Can one who sleeps naked under his cloth recite the קריאת שמע according to Siman 74?
According to the Choulhan Aroukh, Orach Chaim 74:1, היה ישן ערום בטליתו — he must interpose his cloth over his heart (לחוץ בטלית על לבו) and only then recite, for לבו רואה את הערוה אסור : when the heart "sees" the nakedness it is forbidden. He therefore needs a חציצה (separation) between the heart and the ervah, and only then reads. The Rama adds: וה״ה אם לבו רואה ערות חבירו אסור — likewise if his heart sees his fellow's nakedness.
May one bless while bathing naked in water according to Siman 74?
According to the Choulhan Aroukh, Orach Chaim 74:2, הרוחץ ערום במים צלולים ורוצה לשתות covers with a garment below his heart (יכסה בבגד ממטה ללבו) so that לבו רואה את הערוה does not apply when he blesses, and this בבגד דוקא, since hands are not a covering. But if the water is murky (מים עכורים) such that his limbs are not visible in it, he is permitted to recite within it, provided it has no foul smell.

According to the Choulhan Aroukh, Orach Chaim 74:6, היתה טליתו חגורה על מתניו — if his garment is wrapped around his loins to cover him from below, מותר לקרות ק״ש even though he is naked above the waist, for the ervah is covered. But להתפלל אסור עד שיכסה לבו — praying (the Amidah) is forbidden until he covers his heart, for in the תפילה he stands as before the King.
