1. The Text of the Shulchan Aruch

Siman שי״ח contains 19 seifim of the Mechaber (Rabbi Yosef Karo) which codify the halachos relating to mevashel (cooking) on Shabbos — melocho de'oraisa.

Seif 1 — Status of Food Cooked on Shabbos

דין המבשל בשבת. ובו יט סעיפים:
המבשל בשבת [או שעשה א' משאר מלאכות] [טור] במזיד אסור לו לעולם ולאחרים מותר למ״ש מיד ובשוגג אסור בו ביום גם לאחרים ולערב מותר גם לו מיד [ואם אמר לעכו״ם לעשות מלאכה בשבת ע״ל סי' ש״ז סעיף כ']:
Translation: One who cooks on Shabbos (or performs one of the other melochos): This is the principle of מעשה שבת — the penalty that affects the product of a Shabbos transgression.

Seif 2 — Cooking for a Sick Person

השוחט בשבת לחולה בין שחלה מאתמול בין שחלה היום מותר הבריא לאכול ממנו חי (בשבת) אבל המבשל (או שעשה שאר מלאכה) לחולה אסור (בשבת) לבריא או לחולה שאין בו סכנה דחיישינן שמא ירבה בשבילו: הגה ואפי' בישל ע״י עכו״ם אסור בשבת (תוס' פ״ק דגיטין ור״נ פ' אין מעמידין) ואם קצץ פירות מן המחובר לחולה בשבת אפי' היה חולה מבעוד יום אסור לבריא בשבת משום שגדל והולך בשבת ויש בו משום מוקצה (ב״י בשם אורחות חיים):
Translation: One who shechts an animal on Shabbos for a choleh (one in danger) — whether he became sick yesterday or today — a healthy person may eat from it raw on Shabbos (the shechita was permitted for the choleh; eating raw adds no melocho). But what was cooked for the choleh — assur on Shabbos to the healthy person or to a non-dangerously ill person — for we are concerned that more may be added on his behalf (shema yarbeh bishvilo). Rama's gloss: Even cooked by a non-Jew — assur on Shabbos. And fruits picked for the choleh remain assur to the healthy person (growth + muktzah).

Seif 3 — Toldos ha-ur and toldos chamah

כשם שאסור לבשל באור כך אסור לבשל בתולדות האור כגון ליתן ביצה בצד קדרה או לשברה על סודר שהוחם באור כדי שתצלה ואפילו בתולדות חמה כגון בסודר שהוחם בחמה אסור גזרה אטו תולדות האור וכן אסור להטמינה בחול או באבק דרכים שהוחמו מכח חמה אבל בחמה עצמה כגון ליתן ביצה בחמה או ליתן מים בחמה כדי שיוחמו מותר:
Translation: Just as it is forbidden to cook with fire, so it is forbidden to cook with the derivatives of fire (toldos ha-ur) — for instance to place an egg beside a pot, or to break it onto a cloth heated by fire so that it should roast. And even with the derivatives of the sun (toldos chamah) — such as a cloth heated by the sun — it is forbidden, by decree on account of the derivatives of fire. Likewise it is forbidden to bury it in sand or in the dust of the roads heated by the sun. But in the sun itself — to place an egg in the sun, or to put water in the sun so that it should warm — is permitted.

