Nefilat apayim — "falling on the face" — is the supplication (תחנון) that follows the Amida. Its root is Moshe Rabbenu, of whom it is said וָאֶתְנַפַּל לִפְנֵי ה׳ (דברים ט), and Yehoshua, of whom it is said וַיִּפֹּל עַל פָּנָיו אַרְצָה לִפְנֵי אֲרוֹן ה׳ (יהושע ז). It is a מנהג (a custom, not a strict obligation), which is why all its rules follow the custom of the place. We study here at the level of principle; for personal conduct, one refers to the custom and to the ruling of the Rav.

A. Nefilat apayim — the ח׳ seifim
נְפִילַת אַפַּיִם — falling on the face — this is the תחנון said after the Amida, immediately after it, without interruption. In earlier times one would truly fall face to the ground; today the custom is to incline the head and cover the face, seated (מיושב). It is a custom (מנהג): hence the side on which one leans, the places and days on which one does not fall, and the measure (the prohibition of פישוט ידים ורגלים on a stone floor).
Séif 1 — אין לדבר: the order and the leaning on the side
אין לדבר בין תפלה לנפילת אפים. כשנופל על פניו נהגו להטות על צד שמאל; הגה: וי״א דיש להטות על צד ימין, והעיקר להטות בשחרית כשיש לו תפילין בשמאלו על צד ימין משום כבוד התפילין, ובערבית או כשאין לו תפילין בשמאלו יטה על צד שמאל; ולאחר שנפל על פניו יגביה ראשו ויתחנן מעט מיושב, וכל מקום ומקום לפי מנהגו, ומנהג פשוט לומר ואנחנו לא נדע כו׳ וחצי קדיש אשרי למנצח; ואפילו בימים שאין אומרים תחנון אומרים למנצח, מלבד בראש חודש וחנוכה ופורים וערב פסח וערב יום כפור וט׳ באב.
Séif 1: one does not speak between the Amida and nefilat apayim. When one falls on the face, the custom is to lean on the left side. Rama: some say the right side; and the main point: at shacharit, when the tefillin are on the left arm, one leans on the right side out of respect for the tefillin (כבוד התפילין); at arvit, or when one has no tefillin on the left arm, one leans on the left side. After falling, one raises the head and supplicates a little, seated, each place per its custom; the widespread custom is to say "ואנחנו לא נדע", a half-kaddish, Ashrei, Lamnatzeach. And even on days without Tachanun one says Lamnatzeach, except on Rosh Chodesh, Chanukah, Purim, erev Pesach, erev Yom Kippur and Tisha BeAv.
Three ideas. (1) One does not interrupt between the Amida and Tachanun, so the supplication remains attached to the prayer. (2) One leans on the side: customarily to the left, but at shacharit — when the arm-tefillin are on the left — one leans to the right משום כבוד התפילין (so as not to press the face on the tefillin). (3) One raises the head and supplicates seated, then says "ואנחנו לא נדע", a half-kaddish, Ashrei and Lamnatzeach.
Séif 2 — נפילת אפים מיושב: seated, and the place of the sefer Torah
נפילת אפים מיושב ולא מעומד; הגה: י״א דאין נפילת אפים אלא במקום שיש ארון וספר תורה בתוכו, אבל בלא זה אומרים תחנה בלא כיסוי פנים וכן נוהגים, וחצר בית הכנסת הפתוח לבית הכנסת, או בשעה שהצבור מתפללין אז אפילו יחיד בביתו אומר תחינה בנפילת אפים.
Séif 2: nefilat apayim is done seated (מיושב) and not standing. Rama: some say there is no nefilat apayim except in a place where there is an aron with a sefer Torah inside; without this, one says the supplication without covering the face, and so is the custom. The courtyard of the beit haknesset that opens onto it [has the same status], or at the hour when the community prays — then even an individual at home says the supplication in nefilat apayim.
Nefilat apayim is done seated. Per the Rama, one covers the face (full nefilat apayim) only where there is a sefer Torah; elsewhere one supplicates without covering the face. The courtyard open to the synagogue has its status; and when the community prays, even an individual at home may fall with them.
