A. גדר כוס של ברכה — the cup of blessing: rinsing and washing
כּוֹס שֶׁל בְּרָכָה — the "cup of blessing" over which Birkat haMazon is recited. The Gemara (Berakhot 51a) lists ten things said of it — עשרה דברים נאמרו בכוס של ברכה: הדחה, שטיפה, חי, מלא, עיטור, עיטוף, נוטלו בשתי ידיו ונותנו בימין, מגביהו מן הקרקע טפח, ונותן עיניו בו (and some add: one sends it as a gift to his household). The Mechaber codifies the main ones in our siman.
Seif 1 — the cup requires rinsing inside and washing outside
כוס של ברכה טעון הדחה מבפנים ושטיפה מבחוץ ואם הוא נקי ואין בו שיורי כוסות אינו צריך:
Seif 1: the cup of blessing requires rinsing on the inside and washing on the outside ; and if it is clean and has no remnants of other cups, it is not necessary.
The cup must be clean: כוס של ברכה טעון הדחה מבפנים ושטיפה מבחוץ (שו״ע או״ח קפ״ג:א) — one rinses it inside (הדחה) and washes it outside (שטיפה). This is the first of the ten things of the Gemara: עשרה דברים נאמרו בכוס של ברכה (Berakhot 51a). But אם הוא נקי ואין בו שיורי כוסות אינו צריך — if the cup is already clean, without remnants, it is not required.
B. היין: חי, מזיגה, מלא ושלם — the wine, cru then mixed, the cup full and whole
Seif 2 — pour the wine cru until birkat haaretz then mix it
יתן היין לתוכו חי עד שמגיע לברכת הארץ ואז מוזגו להודיע שבח הארץ: הגה וי״א דאם היין אינו חזק אינו צריך למוזגו וכן נוהגים באלו הארצות ויוציאנו מן החבית לשם ברכה ונראה דלדידן שאין לנו הרבה יין א״צ רק לשפכו מן הקנקן ששומרים בו היין לשם ברכה וכן נוהגין במדינות אלו וכוס של ברכה ימלאנו שיהא מלא על כל גדותיו:
Seif 2: one pours the wine into it cru (undiluted) until reaching the blessing "of the land" (birkat haaretz), and then mixes it (with water), to make known the praise of the land. Gloss (Rama): some say that if the wine is not strong there is no need to mix it, and such is the custom in these lands ; one draws it from the barrel for the sake of the blessing ; and the cup of blessing, one fills it so that it is full on all its brims.
The wine first cru: יתן היין לתוכו חי עד שמגיע לברכת הארץ ואז מוזגו להודיע שבח הארץ (שו״ע או״ח קפ״ג:ב) — one dilutes it only at birkat haaretz, to highlight the Land of Israel. The Rama notes that where the wine is not strong, וי״א דאם היין אינו חזק אינו צריך למוזגו (רמ״א או״ח קפ״ג:ב), and above all that the cup must be filled: וכוס של ברכה ימלאנו שיהא מלא על כל גדותיו (רמ״א או״ח קפ״ג:ב) — malé, one of the ten things.
Seif 3 — one must seek a whole cup
צריך לחזור אחר כוס שלם:
Seif 3: one must seek out a whole cup (unchipped).
The cup must be whole: צריך לחזור אחר כוס שלם (שו״ע או״ח קפ״ג:ג) — one seeks, for the blessing, a cup without chip or crack, since a chipped cup is not honorable for Birkat haMazon.
C. אחיזת הכוס והגבהתה — how one holds and raises the cup
Seif 4 — received in two hands, held in the right, raised a tefach, the eyes upon it
מקבלו בשתי ידיו וכשמתחיל לברך נוטלו בימינו ולא יסייע בשמאל: הגה והיינו דוקא שלא תגע השמאל בכוס אבל אם נותן השמאל תחת הימין לסייעה מותר ומגביהו מהקרקע טפח אם הוא יושב על גבי קרקע ואם הוא מיסב בשלחן מגביהו מעל השלחן טפח ונותן בו עיניו שלא יסיח דעתו: הגה וע״כ אין לוקחין כוס שפיו צר שקורין גלוק גלא״ז לברך עליו ומשגרו לאשתו שתשתה ממנו:
Seif 4: one receives it in two hands, and when beginning to bless holds it in the right hand without the left assisting. Gloss (Rama): this is only so that the left does not touch the cup, but if one places the left under the right to support it, it is permitted. One raises it a tefach above the ground if seated on the floor, and if reclining at a table, one raises it a tefach above the table ; and one sets the eyes upon it so as not to be distracted. Gloss (Rama): therefore one does not take a narrow-mouthed cup (called "glok glaz") to bless upon. And one sends it to his wife that she may drink from it.
