Why a Level 4 dedicated to the Alter Rebbe? The Shulchan Aruch of the Alter Rebbe is not a commentary on the Mechaber — it is an independent, complete Shulchan Aruch, written by the Alter Rebbe. Its singularity: it combines halachah + ta'amei hamitzvos + inner dimension in one work, and it paskens with a unique Talmudic rigor.
For Chabad, the Alter Rebbe is the final posek. This page collects, for Siman 328, the full text of the Rav, his originality, and the Rebbe's words illuminating the topic.

דִּין חוֹלֶה בְּשַׁבָּת וּבוֹ נ״ה סְעִיפִים:

Law of the choleh on Shabbos — containing 55 se'ifim:
One who has only a slight ailment (michush be'alma) without any suspicion of danger and is strengthening himself and walking about like a healthy person — it is forbidden to do for him any refuah on Shabbos, even through a non-Jew (al yedei nochri), except in the manner that will be explained.
And even something that has no trace of melachah, not even of the words of the Sages (mi-divrei soferim), and that carries no prohibition at all for a healthy person — nonetheless, since he intends it as a refuah and the thing is recognizable as intended for healing — it is forbidden, on account of the decree (gezeirah) lest he come to grind ingredients (shechikas samanim) for healing, when it would be permitted to engage in remedies on Shabbos, and he would then be liable for tochen (grinding):
One who has an illness involving danger (choleh she-yesh bo sakanah) — it is a mitzvah to desecrate Shabbos for him, and the one who is zealous (zariz) — behold this is praiseworthy. The one who [first] asks [whether it is permitted] — behold this is a shedder of blood, and the one who is asked — behold this is contemptible, for he should have expounded publicly that it is permitted.
And even a doubt of danger to life (safek nefashos) overrides Shabbos, as it is stated "asher ya'aseh ha-adam vachai bahem" — what does "vachai bahem" teach? That is to say: see that he should live definitely through the performance of mitzvos, and not that he come through them to a doubt of danger.
Nonetheless, even in a case of definite danger — one desecrates only with a remedy that is known to all, or on the word of an expert (mumcheh). And when it is a known remedy, even though it is not known whether this person will be healed by it or not — one desecrates out of doubt:

Any internal injury (makkah shel chalal), namely in the internal organs from the teeth inward, and the teeth themselves included — Shabbos is desecrated for it. This is provided that the teeth, or one of the other internal organs, have been damaged as a result of an injury, or that there is an abscess (bu'a) in them and the like. But mere ailments (michushim be'alma) are not called makkah, and one does not desecrate for them even if he is in great pain.
And if he is in such pain that his whole body becomes sick from it — then it is permitted to desecrate through a non-Jew, even with a full melachah (and if he is not in such great pain — it is permitted to do through a non-Jew anything that is forbidden as a shevus), as will be explained. Therefore one whose tooth aches and who is in such pain that his whole body becomes sick from it (then it is permitted to desecrate through a non-Jew) — tells a non-Jew to extract it.
And even though the Jew presents his tooth to him and helps somewhat in extracting it — this is of no consequence, for one who assists has no substance (mesaye'a ein bo mamash), since even had the Jew not helped him with this help, but merely not hindered him — the non-Jew could have done this thing by himself.
(And according to those who say that a melachah she-einah tzerichah legufah is forbidden only by the words of the Sages — it is permitted to extract it through a non-Jew even if his whole body has not become sick from it, as will be explained):

A makkah shel chalal does not require evaluation: even in an ordinary case where there are no experts present who recognize that it requires desecration, and the choleh too says nothing — one does for him everything that is usually done for him on a weekday, in foods and remedies that are good for the choleh, even though there is no danger at all in withholding that thing, since in any case the illness involves danger. But when it is known and recognized of that illness that it can wait (mamtin) and does not require desecration — it is forbidden to desecrate for it, even though it is a makkah shel chalal, and all the more so if the choleh or the expert says that it is not needed.
And some say that even where desecration is needed, one desecrates through a Jew only for something regarding which there is concern that if it is not done for him he will be endangered; but anything whose withholding involves no danger, even though he nonetheless needs it and it is usually done for him on a weekday — it is done on Shabbos only through non-Jews, like the law of the needs of a choleh who is not in danger.
(And so one should conduct oneself in these lands, where the custom is to be stringent even regarding something whose withholding involves danger, not to do it through a Jew whenever it is possible to do it through a non-Jew without any delay or hindrance at all, as will be explained; if so, something whose withholding involves no danger at all — should be done through a non-Jew, even if there is concern that he will be somewhat slow. But if there is no non-Jew there at all — one may rely on the first opinion. And nonetheless, every ba'al nefesh should be concerned for himself regarding a Torah prohibition):