Séif 3 — אין נפילת אפים בלילה: no nefilat apayim at night
אין נפילת אפים בלילה, ובלילי אשמורת נוהגים ליפול על פניהם שהוא קרוב ליום.
Séif 3: there is no nefilat apayim at night; but on the nights of the ashmorot (the pre-dawn vigils, for selichot) the custom is to fall on the face, for it is close to day.
Nefilat apayim is a daytime matter; one does not fall on the face at night. Exception: the לילי אשמורת — the vigils when one rises for selichot before dawn — when one does fall, because that time is close to day.
Séif 4 — בית האבל, חתן ומילה: the places without Tachanun
נהגו שלא ליפול על פניהם לא בבית האבל ולא בבית החתן ולא בבית הכנסת ביום מילה ולא כשיש שם חתן; הגה: ודוקא שהמילה או החתן באותו בית הכנסת, אבל אם אין המילה בבית הכנסת אף על פי שהיא בבית הכנסת האחרת אומרים תחנון, וביום המילה שאין אומרים תחנון דוקא שחרית שמלין אז התינוק, אבל במנחה אף על פי שמתפללין אצל התינוק הנימול אומרים תחנון, מה שאין כן בחתן שאין אומרים תחנון כל היום כשמתפללין אצל החתן, ולא מקרי חתן אלא ביום שנכנס לחופה.
Séif 4: the custom is not to fall on the face — neither in the house of a mourner, nor in the house of a chatan, nor in the beit haknesset on the day of a mila, nor when a chatan is present. Rama: and this only when the mila or the chatan is in that beit haknesset; but if the mila is not in the beit haknesset — even in another one — one says Tachanun. On the day of the mila, Tachanun is omitted only at shacharit (when the child is circumcised), but at mincha, though one prays near the child, one says Tachanun; unlike the chatan, near whom one does not say Tachanun the whole day. And one is called "chatan" only on the day he entered the chupa.
Where there is joy, one does not say Tachanun: the house of a mourner, the house or synagogue of a chatan, and the synagogue on the day of a mila. Nuances: for the mila, only at shacharit (the time of the circumcision) — at mincha one says Tachanun; for the chatan, the whole day, but only on the day of the chupa.
Séif 5 — מילה בתענית צבור: a mila on a fast day
אם חל מילה בתענית צבור מתפללין סליחות ואומרים וידוי ואין נופלין על פניהם, ואין אומרים והוא רחום בשחרית אפילו במקום שנוהגים לאומרו בלא זה.
Séif 5: if a mila falls on a public fast day, one says selichot and vidui, but one does not fall on the face, and one does not say "והוא רחום" at shacharit, even where the custom is to say it on other days.
The fast calls for selichot and vidui — which are said — but the joy of the mila sets aside the nefilat apayim and "והוא רחום" (a Tachanun-type supplication) at shacharit. The two dimensions coexist: one supplicates, yet removes the elements proper to Tachanun.
Séif 6 — הימים: the days without Tachanun
נהגו שלא ליפול על פניהם בט״ו באב ולא בט״ו בשבט ולא בראש חודש ולא במנחה שלפניו ולא בחנוכה, וי״א גם במנחה שלפניו (וכן נוהגין); בפורים אין נופלים על פניהם; בל״ג בעומר אין נופלין; בערב יום כפור אין נופלים, וכן ערב ראש השנה אפילו שחרית.
Séif 6: the custom is not to fall on the face on the 15th of Av, the 15th of Shevat, on Rosh Chodesh, nor at the mincha before it, nor on Chanukah; some say also at the mincha before (and so is the custom). On Purim one does not fall; on Lag BaOmer one does not fall; on erev Yom Kippur one does not fall, likewise erev Rosh Hashana, even at shacharit.
Days of joy or semi-festival set aside Tachanun: ט״ו באב, ט״ו בשבט, ר״ח, חנוכה, פורים, ל״ג בעומר, erev יו״כ and erev ר״ה. Often one also omits Tachanun at the mincha of the eve (since the sanctity of the day already "spills over" onto the end of the preceding day).