Four gestures: מקבלו בשתי ידיו וכשמתחיל לברך נוטלו בימינו ולא יסייע בשמאל (שו״ע או״ח קפ״ג:ד) — received in two hands then held in the right ; the Rama permits the left in support: אבל אם נותן השמאל תחת הימין לסייעה מותר (רמ״א או״ח קפ״ג:ד). Then ומגביהו מהקרקע טפח — raised a tefach ; ונותן בו עיניו שלא יסיח דעתו — the eyes upon it ; and finally ומשגרו לאשתו שתשתה ממנו — sent to his wife.
Seif 5 — a left-handed mevarech holds it in his right, which is everyone's left
יש מי שאומר שאם המברך אטר אוחז הכוס בימינו שהוא שמאל כל אדם:
Seif 5: one authority says that if the one who blesses is left-handed (iter), he holds the cup in his own right, which is everyone else's left.
The case of the left-handed: יש מי שאומר שאם המברך אטר אוחז הכוס בימינו שהוא שמאל כל אדם (שו״ע או״ח קפ״ג:ה) — the iter holds the cup in his right (his strong hand), even though it is others' left, since for him it is the hand of honor.
D. אין להשיח ואמירה עם המברך — not to speak, say quietly
Seif 6 — once given the cup to bless, one does not speak
משנתנו לו כוס לברך לא ישיח המברך והמסובין אין להם להשיח משהתחיל המברך לא מבעיא בשעה שהוא מברך שצריך לשמוע ולהבין מה שאומר המברך אלא אפילו בין ברכה לברכה אין להם להשיח ואם עברו ושחו בין ברכה לברכה בשעה שהמברך שותק מעט יצאו: הגה אפי׳ אם סח המברך עצמו אבל אם שחו בשעה שהוא מברך לא יצאו:
Seif 6: once he has been given the cup to bless, the mevarech does not speak ; and the diners, once the mevarech has begun, must not speak — not only while he blesses, when one must hear and understand what he says, but even between one blessing and the next. And if they transgressed and spoke between two blessings, while the mevarech was silent for a moment, they fulfilled their duty. Gloss (Rama): even if the mevarech himself spoke. But if they spoke at the moment he blesses, they did not fulfill.
Silence attached to the cup: משנתנו לו כוס לברך לא ישיח המברך (שו״ע או״ח קפ״ג:ו) — neither the mevarech nor the diners speak once the cup is given. Interrupting during the blessing is the most serious: אבל אם שחו בשעה שהוא מברך לא יצאו — if they spoke in the midst of the blessing, they did not fulfill.
Seif 7 — each says quietly with the mevarech ; the cup only to a generous eye
נכון הדבר שכל אחד מהמסובין יאמר בלחש עם המברך כל ברכה וברכה ואפי׳ החתימות: הגה ויקדים לסיים קצת קודם המברך כדי שיענה אמן כדלעיל סימן נ״ט. אין נותנין כוס של ברכה אלא לטוב עין:
Seif 7: it is proper that each of the diners say quietly, with the mevarech, each and every blessing, and even the conclusions (hatimot). Gloss (Rama): he should finish a little before the mevarech so as to answer amen, as above at siman 59. One gives the cup of blessing only to one of a good (generous) eye.
Accompanying quietly: נכון הדבר שכל אחד מהמסובין יאמר בלחש עם המברך כל ברכה וברכה ואפי׳ החתימות (שו״ע או״ח קפ״ג:ז) — each says it quietly with the mevarech. The Rama adds to whom the cup is entrusted: אין נותנין כוס של ברכה אלא לטוב עין (רמ״א או״ח קפ״ג:ז) — to a man of generous eye.