And likewise one desecrates for any wound caused by iron (barzel), even though it is on the outer flesh and not on the back of the hand or the foot. And likewise for a boil at the anus (shechin habba bephi hataba'as), and for a simta (which is called plonkro in the vernacular), and for one who has a very hot fever, or even one that is not very hot but comes with shivering (simur), namely a fever with chills (which is called shvideren in the language of Ashkenaz). This is provided that the heat and the cold come upon him at one time. But the ordinary fever, whose beginning is cold and whose end is hot — involves no danger:

One who suffers in his eyes, or in one eye in which there is acute pain, or which discharges tears from pain, or which discharges blood, or in which there is mucus that keeps forming a crust, or in which the pain pierces like a needle, or which burns and is hot — one desecrates at the beginning of the illness in all of these, for then there is danger: if the eye should burst, he would die, because the light of the eye is dependent on the heart (me'or ha-ayin taluy ba-lev).
But at the end of the illness in all of these — namely when he is already close to recovering and only a little of the illness remains, burning slightly — one does not do, even through a non-Jew, anything but what is only a shevus, such as applying an eye-salve of ingredients ground the day before (samanim ha-shechukim me-etmol), as will be explained:

Any illness about which the physicians say it is dangerous, even though it is on the outer flesh — one desecrates Shabbos for it. And if one physician says that desecration is needed and another physician says it is not needed — one desecrates, for any doubt of danger to life is treated leniently (kol safek nefashos lehakel).
And even if there is no expert physician there, but one person says that he recognizes this illness and that it seems to him that it requires desecration — one desecrates on his word, for all people are considered somewhat expert, and any doubt of danger to life is treated leniently. (And even if he says that there is room for doubt whether it perhaps requires desecration — one desecrates. This is provided that he says that he recognizes this illness.) Nonetheless, he is not believed to contradict the expert who says that it does not require desecration, even if he says that it certainly requires desecration.
And all this is regarding a Jew. But ordinary non-Jews who are not physicians — they are not considered expert enough to desecrate Shabbos on their word, when there is no Jew there who recognizes this illness:

If the choleh does not want to accept the remedy, because he does not want Shabbos to be desecrated for him — one compels him, for that is foolish piety (chassidus shel shtus):

The choleh says "I need such-and-such remedy," and the physician says "he does not need it" — one listens to the choleh, for "the heart knows the bitterness of its soul" (lev yode'a maras nafsho). And if the physician says that this remedy is harmful to him — one listens to the physician:

