "מותר" — it is permitted to extinguish the light for a cholé in danger. The heart of the siman is to understand that this "permitted" does not read as a mere authorization: it is in reality an obligation, and confusing the two is the central error.
The misunderstanding. "מותר" suggests an option left to free choice: one may extinguish, one may abstain out of Shabbos scrupulousness. Read thus, the siman would become an encouragement to pious hesitation.

The exact reading. The Mishnah Berurah (s.k.1) rules: "פשוט הוא דלא רק מותר אלא חובה לכבות" — it is obvious that it is not only permitted but obligatory to extinguish. The Mechaber writes "מותר" to lift the Shabbos prohibition; but once life is at stake, פִּקּוּחַ נֶפֶשׁ leaves no margin — abstaining becomes a sin.
The borderline case to understand. And in case of doubt about the severity of the state? One might think a doubt weakens the obligation and allows waiting. The opposite is true: בִּסְפֵק פִּקּוּחַ נֶפֶשׁ מְקִילִין — in doubt one acts as if there were danger. Hesitation itself is an offense against life. And the Rambam adds the dimension of the actor: he who is "מהיר ויודע" — quick and competent — must act himself rather than delegate, for delegation introduces delay. The "permitted" thus reverses into a triple imperative: act, act even in doubt, act without delay.
In practice. Prefer the less severe paths — move the cholé, mask the light, גְּרַם כִּבּוּי, or a non-Jew — as long as they entail no delay. But if any alternative would lose time, one extinguishes oneself, directly: it is no longer a choice.