Seif 4 — Bishul achar bishul — cooking what is already cooked

תבשיל שנתבשל כל צרכו יש בו משום בישול אם נצטנן: הגה וי״א דוקא אם מצטמק ויפה לו [רבי ירוחם ח״ג] ואם לא נתבשל כל צרכו ואפי' נתבשל כמאכל בן דרוסאי שייך בו בישול אפי' בעודו רותח וה״מ שיש בו בישול אחר בישול בתבשיל שיש בו מרק אבל דבר שנתבשל כבר והוא יבש מותר לשרותו בחמין בשבת ואם הוא דבר יבש שלא נתבשל מלפני השבת אין שורין אותו בחמין בשבת אבל מדיחים אותו בחמין בשבת חוץ מן המליח הישן ומן הדג שנקרא קולייס האספנין שאינם צריכים בישול אלא מעט והדחתן היא גמר מלאכתן: הגה וה״ה כל דבר קשה שאינו ראוי לאכול כלל בלא שרייה דאסור לשרותו בשבת דהוי גמר מלאכה [הגהות מרדכי]:
Translation: A dish fully cooked is still subject to cooking if it has cooled. Rama's gloss: and some say: specifically if it reduces and is thereby improved (Rabbenu Yerucham). And if it was not fully cooked — even if cooked to the degree of ma'achal ben derusai — cooking applies to it even while it is still boiling. All this, that there is cooking after cooking, applies to a dish with gravy; but something already cooked and dry may be steeped in hot water on Shabbos. And if it is a dry thing not cooked before Shabbos, one does not steep it in hot water on Shabbos — but one may rinse it in hot water on Shabbos, except for old salted fish and the fish called kulyas ha-ispanin, which need only a little cooking and whose rinsing is the completion of their preparation. Rama's gloss: and the same applies to anything hard that is wholly unfit to eat without steeping: it is forbidden to steep it on Shabbos, since that is the completion of the work (Hagahos Mordechai).

Seif 5 — Bread in a kli sheni

יש מי שאומר דדבר שנאפה או נצלה אם בשלו אחר כך במשקה יש בו משום בישול ואסור ליתן פת [אפי'] בכלי שני שהיד סולדת בו ויש מתירין: הגה בכלי שני ויש מקילין אפי' בכלי ראשון ונהגו ליזהר לכתחילה שלא ליתן פת אפילו בכלי שני כל זמן שהיד סולדת בו:
Translation: One authority says that something baked or roasted, if afterwards cooked in a liquid, is subject to cooking — and that it is forbidden to place bread even in a kli sheni so long as the hand recoils from it (yad soledes bo). And some permit it. Rama's gloss: in a kli sheni; and some are lenient even in a kli rishon. But the custom is to be careful from the outset not to place bread even in a kli sheni so long as the hand recoils from it.

Seif 6 — Placing a vessel on a buried pot

כלי שיש בו דבר חם שהיד סולדת בו מותר להניחו בשבת על גבי קדירה הטמונה כדי שישמור חומו ולא יצטנן ויכול לטוח פיו בבצק אם יש לו בצק שנלוש מאתמול אבל אין מניחין כלי שיש בו דבר שאינו חם כל כך על גבי קדירה שהיא חמה כל כך שהעליון יכול להתחמם מחומה עד שתהא היד סולדת בו:
Translation: A vessel containing something hot enough that the hand recoils from it may be placed on Shabbos upon a buried pot so as to keep its heat and not cool. And he may seal its mouth with dough, if he has dough kneaded from the day before. But one may not place a vessel containing something not so hot upon a pot so hot that the upper one could be heated by its warmth until the hand recoils from it.

Seif 7 — And if the lower vessel is on the fire?

יש מפרשים דהא דשרי להניח כלי שיש בו דבר חם על גבי קדירה הטמונה אפי' כלי התחתון על האש שרי ויש מפרשים שאם כלי התחתון על האש לעולם אסור:
Translation: Some explain that the permission to place a vessel containing something hot on a buried pot applies even where the lower vessel is on the fire. And some explain that if the lower vessel is on the fire, it is always forbidden.