E. שאלה בברכת המזון — asking a question during Birkat haMazon
Seif 8 — its law is like that of the tefila
לענין לשאול בבהמ״ז מפני היראה או מפני הכבוד יש מי שאומר שדינה כתפלה:
Seif 8: regarding greeting (asking / returning a greeting) during Birkat haMazon out of fear or out of honor, one authority says that its law is like that of the tefila (Amida).
Interrupting out of need: לענין לשאול בבהמ״ז מפני היראה או מפני הכבוד יש מי שאומר שדינה כתפלה (שו״ע או״ח קפ״ג:ח) — for greeting out of fear or honor, Birkat haMazon follows the rules of the tefila, where one distinguishes by the gravity of the interruption.
F. לברך מיושב — blessing seated
Seif 9 — one must sit when blessing
צריך לישב בשעה שמברך בין אם היה הולך בביתו כשאכל או עומד או מיסב כשמגיע לברך צריך לישב כדי שיוכל לכוין יותר וגם לא יהא מיסב שהוא דרך גאוה אלא ישב באימה: הגה נ״ל דלאו דוקא המברך אלא ה״ה כל המסובין לא ישבו בקלות ראש אלא באימה מיהו אם לא עשו כן אפי׳ בירך מהלך בדיעבד יצא:
Seif 9: one must sit at the moment of blessing — whether one had been walking about in one's house while eating, or standing, or reclining, when it comes to bless one must sit, so as to concentrate better ; and one will not recline, which is a haughty manner, but will sit with awe. Gloss (Rama): it seems to me this applies not only to the mevarech but likewise to all the diners, who should not sit lightly but with awe ; however if they did not do so, even if one blessed while walking, after the fact he has fulfilled.
Blessing seated: צריך לישב בשעה שמברך (שו״ע או״ח קפ״ג:ט) — even one who was eating standing or walking sits to bless, כדי שיוכל לכוין יותר, and אלא ישב באימה — with awe, not out of pride.
Seif 10 — even the meein shalosh blessing seated
יש אומרים שגם ברכה מעין שלש צריך לאמרה מיושב:
Seif 10: some say that even the "meein shalosh" blessing (al hamichya…) must be said seated.
Even the abridged blessing: יש אומרים שגם ברכה מעין שלש — צריך לאמרה מיושב (שו״ע או״ח קפ״ג:י) — the meein shalosh (said after wine, the fruits of Israel, etc.) is also said seated, according to this view.
Seif 11 — one walking on the road need not sit
אם היה מהלך בדרך ואוכל א״צ לישב ולברך לפי שאין דעתו מיושבת עליו:
Seif 11: if one was walking on the road (traveling) and eating, he need not sit to bless, since his mind is not settled upon him.
The traveler's exception: אם היה מהלך בדרך ואוכל א״צ לישב ולברך לפי שאין דעתו מיושבת עליו (שו״ע או״ח קפ״ג:יא) — on the road the mind is unsettled anyway, so one is not required to sit.
Seif 12 — forbidden to bless while engaged in work
אסור לברך והוא עוסק במלאכתו:
Seif 12: it is forbidden to bless while one is engaged in one's work.
Not blessing while working: אסור לברך והוא עוסק במלאכתו (שו״ע או״ח קפ״ג:יב) — one pauses one's labor in order to bless, so that the blessing is said calmly and with attention.

Case 2 — Holding the cup during the blessing
Situation: how do I hold the cup while I recite Birkat haMazon?
Conduct: one receives it in two hands then holds it in the right — מקבלו בשתי ידיו ונוטלו בימינו (seif 4) ; one raises it a tefach above the table and sets the eyes upon it, שלא יסיח דעתו. A left-handed person holds it in his right (seif 5). In practice, one follows one's Rav.
Case 3 — Blessing seated, in silence
Situation: I was eating standing ; may I say Birkat haMazon standing, and speak in the meantime?
Conduct: no — צריך לישב בשעה שמברך (seif 9), to concentrate better and bless באימה ; and משנתנו לו כוס לברך לא ישיח (seif 6): one does not speak, one says quietly with the mevarech (seif 7). In practice, one follows one's Rav.