A choleh in danger whom they evaluated on Shabbos as needing a known remedy that involves a melachah of Shabbos desecration for eight days — one does not say "let us wait until night, and it will turn out that we desecrate only one Shabbos for him," but they act immediately, even though two Shabbosos will be desecrated for him. For even though he will certainly not die today, since they evaluated that he will survive eight days, nonetheless there is concern that he may die after the eight days if they do not begin the remedy for him immediately.
But where it is possible to do the remedy for him immediately without Shabbos desecration, except that they would need to wait a short while — they should not desecrate in order to do it right away without any delay at all, if it is in a situation where there is no suspicion of danger at all in this short delay; for Shabbos is set aside (dechuyah) in the face of pikuach nefesh and not entirely permitted (hutrah), and wherever it is possible to save him without Shabbos desecration — it is not set aside for him.
And nonetheless, even if there are before us non-Jews or minors who are not obligated in mitzvos — one does not say "it is possible to do it through these, and Shabbos will not be desecrated by Jews obligated in mitzvos"; for since there is no way to save him except by performing a melachah forbidden on Shabbos — Shabbos is set aside for him with respect to the Jews who are obligated to save him, and there is no desecration here at all.
Moreover, even if the Jew comes to be stringent with himself and do it through a non-Jew or a minor, or because he does not want to trouble himself — there is a prohibition in the matter by the words of the Sages (mi-divrei soferim), lest onlookers say that pikuach nefesh was permitted only with difficulty and is not permitted lechatchilah through those obligated in mitzvos, and lest the matter come to this, that when they do not find non-Jews or minors — they will not want to desecrate through the great ones of Israel.
And even with women, who are obligated in mitzvos and with whom there is not this concern — nonetheless one does not hand over to them alone this undertaking and this rescue, to be done by them, for perhaps they will be slow or negligent in it. But they join together with a Jew [man]: one hands the matter to the Jew, and the woman takes part through the Jew, for since the Jew is engaged in it — she too is spurred on through him.
And nonetheless, it is the choicest mitzvah (mitzvah min ha-muvchar) to strive to do everything through Jews great in wisdom, and not through ordinary people and women, so that Shabbos should not be light in their eyes and they come to be lenient where there is no pikuach nefesh, and also in order to teach the practical halachah in public.
And some say that since Shabbos is set aside and not permitted at all, whatever can be done so that there should be no Torah-level desecration — must be done, so that there should be no desecration through a Torah-level melachah. Therefore, if it is possible to do it without delay and without lateness through a shinui (alteration) — one does it through a shinui, for then there is no Torah prohibition. And if it is possible to do it through a non-Jew without any lateness at all — it must be done through a non-Jew by Torah law. And they forbade doing it through a non-Jew only where there is concern that the non-Jew may be slow and it come to delay and hindrance, but not when a Jew stands over him and spurs him on, in a way that there is nothing at all to be concerned about.
And the main [ruling] is like the first opinion. And even so, the custom in these lands is like the latter opinion. But it is good not to conduct oneself so, for there is concern that, seeing now that it is done only through a non-Jew, they will think that there is a prohibition in this through a Jew at all times; and at times no non-Jew will be available, and through this the choleh will be endangered by their waiting for the non-Jew. And in any event, one who wishes to do it through a non-Jew — should make known publicly on that occasion that there is a permission for the Jew himself, except that the non-Jew happens to be at hand here:

If a choleh in danger needs meat — one slaughters for him, and one does not say "let us feed him neveilah, which is a prohibition of a lav, and let us not desecrate Shabbos for him, which is a prohibition punishable by stoning (sekilah)"; for Shabbos has already been given to be set aside for kindling and for cooking. Moreover, with neveilah he transgresses on every single kezayis; and even when he eats less than a kezayis, there is a Torah prohibition in every single eating, which is not so with slaughter, where he commits only one prohibition, even though it is severe. Moreover, perhaps the choleh will be repelled by eating something forbidden, and will hold back and be endangered.
And nonetheless, if the choleh needs to eat right away, and the neveilah is ready immediately while the slaughtered meat would be delayed for him — one feeds him the neveilah.
But if he needs wine to be heated for him — a Jew should fill it and a non-Jew should heat it, according to the custom explained above. And he should watch over him (see Yoreh De'ah, siman 153 and 155) that he not touch it before it boils; and even if he does touch it — it is of no consequence, for even so there will be in it only a prohibition of the words of the Sages, which is not so when the Jew heats it, [where] he would commit a Torah prohibition. And there is no concern at all that the choleh will be repelled by it, since its prohibition is not so severe:

They evaluated him for two dried figs, and they found only two dried figs each attached by a stem of its own, and another three dried figs, all three attached by one stem — one cuts the stem that has three, for even though he takes more figs, nonetheless he lessens the detaching (telishah), which is the essence of the melachah.
But if there were two on one stem and three on one stem — they should cut only the stem that has two, for it is forbidden to increase the quantity of the thing with which the melachah is done, even though he does not increase the effort of the melachah, since it is all done at one time.
And nonetheless, if the matter is urgent — one is not exacting about this, so that it not come to delay and hindrance:

A choleh who because of his illness fell to bed and is not in danger, or who has an ailment from which he is in such pain that his whole body is weakened by it — for then, even though he walks about, he is like one who fell to bed — his needs are done through a non-Jew, even with full Torah-level melachos, such as baking for him or cooking for him if he needs it. And even though all forbidden foods, even those forbidden by the words of the Sages, were not permitted to a choleh who is not in danger — nonetheless they permitted him the prohibition of food cooked by non-Jews (bishulei nochrim) on Shabbos, since it is impossible otherwise, because it is permitted in itself, only that the act of the non-Jew causes it the prohibition.
But Shabbos is not desecrated for him through a Jew with a Torah prohibition, even if there is danger to a limb (sakanas ever), as long as there is no danger to life (sakanas nefesh).
And for a Jew to transgress for him rabbinic prohibitions with his own hands — such as doing for him any refuah that is forbidden because of the decree of grinding ingredients, even though it has in itself no trace of melachah in its performance, not even of the words of the Sages, or even if it does have in itself a rabbinic prohibition in its performance — it is permitted to do it for him without any alteration from the weekday manner, if there is danger to a limb, even though he did not fall to bed and his whole body has not become sick from it.
But if there is no danger to a limb, only that he fell to bed, or is in such pain that his whole body has become sick from it — one does not do for him something forbidden by the words of the Sages through a Jew except with an alteration from the weekday manner (shinui). And through a shinui it is permitted to do even a full melachah, such as one who groans [with heart pain, who is permitted to suckle] with his mouth, as will be explained; for since he alters in its performance — it is forbidden only by the words of the Sages.
And if he needs to eat foods that are recognizable as being for refuah, even though there is in this a prohibition of the words of the Sages because of the decree of grinding ingredients, as will be explained — nonetheless, since it is (something that can never be done through a non-Jew, and he is) sick in his whole body — they permitted it to him. And some forbid. And as to the halachah, in matters of the words of the Sages follow the lenient view:

And if he did not fall to bed, nor is he in such pain that his whole body is weakened (but in any case he has great pain) — it is permitted to do for him through a Jew any shevus performed with a shinui, even if it is a melachah. But he is forbidden to eat foods recognizable as being for refuah, and all the more so other shevusim without a shinui, even though they have no trace of melachah; and likewise to do a full melachah through a non-Jew, for that is a full shevus, without a shinui.
And if he has no great pain nor an illness that encompasses the whole body, but only a partial illness — then it is permitted for him to do any shevus of the words of the Sages through a non-Jew, even without a shinui, as was explained in siman 307, but not through a Jew, even with a shinui. Therefore it is permitted to do for him an external refuah through a non-Jew who places it on him there, since he himself does no act in this. And even though he helps him somewhat — it is of no consequence, as was explained above (but it is forbidden to eat foods recognizable as being for refuah, in the manner that will be explained).
But if he does not have even a partial illness, but only a mere ailment (michush be'alma) — it is forbidden to do for him, even through a non-Jew, anything recognizable as being for refuah, as was explained above:

Anything forbidden to do through a Jew — is forbidden even by the choleh himself. But when the non-Jew does it for him — it is permitted for the choleh to assist him slightly; for example, a non-Jew who applies eye-salve to the eye of a Jew at the end of the illness, when it burns only slightly — he is permitted to open and close the eye so that the salve enters it well, for one who assists has no substance (mesaye'a ein bo mamash), wherever the non-Jew could have done it by himself without the Jew's help, had the Jew merely not hindered him, as was explained above.
And even when the non-Jew does for him a full melachah where it is permitted — the choleh is permitted to assist him in this manner that was explained.
(And even though assisting in a full melachah is a full shevus of the words of the Sages, as will be explained in siman 346, and even when he assists in a shevus it is in any case a shevus de-shevus — nonetheless here, since they permitted the shevus of telling a non-Jew for the need of the choleh — the same applies, that one should permit the shevus of assisting, since it too involves a need of the choleh, for through it the refuah is done better than it would have been done by the non-Jew alone; such as closing the eye so that the salve enters it well, and likewise opening the eye to put the salve into it, or opening the mouth to extract the tooth — even though the non-Jew could have done it by himself, nonetheless the Sages did not require for this that it be imposed on the non-Jew and that the one to whom it pertains to do it, namely the choleh himself, should not do it.
But when the non-Jew does some other forbidden thing, even [forbidden] by the words of the Sages — another Jew should not assist him in doing it, since there is no need at all for his help, for the non-Jew can do it by himself. But if there is some need for his help, in that through it the refuah is done better than what the non-Jew would do alone — it is permitted even for another Jew to assist him slightly).
And if it cannot be done at all without the Jew's help — it is forbidden even for the choleh himself to assist him:

One who let blood and became chilled — it is a danger; one makes a fire (medurah) for him on Shabbos, even in the height of summer (tekufas Tammuz). But for another choleh, even though he is in danger, nonetheless the cold involves no danger for him, since it is possible to warm him with garments; therefore one does not make him a medurah except through non-Jews, unless there are no garments there to warm him with.
And according to those who say that for any choleh in danger one does through a Jew everything that is usually done for him on a weekday, even though there is no danger in withholding that thing — (nonetheless) it is permitted to make him a medurah too through a Jew, in its weekday manner (and it was already explained above how one should conduct oneself in these lands):

But if one washes his mouth with water and afterwards passes it over his eyes, even though fasting saliva is mixed in it — it is permitted, because it is not repulsive to wash with. And if he cannot open his eyes — it is permitted to moisten them even with fasting saliva alone, for they forbade only when he intends refuah, but this is not refuah:

A person may soak soft and clear collyria (kilorin rakkin u-tzelulin) on Friday and apply them to the eye on Shabbos for refuah, and one does not decree on account of shechikas samanim, for since they required him to soak them on Friday — he has a reminder (heker) in this that it is forbidden to engage in remedies on Shabbos. And there is no concern for appearances (maris ha-ayin), because it appears only as washing, for since it is soft and clear — the onlooker thinks it is wine. Provided only that he does not open and close the eye, for then it is evident that he intends refuah. But a thick collyrium — it is forbidden to apply to the eye on Shabbos, because it is recognizable that it is for refuah. But one does not place oil and hot water mixed together upon a wound, nor upon a wad (moch) to place it on it, because it is recognizable that he intends refuah. One places a leaf upon the wound on Shabbos, for it only protects it, except for vine leaves, which are for refuah; and the same applies to all leaves that heal. And if he placed them on it while it was still day and removed them from it after dark, even deliberately — it is permitted to replace it, because they forbade any refuah on account of the decree of grinding ingredients only when he begins it on Shabbos, but not when he already did it for himself while it was still day and replaces it on Shabbos.
But a retiyyah (bandage) that he removed deliberately (be-meizid) from atop the wound, even while it is still in his hand — it is forbidden to replace it, by decree lest he smear (memare'ach) and smooth out its hollows and become liable for memachek (smoothing), as was explained in siman 314.
But if it slipped off the wound by itself after dark — they did not decree in this, and it is permitted to replace it. And even if it fell off him entirely — its law is like one that slipped.
In what case are these words said? When it fell onto a utensil; but if it fell onto the ground, if he comes to replace it — this is like placing it for the first time on Shabbos, and it is forbidden, by decree lest he smear, and on account of the decree of grinding ingredients.
And through a non-Jew it is permitted to place it on the wound even for the first time, if he has pain and a partial illness from it, as was explained above. But it is forbidden to tell a non-Jew to make the bandage on Shabbos, for smearing the bandage is a full melachah, and through a non-Jew it is a full shevus, and they permitted it only if his whole body has become sick or there is danger to a limb, as was explained above.
A person may uncover part of the bandage and wipe the mouth of the wound and return it onto it, and then uncover its other part and wipe the mouth of the wound and return it onto it; for since he does not remove it entirely at one time — it is permitted to replace it. But he should not wipe the bandage, because he is smearing:

One who lances a boil (mefis mursah) on Shabbos, if it is in order to make for it an opening so that air may enter it for refuah — behold he is liable on account of "makeh be-patish," because he fashions an opening for it; and anyone who fashions an opening for any detached thing — is liable on account of repairing a utensil (tikkun keli), which is a derivative of "makeh be-patish," as was explained in siman 314; and what difference is there between repairing a utensil and repairing a wound? Therefore fashioning an opening for a wound too involves "makeh be-patish." And even though one is liable only for an opening made for bringing in and taking out, as was explained there — this opening too is made for bringing in air and taking out pus continually.
And if he lanced it only in order to expel from it the pus that pains him, and not in order to bring air into it for refuah — it is permitted. And even though an opening fit for bringing in and taking out is formed of itself, nonetheless, since he does not need it — this is a melachah she-einah tzerichah legufah.
And even according to those who say that one is liable for a melachah she-einah tzerichah legufah, nonetheless here, since his liability is on account of repairing the wound, then when he does not need this repair and does not intend it, even though it comes about of itself — this is not considered a repair at all, and it is as though he did nothing whatsoever; except that there is room to decree lest he intend [to make] an opening, and because of his pain they did not decree.
And there is one who says that they permitted it only when he lances it for the need of the present pus alone, and is not concerned if it closes up again. But if he wishes it to remain open so, to expel pus continually, even though he does not intend to bring air into it — it is forbidden. And it is good to be concerned for his words, to do it through a non-Jew:

But it is forbidden to scrape (lachuch) a boil (shechin), since it brings out blood, and there is in it chovel. And it is not comparable to expelling pus, for the pus is not attached and absorbed in the flesh, but is as if deposited and placed within a vessel, and when he lances the boil to expel it — he is merely like one who opens a vessel to take out what is inside it; which is not so with blood, which is attached and absorbed in the flesh:

A hole in the wound that has already opened and one comes to widen it on Shabbos, even by any amount — is forbidden. But if it closed up again — there is room for doubt whether it is permitted to open it again on Shabbos as at first, just as it is permitted to open a hole that became blocked in a utensil, as was explained in siman 314. And likewise those who have a hole in the arm, which is called aptura, and the hole became somewhat blocked — there is room for doubt whether it is permitted to place a legume in it so that it opens. And it is permitted to place a bandage on the aptura, for it is like a wound that has healed, on which it is permitted to place a bandage, as was explained (in this siman). And there is one who forbids placing [it] on the aptura (and one should be concerned for his words).
And if he knows that it brings out blood when he wipes it — he shall not wipe it on Shabbos, for it is a "pesik reishei." But it is permitted to change to another cloth, for if he does not change it, it will become foul-smelling, and great is human dignity (kevod ha-beriyos), and he also suffers from it.
In what case are these words said? With this wound that has a hole; but with a wound that has no hole — it is forbidden to change the cloth or the paper, (because) he draws pus from the wound (when he takes them off it), and it is forbidden on account of mefarek, as will be explained. And all the more so he should not place tzukker zelb on a wound that has no hole:

A nail that has detached, and tzitzin, which are like thin strips that have detached from the skin of the finger around the nail: if most of them have detached, since they are close to being severed — there is in them no Torah-level transgression of gozez (shearing), even when he cuts them with an instrument, but it is forbidden by the words of the Sages.
But to remove them by hand, which is not the manner of shearing — is permitted even lechatchilah, if they cause him pain. This is provided that they detached upward, namely that they began to detach toward the nail. And some say that "upward" means toward the body and not toward the nail. And one must be concerned for both interpretations.
And if most of them did not detach and he took them off by hand — he is exempt, but it is forbidden. If he cut them with an instrument — he is liable on account of gozez; and even according to those who say that one is exempt for a melachah she-einah tzerichah legufah — some say that with gozez one is liable according to all, for the reason that will be explained in siman 340:

But it is permitted to put much oil into anigron and swallow it, for then it does not harm the body, and it is not recognizable that he intends refuah but only drinking. One who groans from heart pain (hagonea'ch), whose remedy is to suckle with his mouth from an animal — it is permitted to suckle on Shabbos, because it is mefarek ki-le'achar yad (extracting in an unusual manner), for it is not the way to suckle with one's mouth, but to milk into a vessel and drink from it; therefore suckling involves no Torah prohibition but only one of the words of the Sages, and because of his pain they did not decree.
In what case are these words said? With the pain of one who groans; but if he has only the pain of hunger — it is forbidden to suckle from the animal on Shabbos; but on Yom Tov it is permitted to suckle (if it is impossible for him to milk through a non-Jew, and he also has no food to milk into):