Séif 7 — התקופות: the periods without Tachanun
ומנהג פשוט שלא ליפול על פניהם בכל חודש ניסן ולא בט׳ באב ולא בין יום הכיפורים לסוכות (ולא מתחלת ראש חודש סיון עד אחר שבועות).
Séif 7: and the widespread custom is not to fall on the face during the whole month of Nissan, nor on Tisha BeAv, nor between Yom Kippur and Sukkot (nor from Rosh Chodesh Sivan until after Shavuot).
Beyond isolated days, whole periods are without Tachanun: all of ניסן (a month of festival and of the nesiim), ט׳ באב (a day called מועד), between Yom Kippur and Sukkot, and from Rosh Chodesh Sivan until after Shavuot. The character of the time or the joy prevails over the supplication.
Séif 8 — אדם חשוב ואבן משכית: the measure of the gesture
אין אדם חשוב רשאי ליפול על פניו כשמתפלל על הצבור אלא אם כן הוא בטוח שיענה כיהושע בן נון; הגה: וכן אסור לכל אדם ליפול על פניו בפישוט ידים ורגלים אפילו אם אין שם אבן משכית, אבל אם נוטה קצת על צידו מותר אם אין שם אבן משכית, וכן יעשו ביום הכיפורים כשנופלין על פניהם אם יציעו שם עשבים כדי להפסיק בין הקרקע, וכן נוהגין.
Séif 8: an important person (אדם חשוב) is not permitted to fall on his face when praying for the community, unless he is confident he will be answered like Yehoshua bin Nun. Rama: likewise it is forbidden for anyone to fall on his face with outstretched hands and legs (פישוט ידים ורגלים), even if there is no stone floor there; but if he leans a little on his side, it is permitted where there is no stone. So they will do on Yom Kippur, if they spread grasses to separate from the ground; and so is the custom.
Two limits. (1) An אדם חשוב praying for the public does not fall on the face in public, unless he is certain to be answered like Yehoshua bin Nun — for if he is not answered it disgraces him. (2) Falling בפישוט ידים ורגלים (hands and legs outstretched) is forbidden (close to the prohibition of prostrating on an אבן משכית, a stone floor); but leaning a little on the side is permitted where there is no stone. On Yom Kippur, one interposes grasses between the body and the ground.
The siman in one sentence: nefilat apayim is the תחנון that follows the Amida without interruption; one leans on the side (to the right at shacharit because of the tefillin) and supplicates seated; one does not fall at night, nor in a place of joy (אבל, חתן, מילה), nor on the many days and periods without Tachanun; and an אדם חשוב does not fall in public unless כיהושע בן נון, nor does anyone fall בפישוט ידים ורגלים on an אבן משכית.
Practical cases
Case 1 — Which side to lean on
Situation: when does one lean to the left and when to the right during nefilat apayim?
Conduct: at shacharit, when wearing the tefillin on the left arm, one leans to the right משום כבוד התפילין (out of respect for the tefillin). At mincha / arvit, or when one has no tefillin on the left arm, one leans to the left. For the practical application and the nuances of custom, one follows one's Rav.
Case 2 — A day on which Tachanun is not said
Situation: how does one know whether Tachanun is said today?
Conduct: Tachanun is not said on days of joy — Rosh Chodesh, Chanukah, Purim, Lag BaOmer, all of Nissan, erev Yom Kippur and erev Rosh Hashana — and often not at the mincha of the eve; nor in a place of joy (בית האבל, חתן, מילה). In doubt (mincha of the eve, the presence of a chatan), one follows the community's custom and one's Rav.
Case 3 — Falling on the face (פישוט ידים ורגלים)
Situation: may one prostrate fully, face and body to the ground, during nefilat apayim?
Conduct: it is forbidden to fall בפישוט ידים ורגלים (hands and legs outstretched), all the more so on an אבן משכית (stone-paved floor). The custom is to lean a little on the side, the face on the arm. On Yom Kippur, when one prostrates, one interposes a separation (grasses, a cloth) between the body and the ground.