Seif 8 — Placing something cold on a kettle over the fire

להניח דבר קר שנתבשל כל צרכו על גבי מיחם שעל האש י״א שדינו כמניחו כנגד המדורה וכל דבר שמותר להניחו כנגד המדורה במקום שהיד סולדת בו כגון שיבש מותר להניחו ע״ג מיחם שעל גבי האש וי״א דהוי כמניח על גבי כירה לכתחלה ואסור אפי' אם נתבשל כל צרכו ואפי' אם מצטמק ורע לו אפי' אם נותנו שם לשמור חומו וראשון נראה עיקר ומ״מ אם הוא תבשיל שיש בו רוטב ומצטמק ויפה לו אסור לדברי הכל:
Translation: To place a cold, fully cooked item upon a kettle standing on the fire: some say its law is that of one who places it facing the hearth — and whatever may be placed facing the hearth where the hand recoils, such as something dry, may be placed upon a kettle standing on the fire. And some say it is like placing it on a stove from the outset, and it is forbidden — even if fully cooked, even if it reduces to its detriment, and even if he puts it there to keep its heat. The first view appears to be the main one. In any case, if it is a dish with gravy that reduces and is thereby improved, it is forbidden according to all.

Seif 9 — Kli rishon — spices and salt

כלי ראשון (פי' הכלי שמשתמש בו על האש) אפי' לאחר שהעבירוהו מעל האש מבשל כל זמן שהיד סולדת בו לפיכך אסור ליתן לתוכו תבלין אבל מלח מותר ליתן לתוכו כיון שהעבירו מעל האש דצריכא מילחא בישולא כבישרא דתורא ויש מי שאוסר לתת לתוך כלי זה בשר מליח אפי' הוא של שור: הגה ונ״ל דבלא מליח נמי אסור אלא דנקט מליח דבלאו הכי אסור משום דם שבו ויש אוסרים לתת מלח אפי' בכלי שני כל זמן שהיד סולדת בו והמחמיר תבא עליו ברכה: (תוס' ומרדכי פרק כירה) ואם עבר ונתן מלח אפי' בכלי ראשון אפי' הוא על האש שעבד איסורא מותר המאכל דהמלח בטל על גבי המאכל (ב״י בשם שבולי הלקט):
Translation: A kli rishon (explanation: the vessel used on the fire) — even after it has been removed from the fire — cooks so long as the hand recoils from it. Therefore it is forbidden to put spices into it. But salt may be put into it once removed from the fire, since salt requires as much cooking as ox meat. And one authority forbids putting salted meat into such a vessel, even beef. Rama's gloss: and it seems to me that without salt too it is forbidden — he mentioned “salted” only because it is in any case forbidden on account of the blood in it. And some forbid putting salt even into a kli sheni so long as the hand recoils from it; may blessing come upon one who is stringent (Tosfos and Mordechai). And if he transgressed and put in salt, even into a kli rishon, even on the fire — though he committed a transgression — the food remains permitted, since the salt is nullified in the food (Beis Yosef in the name of the Shibolei ha-Leket).

Translation: It is forbidden to put spices in a bowl and pour over them from a kli rishon.

Seif 11 — The bath-tub

אמבטי [פי' כלי שרוחץ בו] של מרחץ שהיא מלאה מים חמין [אע״פ שהוא כלי שני] [תוס' והרא״ש וטור] אין נותנין לה מים צונן שהרי מחממין הרבה אבל נותן הוא ממים חמין שבזה האמבטי לתוך אמבטי אחר של צונן:
Translation: A bath-house ambati [explanation: the vessel in which one washes] full of hot water [although it is a kli sheni] (Tosfos, Rosh and Tur) — one may not put cold water into it, for it would heat up considerably. But he may pour from the hot water of this tub into another tub of cold water.

Seif 12 — Taking the chill off cold water

מיחם שפינה ממנו מים חמין מותר ליתן לתוכו מים צונן [מרובים] כדי להפשירן ומותר לצוק מים חמין לתוך מים צונן או צונן לתוך חמין והוא שלא יהיו בכלי ראשון מפני שמחממין הרבה: הגה ואם המים מרובים כל כך שאי אפשר שיתבשלו רק שיפיגו צינתן אפי' בכלי ראשון שרי רק שלא יהיה על האש [טור]:
Translation: A kettle from which the hot water has been emptied: it is permitted to put cold water into it [in quantity] in order to take the chill off. And it is permitted to pour hot water into cold, or cold into hot — provided they are not in a kli rishon, since they would heat up considerably. Rama's gloss: and if the water is so plentiful that it cannot cook but only lose its chill, it is permitted even in a kli rishon — provided it is not on the fire (Tur).

