And it contains only one seif (סעיף אחד): after dozens of simanim that told us which blessing to say over what, this one steps back and asks the most elementary question of all — what makes a blessing a blessing? The answer holds in two words: שם ומלכות, the divine Name and kingship, that is, the words ה׳ אלהינו and מלך העולם. Without them what was uttered is not a defective blessing: it is not a blessing at all. Hence the two lines that follow: one who omitted either of them repeats; and the demand is so strict that even skipping the single word העולם obliges him to repeat — for מלך on its own, without its complement, does not yet say kingship.

Frequently asked questions — Siman רי״ד
Is a blessing valid without the divine Name or without “King of the universe” (Siman 214)?
No. The Rambam joined the Guemara's two requirements into a single law: « וכל ברכה שאין בה הזכרת השם ומלכות אינה ברכה אלא אם כן היתה סמוכה לחברתה » (רמב״ם הלכות ברכות פרק א הלכה ה), and the Beit Yosef draws the ruling from it: « וכ״פ הרי״ף ג״כ וכיון ששניהם מסכימים לדעת אחת הכי נקטינן » (בית יוסף או״ח סימן רי״ד). That is what the Shulchan Aroukh rules: « כל ברכה שאין בה הזכרת שם ומלכות אינה ברכה » (שו״ע או״ח רי״ד:א), and the Mishnah Berurah stresses that it holds for every category of blessing: « היינו בין ברכת הנהנין ובין ברכת המצות בין ברכה ארוכה שיש בה פתיחה וחתימה בין שאין בה אלא פתיחה בלבד » (משנה ברורה, מ״ב רי״ד:א). The source is in the Guemara, where two Amoraim each state one half of the requirement: « אמר רב כל ברכה שאין בה הזכרת השם אינה ברכה » (ברכות מ׳ ע״ב), and « ורבי יוחנן אמר כל ברכה שאין בה מלכות אינה ברכה » (ברכות מ׳ ע״ב).
Must one repeat the blessing if only the word העולם was omitted (Siman 214)?
Yes. The rule comes from Tosafot, quoted by the Beit Yosef: « וכתבו עוד התוס׳ דלר׳ יוחנן אפי׳ לא דילג אלא תיבת העולם לבד צריך לחזור ולברך דמלך לבד אינו מלכות » (בית יוסף או״ח סימן רי״ד). The Biur Halacha adds a nuance of wording: « מ״מ אם אמר המלך בה״א הידיעה מסתברא דאינו חוזר » (ביאור הלכה סימן רי״ד), and the Kaf haChaim attests the accepted practice: « וכן המנהג להורות כדברי מרן ז״ל שאם דילג שם או מלכות או אפילו תיבת העולם שחוזר ומברך » (כף החיים רי״ד:ג). So the Shulchan Aroukh rules: « ואפי׳ לא דילג אלא תיבת העולם לבד צריך לחזור ולברך דמלך לבד אינה מלכות » (שו״ע או״ח רי״ד:א), and the Mishnah Berurah notes a dissent: « ובספר אבן העוזר חולק ע״ז ועיין בבה״ל מה שכתבנו בזה » (משנה ברורה, מ״ב רי״ד:ו). For practice, one follows one's Rav.
Why does the Amidah never say “מלך העולם” (Siman 214)?
The Rishonim give several reasons, gathered by the Beit Yosef. The first is the Rosh's: « וכתב הרא״ש דברכה ראשונה של י״ח כיון דאית בה האל הגדול חשוב כמו מלכות » (בית יוסף או״ח סימן רי״ד) — and the other blessings follow the first, as the Taz writes: « ונרא׳ דאע״ג דבשאר ברכות י״ח לית בהו האל הגדול מ״מ כולהו בתר׳ דראשונה גרירי דהויין ברכ׳ הסמוכה לחברתה » (ט״ז רי״ד:א). The second is the Rokeach's: « אבל בתפלת י״ח שאינה הודאה על שם הנאה ומצוה אלא על תביעת צרכיו של אדם » (בית יוסף או״ח סימן רי״ד). The Ateret Zekenim states the same principle: « ה״מ בברכת הפירות או ברכת המצות או ברכת הראיה כגון הרואה קשת אבל בברכה שהוא שבח והודאה א״צ מלכות » (עטרת זקנים או״ח רי״ד). The Mishnah Berurah points to these answers: « תירץ ב״י בשם הרא״ש דברכה ראשונה כיון דאית בה האל הגדול חשוב כמו מלכות וכולהו בתרה דראשונה גרירא ועי״ש עוד תירוצים » (משנה ברורה, מ״ב רי״ד:ב).
