Our Sages taught (מגילה כ״ג:): אין אומרים דבר שבקדושה בפחות מעשרה — one does not say a דבר שבקדושה (such as the קדושה) with fewer than ten. That is why the קדושה is said by the ש״ץ on behalf of the whole congregation: the public is silent, listens and concentrates, then answers with him « קדוש… ». The siman sets out this conduct: what the congregation says and what it listens to, how one stands during the קדושה, and the prohibition of interrupting in its middle. We study here at the level of principle; for personal conduct, one refers to the custom and to the ruling of the Rav.

A. The laws of the קדושה — the ב׳ seifim
קְדֻשָּׁה — the sanctification said with the ש״ץ — the קדושה is a דבר שבקדושה, said only with ten (בעשרה). It is structured as a dialogue: the ש״ץ opens (נקדישך / נקדש), and the congregation answers « קדוש… », « ברוך… », « ימלוך… ». Hence: what the public says and what it listens to (séif 1); and how one stands and does not interrupt during the קדושה (séif 2).
Séif 1 — שותקין ומכוונין: to be silent, to listen, and to answer קדוש
אין הצבור אומרים עם שליח ציבור נקדישך, אלא שותקין ומכוונין למה ששליח ציבור אומר עד שמגיע לקדושה, ואז עונים הצבור קדוש.
Séif 1: the congregation does not say נקדישך with the שליח ציבור, אלא שותקין ומכוונין — but are silent and concentrate their attention on what the שליח ציבור says, עד שמגיע לקדושה — until he reaches the קדושה, ואז עונים הצבור קדוש — and then the congregation answers « קדוש ».
The public does not recite the opening נקדישך with the ש״ץ: it is silent and concentrates (שותק ומכוון). It is the ש״ץ who « opens » on behalf of all; the congregation answers when he reaches the verse — « קדוש קדוש קדוש… » — and likewise at « ברוך » and « ימלוך ». There is thus an order: first the ש״ץ, then the congregation's answer.
Séif 2 — the conduct in the קדושה: the feet, the eyes, and not speaking
טוב לכוין רגליו בשעה שאומר קדושה עם שליח ציבור. הגה: ויש לישא העינים למרום בשעה שאומרים קדושה, וכן מנענעים גופן ונושאין אותן מן הארץ; ואין לדבר באמצע הקדושה; ומי שאמר סדר קדושה ובא לבית הכנסת ומצא צבור עונין קדושה, חוזר ועונה עמהם.
Séif 2: it is good to join one's feet (לכוין רגליו) while saying the קדושה with the שליח ציבור. Rama: one should raise the eyes on high (לישא העינים למרום) when saying the קדושה, and likewise one sways the body and lifts it off the ground; and one does not speak in the middle of the קדושה (ואין לדבר באמצע הקדושה); and one who has already said the order of the קדושה, enters a synagogue and finds a tzibbour answering the קדושה — חוזר ועונה עמהם — repeats and answers with them.
During the קדושה, one stands as before the King: with the feet joined (כיוון רגלים, as in the עמידה — see סימן צ״ה), the eyes raised on high (לישא העינים למרום), and one lifts the body at « קדוש ». One does not speak in the middle of the קדושה (אין להפסיק). And one who has already said the קדושה, if he then finds a tzibbour answering it, answers it again with them (חוזר ועונה עמהם).

Practical cases
Case 1 — Being silent, listening, and answering קדוש
Situation: the ש״ץ begins the קדושה. Should the public say נקדישך with him?
Conduct: no — the congregation does not say נקדישך with the ש״ץ; it is silent and concentrates (שותק ומכוון) on what he says, then answers « קדוש… » when the ש״ץ reaches it, and likewise « ברוך » and « ימלוך ». Many have the custom to say נקדישך too, quietly, with the ש״ץ; for the exact custom of one's community, one follows one's Rav.
Case 2 — Standing during the קדושה
Situation: how should one stand physically while saying the קדושה?
Conduct: it is good to join the feet (לכוין רגליו) as in the עמידה; to raise the eyes on high (לישא העינים למרום); and the custom is to lift the body (נושאין אותן מן הארץ) at each « קדוש ». For the details of the custom (at which words one rises), one follows one's Rav.
Case 3 — Speaking during the קדושה, or entering after one has said it
Situation: may one speak in the middle of the קדושה? And one who has already said the קדושה, then finds another tzibbour saying it?
Conduct: one does not speak in the middle of the קדושה (אין לדבר באמצע הקדושה) — it is a forbidden interruption. And one who has already said the order of the קדושה, enters a synagogue and finds a tzibbour answering the קדושה — חוזר ועונה עמהם: he answers it again with them. For borderline cases, one follows one's Rav.