Seif 13 — The jug in a kli sheni

מותר ליתן קיתון של מים או של שאר משקים בכלי שני שיש בו מים חמין אבל בכלי ראשון אסור:
Translation: It is permitted to place a jug of water or of other liquids in a kli sheni containing hot water; but in a kli rishon it is forbidden.

Seif 14 — Before the fire — the measure of yad soledes

מותר ליתן קיתון של מים או של שאר משקים כנגד האש להפיג צינתן ובלבד שיתנם רחוק מהאש בענין שאינו יכול להתחמם באותו מקום עד שתהא היד סולדת [פירוש מתחממת ונכוית] בו דהיינו שכריסו של תינוק נכוית בו אבל אסור לקרבו אל האש למקום שיכול להתחמם שתהא היד סולדת בו ואפי' להניח בו שעה קטנה שתפיג צנתן אסור כיון שיכול להתבשל שם: הגה וה״ה בפירות או שאר דברים הנאכלין כמות שהן חיין (מרדכי פ' כירה וע״ל סי' רנ״ד):
Translation: It is permitted to place a jug of water or of other liquids before the fire to take the chill off — provided he sets it far from the fire, such that it cannot heat at that spot until the hand recoils from it [explanation: grows hot and is scalded], that is, to the point where an infant's belly would be scalded by it. But it is forbidden to bring it near the fire to a place where it can heat until the hand recoils — and even to leave it there a short while to take the chill off is forbidden, since it could cook there. Rama's gloss: and the same applies to fruit or other things eaten raw (Mordechai — and see above siman 254).

Seif 15 — Dry food before the hearth

דבר שנתבשל כל צרכו והוא יבש שאין בו מרק מותר להניחו כנגד המדורה אפילו במקום שהיד סולדת בו: הגה ואפי' נצטנן כבר אבל אם הוא רותח אפי' בדבר שיש בו מרק מותר ויש מקילין לומר דכל שאינו נותנו על גבי האש או הכירה ממש רק סמוך לו אפילו נצטנן מותר [המגיד פכ״ב] ונהגו להקל בזה אם לא נצטנן לגמרי וכמו שכתבתי לעיל סי' רנ״ג:
Translation: Something fully cooked and dry, with no gravy, may be placed facing the hearth, even where the hand recoils. Rama's gloss: and even if it has already cooled. But if it is boiling hot, it is permitted even for something with gravy. And some are lenient to say that so long as he does not set it on the fire or the stove itself but only beside it, it is permitted even if it has cooled (Maggid); and the custom is to be lenient in this if it has not cooled entirely, as I wrote above in siman 253.

Seif 16 — The empanada and the fat that melts again

מותר ליתן אינפאנדה כנגד האש במקום שהיד סולדת ואף ע״פ שהשומן שבה שנקרש חוזר ונימוח: הגה וכל שכן קדירה שיש בו רוטב שנקרש שכשהשומן נימוח אינו בעין דשרי (ר' ירוחם ח״ג) ויש מחמירין (ר״ן פרק במה טומנין) ונהגו להחמיר מיהו במקום צורך יש לסמוך אסברא ראשונה:
Translation: It is permitted to place an empanada before the fire where the hand recoils — even though the congealed fat in it melts again. Rama's gloss: and all the more so a pot containing congealed gravy, where the fat on melting is not visibly distinct — which is permitted (Rabbenu Yerucham). And some are stringent (Ran), and the custom is to be stringent; nevertheless, where there is need, one may rely on the first view.