A woman shall not extract milk (takil chalav) from her breasts into a cup or into a pot to nurse her son, because one who milks into a vessel is a full mefarek and is liable on account of dash (threshing), and they permitted a full melachah only where there is danger.
But it is permitted for a woman to squirt some of the milk so that the infant will grasp the nipple and nurse, for since this milk goes to waste — there is in it no Torah-level mefarek, but only one of the words of the Sages, as was explained in siman 320, and for the need of the infant they did not decree.
But it is forbidden to spray some of her milk on one upon whom an evil spirit (ru'ach ra'ah) has blown, because there is in it no danger, nor great pain such that we would permit him a prohibition of the words of the Sages through a Jew, as they permitted for one who groans and the like; see the end of siman 330:

All foods and drinks that are food of the healthy — it is permitted to eat and drink them for refuah, even though they are hard on some things and it is somewhat recognizable that he intends refuah; such as spleen, which is hard on the intestines but is good and beneficial for the teeth, or vetch (karshinin), which is hard on the teeth but is good and healing for the intestines — even so, since it is the way of the healthy to eat them sometimes also not for refuah — it is permitted to eat them also for refuah.
And anything that is not food and drink of the healthy — it is forbidden to eat and drink it for refuah, by decree on account of grinding ingredients. This is when he has a mere ailment and is strengthening himself and walking about like a healthy person, even though he is in great pain. But if his whole body has become sick from it, even though he strengthens himself and walks about, and needless to say if he fell to bed — it was already explained above that there are those who permit.
But if he has no ailment at all, and does not intend refuah at all but some other need, such as one who eats sweet resins and swallows a raw egg in order to make the voice pleasant — it is permitted. But when he intends refuah — it is forbidden, even though he is entirely healthy and has no ailment whatsoever:

One does not make apiktevizin, namely inducing vomiting, not for the need of refuah, even on a weekday, because of the waste of the food in his innards, for as a result he becomes hungry and eats again. And if he suffers from too much food — on a weekday it is permitted even by a drug, and on Shabbos it is forbidden by a drug, because it resembles refuah; but it is permitted to insert his hand into his throat until he vomits:

One who suffers in his intestines — it is permitted to overturn upon them a cup from which hot water has been emptied, even though there is still warmth in it. And likewise it is permitted to raise the "ears" (oznayim), namely the tendons of the ear that sometimes descend so that the jaws become dislocated — one raises them, whether by hand or with a utensil. And likewise to raise the unkali, namely the cartilage opposite the heart that has bent inward. For each of these is not done with drugs, such that we would be concerned about grinding, and he suffers from them:
סעיף מווְכֵן מִי שֶׁנִּשְׁתַּכֵּר, שֶׁרְפוּאָתוֹ לָסוּךְ כַּפּוֹת יָדָיו וְרַגְלָיו בְּשֶׁמֶן וּמֶלַח — מֻתָּר…

And likewise one who became drunk, whose remedy is to anoint the palms of his hands and the soles of his feet with oil and salt — it is permitted to anoint them on Shabbos, for this reason. And one should not derive from this a permission for those who are accustomed in some lands to put into the nose ash of a crushed herb to dispel the drunkenness, for that ash acts as a refuah for other things as well, and the decree of grinding ingredients applies to it:

It is forbidden to rub forcefully on the body (shafshef), even for mere pleasure, as was explained in siman 327, and it goes without saying in order to exert himself and sweat. And likewise it is forbidden to exert oneself by any other means in order to sweat for refuah, by decree lest he grind and drink drugs that bring on sweat.
And it is forbidden to press an infant's belly to extract the excrement, lest one come to give him laxative drugs to drink:

One bathes for refuah in the waters of Gerar, the waters of Chamtan, the waters of Tiveria, and in the good waters of the Great Sea, even though they are salty, because it is customary to bathe in them even not for refuah, and it is not recognizable that he intends refuah.
But not in the bad waters of the Great Sea, nor in soak-waters (mei mishrah), for they are repulsive, and it is not customary to bathe in them not for refuah.
In what case are these words said? When he tarries in them; but if he does not tarry — it is permitted, for he appears only as one cooling himself.
And in a place where it is not customary to bathe in the hot springs of Tiveria except for refuah — it is forbidden to bathe in them on Shabbos for refuah, even if he does not tarry in them:

One does not bathe in laxative waters, nor in mud in which one sinks. And one does not drink a laxative beverage, even if it is food of the healthy and he does not drink it for refuah at all. Because all these are distressing (tza'ar), and it is written: "ve-karasa la-Shabbos oneg" ("and you shall call the Shabbos a delight"):

One places a vessel upon the eye to cool it, provided the vessel is permitted in tiltul. And the same applies to what is done for a person whose eyes ail, that they encircle it with a ring so that the swelling should not spread:

A bone that came out of its place (etzem she-yatza mim-mekomo) — it is forbidden to put it back, because it resembles building (boneh) (and moreover, because all refuah is forbidden). And even to rub it much in cold water — is forbidden, as was explained above.
But a bone that broke — it is put back into its place, for if it is not put back on Shabbos, there is in this danger to that limb, and where there is danger to a limb (sakanas ever) they did not decree:

It is forbidden to place a cloth on a wound from which blood is coming out, because the blood will dye it. And even though he is spoiling it (mekalkel), nonetheless it is forbidden by the words of the Sages; and all the more so with a red cloth, which he improves. And he cannot first remove the blood from the wound, on account of chovel, as was explained above.
Rather, how should he act? He wraps cobwebs (korei akavish) around it and covers with them all the blood and the whole wound, and afterwards wraps a rag (smartut) around it. And some have misgivings and forbid wrapping cobwebs, because they heal; rather he should first wash with water or with wine to remove the blood that is on the wound, and afterwards wrap the rag around it. And it is good to be concerned for their words, but the main [ruling] is like the first opinion:

It is forbidden to suck blood from the wound with one's mouth, on account of chovel; and at a circumcision (milah) they permitted it only because of the danger. And therefore it is forbidden to suck the blood that is between the teeth (and even though it is mefarek ki-le'achar yad, nonetheless it is forbidden by the words of the Sages).
And it is forbidden to place upon a wound something that draws blood and pus, on account of mefarek. And even though it draws of itself, nonetheless, since he places it there in order to draw — this is like wounding and extracting with his hands:

It is forbidden to insert a suppository (petilah) in the rectum in the manner that is customarily done for one who is constipated, unless he inserts it with a shinui, holding it with two of his fingers and placing it gently — because of the removal of hairs (hasharas nimin), as was explained in siman 312.
But it is forbidden to perform a kristir (enema) even with a shinui, even if he prepared it the day before (because of the decree of grinding ingredients), unless for a choleh. And even with a choleh one must take care not to come to a full Torah-level melachah; (and likewise) if it is possible to do it through a non-Jew — he should do it through a non-Jew:
חידושים מיוחדים של הרב
The Rav's special chiddushim on this siman
Originalities that distinguish the Alter Rebbe on Siman 328. Each chiddush follows the structure: classical position → chiddush of the Rav → practical consequence.
Chiddush 1 — 4 structured gradations (328:1-5)
Chiddush I — four structured gradations (328:1-5)
System of the 4 levels of the choleh
The Rav's chiddush: the Alter Rebbe articulates a structured system of 4 gradations enabling classification of each case. A major pedagogical tool.
Practical consequence: test for each case: identify the gradation to apply the right permissions.
Chiddush 2 — pikuach nefesh comes first (328:31-45)
Chiddush II — pikuach nefesh comes first (328:31-45)
Speed and zerizus
The Rav's chiddush: the Alter Rebbe articulates: for pikuach nefesh, not only does one transgress Shabbos — one must do so with speed. No hesitation.
Practical consequence: For emergencies, act without deliberation. Delay for reflection = additional transgression.

The Rebbe's words on the themes of Siman 328
Sicha of the Rebbe — not scheduling surgery close to Shabbat. The Rebbe teaches that it is forbidden to schedule non-urgent surgery for Friday — or even Thursday or Wednesday — since the hospital stay would entail Shabbat desecration: one must not deliberately place oneself in a situation requiring chillul Shabbat — directly bearing on siman שכ״ח (medical care close to and on Shabbat). (Likkutei Sichot vol. 16, p. 518ff.; talk of the second day of Shavuot 5738.)
Beyond the confirmed source above, we attribute to the Rebbe only verifiable primary sources (a letter, sicha, or identifiable pesak, with a link); no further statement of the Rebbe directly tied to this siman could be confirmed against Sefaria or Chabad.org.