Seif 17 — Placing cold on the kettle

אסור ליתן צונן (על המיחם) אפי' להפשיר כל שהמיחם חם כל כך שאלו היה מניחו שם הרבה היה בא לידי בישול דהיינו שיהיה היד סולדת בו שדין מניח על גבי מיחם כדין מניח כנגד המדורה ואם אינו חם כל כך מותר:
Translation: It is forbidden to place something cold (on the kettle), even merely to warm it, so long as the kettle is hot enough that, were he to leave it there a long while, it would come to cook — that is, that the hand would recoil from it; for the law of one who places on a kettle is that of one who places facing the hearth. And if it is not that hot, it is permitted.

Seif 18 — Meigis — stirring the pot

האלפס והקדירה שהעבירן רותחין מעל גבי האור אם לא נתבשל כל צרכו אין מוציאין בכף מהם שנמצא מגיס ואיכא משום מבשל ואם נתבשל כל צרכו מותר אבל צמר ליורה אע״פ שקלט העין אסור להגיס בו [פי' לנענע אותו בכף]: הגה ולכתחילה יש ליזהר אף בקדירה בכל ענין: [פסקי מהרי״ו]:
Translation: The pan and the pot removed boiling from the fire: if the food is not fully cooked, one does not take from them with a ladle, for that would be meigis (stirring), and there would be bishul. And if it is fully cooked, it is permitted. But wool in the dyer's vat, although it has already taken the colour, may not be stirred [explanation: moved about with a ladle]. Rama's gloss: and from the outset one should be careful even with a pot, in every case (Piskei Mahariv).

Translation: It is forbidden to smear oil and garlic on the roast while it is still facing the hearth — even if the roast was roasted while it was still day, since in any case the garlic and oil would cook.

2. The General Context — the Melocho of Bishul

What does this siman discuss?

Siman שי״ח deals with the melocho of בישול (bishul — cooking), one of the 39 melochos — and one of the most present in daily life. It is the key siman of the Shabbos kitchen. It is extremely dense: 19 seifim and 118 entries of the Mishnah Berurah.

The melocho of bishul: transforming food through heat. Source: In the Mishkan, herbs were cooked for dyes. Cooking in water, roasting, frying, melting metal, hardening an object with fire — all are bishul. Cooking = av melocho, mid'oraisa.
The 3 concepts that govern the entire siman:

Place in Hilchos Shabbos

Siman שי״ח is the heart of the Shabbos kitchen. It follows simanim 253-259 (shehiya, hatmana) and precedes 319 (borer — selecting). Without this siman, one cannot properly warm or serve on Shabbos.

3. Key Halachic Concepts

1. יד סולדת בו (yad soledes bo) — threshold of bishul. Temperature at which "the hand recoils" from pain (≈ 45°C / 113°F by lenient opinions, ≈ 71°C / 160°F by strict). Below → no bishul mid'oraisa.
2. אין בישול אחר בישול — no bishul after bishul. A solid food entirely cooked is no longer "re-cooked" → it may be warmed. So for what is dry: אבל דבר שנתבשל כבר והוא יבש מותר לשרותו בחמין בשבת But for a dish that has cooled, the Mechaber writes the opposite (שי״ח:ד): תבשיל שנתבשל כל צרכו יש בו משום בישול אם נצטנן — and the Rama's gloss adds a restriction: וי״א דוקא אם מצטמק ויפה לו The practical ruling is to be verified with the Rav.
3. כלי ראשון / כלי שני — first / second vessel. The kli rishon (pot, even removed from fire) cooks as long as it is yad soledes. The kli sheni (into which content was transferred) does not cook — except for a davar kal habishul (food that cooks easily).
4. עירוי (iruy) — pouring from a kli rishon. The stream of boiling water poured cooks the "outer surface" of what it touches (kdei klipa). Intermediate status between kli rishon and kli sheni.
5. תולדות האור and תולדות חמה — derivatives of fire / sun. Cooking by heat derived from fire = assur. Cooking by sun directly = permitted; but by derivatives of the sun (sand heated by sun) = assur (gezeirah, lest one confuse with derivatives of fire).
